ExecutiveSummary

Inthe1970s,theincreasinguseofpesticideselevatedcitizens’concernsregardingthepossibleadverseeffectsofthesechemicalsonpeople,animals,andtheenvironment.Inresponsetotheseconcerns,theOregonStateLegislatureestablishedin1979thePesticideAnalyticalandResponseCenter(PARC)(PerOregonRevisedStatute634.550).Inadditiontoactingasacentralizedlocationtoreceivepesticide-relatedincidentinformation,PARCalsofunctionstoassistwithinteragencycommunicationandcoordinationwheninvestigatingpesticideincidents(SeeFigure1nextpage).CoordinationandcommunicationisprimarilybetweenPARCmemberagencyBoardmembers(SeeAppendix1forPARCBoardcontacts).

PARCdoesnotconductinvestigations.However,PARCmemberagenciesmayeitherconductaninvestigationorconductactivitiesinsupportofaninvestigation,dependinguponeachindividualagency’sregulatoryauthority.ThePARCmemberagenciesthatmayconductpesticideinvestigationsare:

IntroductionOregonRevisedStatute634.550establishedthePesticideAnalyticalandResponseCenter(PARC)in1979.PARCisopenMondaythroughFriday,8a.m.to5p.m.toreceivepesticideincidentinformationandisstaffedbythePARCCoordinatorandCitizenAdvocateandLiaison.PARCstaffareresponsibleforreceivingpesticide-relatedincidentinformationandmakingtheappropriatereferralstoPARCmemberagencies.IfaPARCmemberagencyconductsaninvestigation,thesummaryreportsaresubmittedtoPARC.PARC,alongwiththeOregonHealthAuthority’s(OHA’s)PesticideExposureSafetyandTracking(PEST)Coordinator,assigncontributingfactors(SeeAppendix3)toPARCincidents.Foroccupationalincidents,theassigningofcontributingfactorsisperformedinconjunctionwiththeOregonOccupationalSafetyandHealthAdministration’s(OSHA)PesticideCoordinator.SelectdraftCFsheetsarepresentedtotheBoardwhothenreachesaconsensusonthefactorsthatcontributedtothePARCincident.ThePARCCoordinatordraftsabienniallegislativereportanddistributesittotheBoardforreviewbeforeitisfinalized.ThestandardoperatingproceduresthatfollowgovernthedailyoperationsofPARC.

I. ReceivingPesticideIncidentInformation

a. Telephonecalls-PARCtracksincomingtelephonecalls.Whennecessary,outgoingtelephonecallsaretrackedaswell.

i. PARCreceivespesticide-relatedincidentinformationthroughPARC’stelephonenumber.Voicemailmessageswillbereturnedwithinonebusinessday.PARCstaffisresponsibleforobtainingtheinitialinformationnecessarytomaketheappropriatereferralPARCmemberagencies.Thisinformationincludes,butisnotlimitedto:

1. Name2. Telephonenumber3. Address,includingzipcode4. Incidentdetails,includingtime,date,andlocationofexposure,

productinformation,andtypeofincident.Typesofincidentsmayincludeoccupational;forestry-related;affectingsoil,water,orair;affectingfish/wildlifeortheirhabitat.

5. Detailsconcerninganimalsexposed(e.g.number,type,andifseenbyaveterinarian,theveterinarian’scontactinformation

ii. Followingthereceiptofpesticideincidentinformation,informthecomplainantofPARC’sroleandtheroleoftheagencyinwhichtheircomplaintisbeingreferred.

iii. WhenthePARCCoordinatorisunavailable,theprimarydesigneewillserveasPARCCoordinator.

b. Email-PARCalsoreceivespesticide-relatedincidentinformationviaemail.ALLemailreferralstoPARCshouldusethefollowingemailaddress:PARC@ODA.state.or.us.ThisemailaddressisroutedtoseveralotherODAstaffinadditiontothePARCCoordinator,sothatappropriateactionswillstillbetakenintheeventthatthePARCCoordinatorisunavailable.(e.g.referralstoPARCmemberagencies)

II. SourcesofPesticideIncidentInformationa. PARCreceivesinformationregardingpesticide-relatedincidentsaffecting

people,animals,ortheenvironment,throughavarietyofsourcesincluding:i. ODApesticideinvestigatorsii. ODA’sonlinepesticidecomplaintformiii. DEQ’scomplaintline/onlinecomplaintformiv. OregonEmergencyResponseSystem(OERS)

1. OERS-ThePARCCoordinatorreceivesdailyreportsfromtheOregonEmergencyResponseSystem(OERS)viaemail,whichisresponsibleforcoordinatingandmanagingstateresourcesinresponsetonaturalandtechnologicalemergenciesandcivilunrestinvolvingmulti-jurisdictionalcooperationbetweenalllevelsofgovernmentandtheprivatesector.http://www.oregon.gov/omd/oem/pages/tech_resp/oers.aspx

2. Often,OERSreportsdonotcontainspecificinformation,i.e.EPAregistrationnumbers.PARCfollowsupwiththoselistedascontacts,formoreinformation.

3. OERSidentifieschemicalsusing‘UN’numbers;thesearefourdigitnumbersthatidentifyhazardoussubstances,includingpesticides.UNnumbersandthesubstancesthattheyidentifycanbefoundatWikipedia’spagetitled“ListofUNnumbers.”

4. Additionally,thespecificidentityoftheproductmaybeobtainedbycontactingthemaincontactpersonlistedontheOERSreport.

5. WhenareferralismadetoaPARCmemberagencybasedupontheOERSreport,PARCprovidesthespecificOERSreporttotheagencyreceivingthereferral.

6. DEQfollowsuponpesticidespillsreportedtoOERS.PARCrequeststhatDEQprovidethesereportstoPARCviaemail,referringtotheOERSreportnumberinthesubjectline.

7. WhentheDEQreportisreceived,PARCprintsahardcopy,placesitinahardcopyfolderwiththeoriginalOERSreport,andfilesitappropriately.

8. ODA’sPesticidesprogramtrackspesticidespills.ThePARCCoordinatorrequeststhespecificOERSreportsrelatedtopesticideincidents(identifiedbytheyear,thenthespecificnumber,i.e.2014-0967)andsendsthereporttoODA’sLeadPesticideInvestigator/CaseReviewer.

III. MakingPARCReferralstoPARCMemberAgencies

a. Referralcriteria–PARCusesthecriteriabelowtodeterminewhichPARCmemberagencywillreceiveareferralfollowingPARCreceivingpesticide-relatedinformation.EachPARCboardmemberortheirdesigneearecc’donPARCreferrals.

i. ODA-ODAPesticideprogramshallbenotifiedassoonaspossiblewheneveraviolationofOregon’sPesticideControlLaw(ORS634andOAR603)issuspected.Thiswouldincludeanysuspectedmisuse,drift,orotherwisefaulty,careless,ornegligentactsrelatedtopesticideuse,storage,distribution,ordisposal.

ii. ODF-ODFisnotifiedanytimethereisareportorallegationofdamagetonaturalresources,humanhealth,orhumanpropertyasaresultofaforestpesticideapplication,spill,orotherrelatedactivity.Iftheapplicationisongoing,immediatenotificationisrequested.Ifitisfinished,ODFrequestsnotificationassoonasispractical.

iii. OR-OSHA-OregonOSHAisnotifiedanytimeapesticide-relatedincidentorunreasonableexposureriskisoccupationalinnature.

iv. DEQ–DEQisnotifiedwhenthereisapossibleviolationofthegeneralpesticidepermit(2300permit),apossiblemisuseofapesticidethathasimpactedadrinkingwatersourceorgroundwater,orimproperdisposalofsolidwastecontainingpesticides.DEQisnotifiedthroughtheagencycomplainthotline(888-997-7888)ortheonlinecomplaintformathttp://www.deq.state.or.us/complaints/whenstateagenciesconductanon-urgentpesticideuseinvestigationthatinvolvespotentialimpacttosurfacewater,sourcewaterforpublicdrinkingwatersystems,orotherenvironmentalimpact.ReferralstoDEQaresenttocomplaintintake@deq.state.or.us

v. OHA-ThePesticideExposureSafetyandTracking(PEST)ProgramtheOregonHealthAuthority(PublicHealthDivision),isnotifiedwithinonebusinessdayofreportsofsuspectedpesticide-relatedhumanhealthsymptoms.

vi. ODFW-ODFWisnotifiedwithinonebusinessdayofanysuspectedpesticide-relatedpoisoningoffish,wildlife,orimpactstohabitat.

vii. OSFM-OSFMisnotifiedviaemailregardingpesticide-relatedincidentsatfixedlocationswherepesticidesarestored.

viii. OPC-OPCisnotifiedimmediatelyat800-222-1222ofanypesticideexposuresforwhichassistanceisneededinacuteclinicalmanagementofthepatient.PARCwillprovideOPC’snumberto

individualswhorequireimmediatemedicalattentionassociatedwithpesticideexposure.Anameandnumberwillberequestedifconvenient.

ix. ODOT-ODOTisnotifiedofanypesticide-relatedincidentthatinvolvesOregon’sstatehighwaysystem.

b. Followingthereceiptofpesticide-relatedinformation,PARCmakesareferraltotheappropriatePARCmemberagencywithinonebusinessdayofreceipt.AllreferralstoPARCmemberagenciesaremadeinasimilarmannerviaemail,baseduponthePARCMemberAgencyReferralCriteriadescribedabovewiththeexceptionofODA.TheLeadPesticideInvestigatorandPARCstaffreceivesthesameemailnotificationwhencomplaintscomeinviaPARC’semail,ODA’sonlinePesticideComplaintform,ortheOERSreport.PARCstaffcoordinateswithODA’sPesticideprogramstaffonhandlingthesecomplaints.EachPARCmemberagencyconductsitsowninvestigation,accordingtoeachmemberagency’spoliciesandprocedures(SeeSOPsinAppendix).Thereferralcontainsalltheinformationnecessarytofollow-uponthereferral.(Seetheinformationcollectedviatelephoneabove.)

c. Ifthepesticide-relatedincidentinformationisfromtheOHA’sPESTprogramandincludespartAofanExposurePathway(EP),PARCfurnishestheEPtoallagenciesreceivingthePARCreferral.

IV. PARCBoardmeetings

ThePARCBoardmeetseveryothermonth,beginninginJanuary,andmeetinglocationsrotatebetweenSalem(ODA)andPortland(OHA).a. Agenda-ThePARCCoordinatorschedulesagendaplanningmeetingswith

bothPARCCo-ChairsandthePARCAdministratoratleastthreeweeksinadvanceofthePARCBoardmeeting.ThisgivestheCoordinatortimetopublishthedraftmeetingagendatoPARC’swebsite.(http://www.oregon.gov/ODA/programs/Pesticides/Pages/PARC.aspx)

i. AccordingtotheOregonAttorneyGeneral’sPublicRecordsandMeetingsManual,agendasarepostedtwoweeksinadvanceofthemeeting.

b. Meetingminutes–ThePARCCoordinatorisresponsibleforproducingPARCmeetingminutesfollowingeveryPARCBoardmeeting.DraftmeetingminutesofthepriormeetingareprovidedtoBoardmembersateveryPARCBoardmeeting,forapproval.

i. ThedraftmeetingminutesarepublishedonPARC’swebsitewithintwoweeksofthemeeting.

ii. Followingtheapprovalofthepriormeetingminutes,thePARCCoordinatormakescorrections,removesthe‘draft’watermark,andrepostsminutesas‘final.’

c. PARCBoardmeetingaudiorecordings–PARCBoardmeetingsarealsorecordedandtherecordingsarepostedonPARC’swebsite.

d. Recentincidents–PARCisresponsibleforprovidingPARC’srecentincidenttoBoardmembersateveryPARCBoardmeeting.IncidentsincludethosethatoccurredsincethelastPARCBoardmeeting.

e. Contributingfactorsheetsi. Attheconclusionofanagency’sinvestigation,investigationsummariesandreportsareemailedtoPARC.EachPARCincidentisassigned“contributingfactors(CF)”Thesearefactorsthatmayhavecontributedtotheincident(SeeAppendix3).TheBoardreachesaconsensusontheCFsforselectPARCincidentsastimeallows.Duetothehighvolume,notallCFsforeachincidentcanbereviewedattheBoardmeetings.

ii. OR-OSHA’sPesticideCoordinator,OHA’sPESTCoordinator,andthePARCCoordinatorwillinitiallyassignCFsforeachPARCpesticideincident,usingPARCmemberagencies’investigationsummariesandreportinformation.

iii. AtPARCBoardmeetings,PARCBoardmemberswillreachaconsensusonthecontributingfactorsassociatedwithselectPARCincidents.

iv. CFsheetsforthePARCincidentsthatwillbediscussedatPARCBoardmeetingsareprovidedtoPARCBoardmeetingsinadvanceofthemeeting.

f. Bienniallegislativereporti. ThePARCCoordinatorpreparesthePARCbienniallegislativereportsandsubmitstotheBoardforreviewbeforeitisfinalized.ThesereportsprovideasummaryandanalysisofallPARCincidentsthatoccurredinthetwomostrecentfiscalyears(FYs)

ii. PerORS634.550,thepesticide-relatedincidentswillbeexaminedto“…identifytrendsandpatternsofproblems,”bothinthereportingperiod,butalsoastheymayoccurovermultiplereportingperiods.

V. CoordinatingPesticideIncidentInvestigations

a. PARCassistsinthecoordinationofpesticideinvestigationsbyinitiatingconferencecallsbetweenPARCmemberagencies(andothers)whenaninvestigationinvolvesthreeormorePARCagencies.ThePARCCoordinatordetermineswhentoinitiateaconferencecallwithassistancefromODA’sPesticideProgramManager,followingtheguidanceprovidedintheIncidentCoordinationMatrix.(SeeAppendix2forIncidentCoordinationMatrix)

b. PesticideusedatarequestsmaybenecessarytoassisttheOregonHealthAuthority(OHA)inconductingPublicHealthAssessments(PHA)orExposure

Investigations(EI).ThePesticideUseDataRequests:OregonDepartmentofAgriculture(ODA)andOregonDepartmentofForestry(ODF)documentguideshowtoprovidethatinformation.(SeeAppendix12,Attachment5)

VI. Respondingtorequestsfromhealthcareproviders

Requestsfromahealthcareprovider(HCP)forpesticideuseinformationtotreatpeopleoranimalsshouldbedirectedtoPARCviaemailatPARC_HCP@oda.state.or.us,usingtheHCPRequestForm(AvailableonPARC’swebsite;urlabove).ThePARCCoordinator,ODA’sPesticideProgramManager,ODA’sPesticideProgramOfficeManager,ODA’sLeadPesticideInvestigator,andCitizenAdvocateandLiaisonallreceivetheemail.PesticideuseinformationtobeprovidedtoHCPs:

i. Activeingredients,bypercentii. EPAregistrationnumberiii. Manufacturer’scontactinformationiv. Amountofproductappliedwithdilutionratev. Identityofadditionaltankcomponents

b. Followingthereceiptoftherequestform,thePARCCoordinatorcontactstheHCPwithinonebusinessdayofreceipt,informingtheHCPthathisorherrequesthasbeenreceived.ThePARCCoordinatorwillcc:OHA’sPESTProgramforhumanincidents.

c. FollowingthereceiptoftheHCPrequestformbyPARC,thecoordinatorcontactsODA’sPesticideProgramManagerforassistance(onthesamedaythattherequestwasreceivedorwithinonebusinessday).ThePARCCoordinatorprovidestothePesticidesProgramManageracopyoftheHCPrequestform.ODA’sPesticideProgramManagerenliststheassistanceofODF’sFieldSupport,PolicyandMonitoringManagerorODA’sLeadPesticideInvestigator,whenneeded,toidentifytheapplicator.

d. Whentherequestformisreceivedonaweekendday(SaturdayorSunday)orduringaholiday,thePARCCoordinatorwillcontactODA’sPesticidesProgramManagerthenextregularlyscheduledworkday.

e. ODA’sPesticidesProgramManagerprovidestherequestedpesticideuseinformationtothePARCCoordinatorwithinonebusinessdayfollowingreceiptoftherequestform,ifavailable.

f. ThePARCCoordinatorprovidesthecompletedrequestformtotheHCP.ThePARCCoordinatoralsoprovidesthepesticidemanufacturer’scontactinformation.Incasesofanimalpesticideexposures,thePARCCoordinatormakesaprofessionalreferraltoDr.FredBerman(PARCBoardConsultant),providingtoDr.Bermanthecontactinformationofthetreatingveterinarian(s).

Purpose:ToestablishuniformprocedurestomakepesticiderelatedincidentreferralstothePesticideAnalyticalandResponseCenter(PARC)andtoguideOregonOSHA’sactionsfollowingthereceiptofaPARCinvestigationreferral,inaccordancewiththeOregonOSHA’sComplianceOfficer’sGuide.ThisSOPalsoestablishesuniformproceduresforcoordinatingandcommunicatingwithotherPARCmemberagenciesduringPARCincidentinvestigations.Scope:OregonOSHA’sinvestigationofwork-relatedpesticideexposures.Pesticidesincludedefoliants,desiccants,fungicides,herbicides,insecticides,nematocides,plantregulators,disinfectants,antimicrobialsandanysubstance,ormixtureofsubstancesintendedtobeusedfordefoliatingplantsorforpreventing,destroying,repellingormitigatingallinsects,plantfungi,weeds,rodents,predatoryanimalsoranyotherformofplantoranimallifewhichis,orwhichtheDepartmentofAgriculturemaydeclaretobeapest,whichmayinfestorbedetrimentaltovegetation,humans,animals,orbepresentinanyenvironmentthereof.[See:ORS634.006(8)].Background:PARCdoesnothaveregulatoryauthority;however,PARCismandatedtoperformthefollowingfunctionswhenpesticide-relatedincidentsadverselyaffecthumans,animalsortheenvironment:

PARC’sprimaryfunctionistocollectandanalyzeinformationaboutreportedincidentsandtocoordinatenecessaryinvestigations.Memberagenciesconductmostoftheinvestigationsandtakeanynecessaryenforcementaction(s).MemberagenciesarelistedinAttachment#1.OregonOSHAisabindingmemberofPARCandisrepresentedonthePARCBoardbyOregonOSHA’sPesticideCoordinator.

StaffwillreceivetrainingontheproceduresoutlinedinthisSOP,andanannualreviewoftheprocedureswilloccurtoensureeffectiveness.Action#1:WhenOregonOSHAisnotifiedofawork-relateddeath,catastropheorahospitalizationduetoapesticideexposure,thefollowingproceduresapply:Procedures:

1. TheOregonOSHAemployeereceivingthepesticide-relatednotificationwillimmediatelyforwardthatinformationtoOregonOSHA’sPesticideCoordinatoror,inhis/herabsence,arepresentativedelegatedtheseduties.

2. OregonOSHA’sPesticideCoordinatororrepresentativewillcollecttheinformationandnotifyPARCwithin24hoursviatelephoneandalsobye-mail.ContactinformationisincludedinAttachment#1.

3. OregonOSHAwillprioritizeresponsetothesepesticideexposureeventsbased

onthefollowingcriteria:• Imminentdanger:Investigationinitiatedwithin24hours.

Imminentdangerisacondition,practiceoractwhichexistsinanyplaceofemploymentandcouldreasonablybeexpectedtocausedeathorseriousphysicalharmimmediately.

• Serious: Investigation initiated within 5 days. A serious condition,practiceoractisonewhichexistsinanyplaceofemploymentandcouldreasonablybeexpectedtocauseseriousphysicalharm.

4. WhenanOregonOSHAinvestigationrevealsissuesrelatedtopotentialviolationsof

therulesofotherenforcementagencies,OregonOSHA’sPesticideCoordinatorwillestablishcommunicationwiththoseagenciesandmaintainappropriatecommunicationthroughoutthecourseoftheinvestigation.Theotherenforcementagenciesare:OregonDepartmentofAgriculture,OregonDepartmentofForestry,andOregonDepartmentofEnvironmentalQuality.OregonOSHA’sPesticideCoordinatorwillalsocoordinatecommunicationsbetweenOregonOSHAfieldstaffandtheOregonHealthAuthority’sPESTProgramincludinganynecessaryclarificationsregardingexposurepathwaysandpesticideidentitiesiftheyweremisidentifiedorunknownatthetimeoftheoriginalintake.

Action#2:WhenOregonOSHAreceivesaninvestigationreferralfromPARC,thefollowingproceduresapply:

Procedures:1. OregonOSHA’sPesticideCoordinatororrepresentativewilltakeall

investigationreferralsfromPARCandinitiateOregonOSHA’sresponse.

2. ReferralsfromPARCwillbeinvestigatedinaccordancewiththeOregonOSHA’sComplianceOfficer’sGuide.(UseOccupationalPesticideExposureIncidentReportForm.)OregonOSHAwillprioritizeresponsetothesereferralsbasedonthefollowingcriteriaandrespondwithintheindicatedtimelimits:

• Imminentdanger:Imminentdangerisacondition,practiceoractwhichexistsinanyplaceofemploymentandcouldreasonablybeexpectedtocausedeathorseriousphysicalharmimmediately.Investigationinitiatedwithin24hours.

• Serious:Aseriouscondition,practiceoractisonewhichexistsinanyplaceofemploymentandcouldreasonablybeexpectedtocauseseriousphysicalharm.Investigationinitiatedwithin5days.

3. Whentheinvestigationiscompleted,thecomplianceofficerwille-mailacopyof

thenarrativeandinspectionnumbertothePesticideCoordinator.ThePesticideCoordinatorwillreportthefindingsoftheinvestigationtoPARCwhentheenforcementactionisissued.

Action#3:TheseproceduresapplywhenOregonOSHAdiscoversorisnotifiedofanincidentoraccidentinvolvingpesticides.(Thistypeofaccidentorincidentdoesnotincludepesticide-relateddeath,catastrophe,orhospitalizationcoveredinAction#1.)Informationmaybeobtainedfrom:

• Acomplaintalleginghealthorsafetyhazardsinvolvingpesticides.• AreferralfromanothersectionofOregonOSHAoranotheragencyor

department.• Areportedaccidentwhereanindividualhasexperiencedanadverse

healtheffectsduetoapesticideexposure.• Amediareferral.• Inspection-relatedinterviewsortheinspectedemployer’sOSHA300log.

Procedures:

1. TheOregonOSHAemployeewhodiscoversorreceivesthepesticide-relatednotificationwillforwardthatinformationtoOregonOSHA’sPesticideCoordinator--or,inhis/herabsence,arepresentativedelegatedtheseduties--withinonebusinessday.

2. Thereferralinformationshouldalwaysinclude:

• Contactinformationforanypersonsexposed.• Locationwhereexposureoccurred.• Timeanddateofexposure.• Natureofthepesticideincident(potentialmisuse,threatsto

3. OregonOSHA’sPesticideCoordinatororrepresentativewillusethatinformation

4. This type of occupational pesticide exposure incident will be investigated in

Introduction/PurposeTheOregonDepartmentofForestry(ODF)administerstheregulationsoftheOregonForestPracticesAct(FPA),includingrulesfornaturalresourceprotectionwhenpesticidesareusedonforestland.

Thepurposesofthisdocumentaretooutlinethefollowing:

◻ ODFresponsestocomplaintsorreportsfromthepublicofproblemsrelatedtoforestpesticideuse.

◻ ODFresponsestoreferralsofinformationfromthePesticideAnalyticalandResponseCenter(PARC),basedonODF’sreferralcriteria.

◻ ODF’sroleinsupportinguniformproceduresforcoordinatingandcommunicatingwithandamongPARCmemberagenciesduringallpesticide-relatedincidents.

ThisdocumentisanextractfromODFpolicy,procedure,andguidancedocumentsrelatedtoinvestigationandreportingforallreportedincidents(notjustthoserelatedtopesticides).2

ScopeTheOregonForestPracticesActregulatesforestpesticideuseandothercommercialforestryactivities.TheFPAestablishesthepolicyoftheStateofOregonasencouragingthegrowingandharvestingofforesttrees,consistentwithsoundmanagementofsoil,air,water,andfishandwildliferesources.TheFPAfocusesonnaturalresourceprotection,recognizingthatotherfederalandstateregulationsprotecthumanhealthandproperty,aswellastheenvironment.TheOregonDepartmentofForestryadministerstheFPA.

TheOregonBoardofForestry(Board)hasadoptedforestpracticerulesrequiringoperatorstonotifyODFbeforestartingpesticideapplicationsorotherforestryactivities.NotificationstoODFmustcontainadescriptionoftheplannedactivity,includingalistof

pesticidestheoperatorplanstouse.3Notificationdataisavailabletothepublicuponrequest.

TheBoardhasalsoadoptedrulesregulatingforestpesticideuse.Thoserulesfocusonnaturalresourceprotection,recognizingthatotherfederalandstateregulationsprotecthumanhealthandproperty,aswellastheenvironment.Whenoperatorsapplypesticides,theymustrecordweatherconditionsandpesticidesthatwereused.4OperatorsmustprovidetherecordstoODFuponrequest.ODFrequeststherecordsasneededforODFinvestigationsorattherequestofPARC.

ODFinvestigatesreportsofconcernsoverforestpesticideapplications,andtakesenforcementactionforviolationsoftheFPA.ODFworkswithPARCandmemberagenciesininvestigatingpesticide-relatedincidents.

StandardOperatingProcedures

I. Uponreceivingreports(fromPARC,thepublic,orothersources)ofconcernsrelatedtopesticideapplicationsonforestland,ODFwill:a. DesignateanODFemployeeasinvestigatorforthereport.b. DesignateanODFemployeeasthelocalcontactforthereportingparty.c. Ifacallbackisneeded,contactthereportingpartywithinonebusinessday5

ofreceiptofthereport.d. Duringthefirstconversationwiththereportingparty,gatherthefollowing

information:1. Nameandcontactinformationofthereportingparty,ifwilling

toprovide.2. Thenatureofthereportedproblem.3. Thelocationofthereportedproblem.4. Namesofoperatorsorlandownersinvolved,ifknown.

e. Provideinformationtothereportingpartyasindicatedinthisdocument,includingthecontactinformationfortheODFcontact.

f. Advisereportingpartiesasfollows:i. Forahealthorsafetyemergency,contact911ortheOregonPoisonCenter.ClarifythatPARCisnotanemergencyservice.

ii. Foranon-emergencyhealthproblem,contactahealthcareprovider.iii. Ifreportingpersonalhealthinformation,reportittoPARCandnotto

k. ForwardreportedinformationtoPARCwithinonebusinessdayofODFreceiptwhentheinformationinvolvesreportsofadverseeffectsfrompesticidesonhumanhealthorproperty(includingpetsorotherdomesticanimals),ortheenvironment.ODFwillforwardcomplaintsorreportsrelatedtothefollowing:

i. Ongoingorcompletedpesticideapplicationsoractivities;ii. Reportsregardingspecificpesticideapplicationsoractivitiesthat

havenotyetoccurred.Thisdoesnotincludereportsofconcernsaboutpesticideuseingeneral.

l. DetermineifareportedsituationisrelatedtoactivitiessubjecttotheFPA.i. Ifitis,ODFwillopenaninvestigationincoordinationwithotherinvolvedagencies.

ii. Ifitisnot,ODFwillreferthereporttotheappropriateagencyorentity,includingPARC.

II. IfareportedsituationisrelatedtoactivitiessubjecttotheFPA,ODFwillinvestigatetoevaluatecompliancewiththeregulationsinthatlaw.a. Ifareportinvolvesanongoingoperation,ODFwillvisittheactivitysite

withinonebusinessdayofreceivingthereport.b. Ifanongoingoperationisnotinvolved,ODFwillvisitthesiteassoonasis

feasible,buttypicallywithinthreebusinessdaysofreceivingareport.c. UpondiscoveryofaviolationoftheregulationsoftheFPA,ODFmaytake

enforcementaction,includinganordertostopanyongoingviolations,andtorepairnaturalresourcedamage.

d. ODFwilldocumentinvestigationresultsusingitsComplaintReportForm,showninAttachment2.

e. ODFwillcommunicateODFinvestigationresultstoreportingpartiesandtoPARCandmemberagencies.Typically,ODFinvestigationswillbecompletewithin120calendardaysofreceivingacomplaintorreportaboutapesticideapplicationoractivitythatwasinprogressorhadalreadyoccurredatthetimeofthereport.

III. IfPARCnotifiesODFthatamemberagencyinvestigationofaforestpesticideapplicationhasbeenopened,ODFwillopenaninvestigationoftheapplicationforcompliancewithFPAstandardsandwillcoordinatetheinvestigationwithPARCandmemberagencies.

IV. InconsultationwithPARCandmemberagencies,ODFwillprovidePARCwithinformationfromtheODFnotificationofoperationdatabaseorODFfiles.

V. ODFwillobtainapplicationinformationfromanoperatorunderthefollowingconditions:a. UponrequestfromPARC.Basedonthedatarequested,ODFmayneedto

obtainthedailychemicalapplicationrecordsfromtheoperator.Inthatcase,ODFwillforwardtherecordstoPARC.WheretherequestisforanOregonHealthAuthority(OHA)PublicHealthAssessmentorExposureInvestigation,ODFwillfollowtheproceduresoutlinedintheODA/ODFdocumenttitled“PesticideDataUseRequest”(seeAppendix12,Attachment5).

b. WhenODFopensaninvestigationrelatedtocompliancewiththeFPA,ODFwillobtainthedailychemicalapplicationrecordsfromtheoperator.

VI. WhenODFreceivesdailychemicalapplicationrecordsfromoperatorsundertheauthorityofOAR629-620-0600,ODFwill:a. ReviewtherecordsforcompliancewithFPAstandards.b. ForwardtherecordstotheOregonDepartmentofAgriculture(ODA)through

PARCtoallowODAtoreviewtheapplicationdataforcompliancewithstandardsinpesticideproductlabelsand/orOregon’sPesticideControlLaw.

VII. WhereODForotheragencyinvestigationsinvolvelargeamountsofnotificationorapplicationrecorddata,ODFwillprovideGIS-relatedservicestomanagethedata,incoordinationwithPARCandmemberagencies.

VIII. ODFwillrespondasfollowstopublicrequestsforinformationonpesticidesthathavebeenapplied:a. Ifaninterestedpartyrequestsinformationonforestpesticideapplications,

ODFwillprovideanyinformationcurrentlyinitsnotificationofoperationdatabase6orotherfiles,includinganyinformationithasreceivedonwhatwasactuallyapplied.

b. ODFdoesnothavetheresourcestorespondtopublicrequeststoobtaindailyapplicationrecordswhenthereisnotanODForPARCmemberagency-basedneedtoobtaintherecords.IfODFhasalreadyobtainedtherecordsorplanstoobtainthem,ODFwillprovidetherecordstotherequestingparty.ODFwillencourageconcernedcitizenstocontactthelandownerand/oroperatorforinformationonwhatwasapplied.

c. Healthcareprovidersmayrequestinformationonpesticidesthatwereappliedataspecificplaceandtime.Withinonebusinessdayofreceivingsucharequest,ODFwillrefertherequesttoPARCforfollowupbasedonPARCstandardoperatingprocedures.

IX. AnynotificationorapplicationinformationODFholdsispublicrecord.ODFwillprovidesuchinformationtorequestingpartiesassoonasisfeasible,basedonstatepublicrecordslaw.

X. ODFwillreviewavailableinformationtodetermineiftheremaybeviolationsofpesticideproductlabelstandardsorOregon’sPesticideControlLaw.ODFwillforwardanyindicationsofpotentialviolationstoPARCforreferraltoODA.

PARCandmemberagenciesonanongoingbasis.b. ODFwillprovidePARCandmemberagenciesafinalreportwithinfive

businessdaysofcompletionofanODFinvestigation.Ifenforcementactionisneeded,theremaybeanextendedtimelineforcompletionofthefinalreport.Inthatinstance,ODFwillcontinuetoprovideinterimreports.

XII. InformationtoReportingPartiesa. Asappropriateforeachsituation,ODFwillprovideinterimreportsbackto

reportingparties.OneexamplemightbereportingbackonaquestionofwhetherapersonapplyingpesticidesonforestlandhadfiledanotificationofoperationswithODF.

b. ODFwillprovidereportingpartieswithafinalinvestigationreportwithinfivebusinessdaysofcompletionofanODFinvestigation.Reportsmaybeverbal(inpersonortelephone)forrelativelysimpleincidents,andinwriting(includingemailorsimilarmedia)formorecomplexinvestigations.

c. Atanypointinaninvestigation,uponrequest,ODFwillprovidereportingpartiesorotherinterestedpartieswithinformationitholds.

XIII. ODFwillcoordinatewithandassistODAwhensamplecollectionmaybeneeded.ODFhasneithertheresourcesnortheexpertisetocollectand/oranalyzesamples,sothisworkwillbeinsupportofODA,whichwouldbetheleadagency.

XIV. IncoordinationwithPARCandmemberagencies,ODFwillprovidepublicinformationsupportforworkingwiththemediaandforrespondingtopublicinformationrequests.

I. Introduction/Purpose

ThePesticidesProgramseekstoprotectpeopleandtheenvironmentfromadverseeffectsofpesticideusewhilemaintainingtheavailabilityofpesticidesforbeneficialuses.Thisincludesregulatingtheformulation,distribution,storage,transportation,application,anduseofpesticides.Investigativefindingsmayresultinenforcementactionsasauthorizedbystatute.Theprogramalsoprovideseducationalassistancetothepublicandpesticideusers,andprovidescomplianceassistance.

Thisdocument,incombinationwiththeInvestigatorNotebook,coversallaspectsofanInvestigator’sactivities,includingtypesofinvestigationsandhowtoconductthem,howtosample,andhowtousetheInvestigatordatabase(databaseuserguide).Additionally,itestablishesuniformprocedurestorespondtopesticidecomplaintsandreferrals,forexamplenotificationstothePesticideAnalyticalandResponseCenter(PARC).

II. Scope

OregonDepartmentofAgriculture(ODA),PesticidesProgram,isthestateleadagencyregulatingtheformulation,distribution,storage,transportation,application,anduseofpesticidesintheStateofOregon.IncarryingoutandenforcingtheprovisionsofORS634,ODAmayestablishlimitationsandproceduresdeemednecessaryandproperfortheprotectionofpersons,pollinatinginsects,bees,animals,crops,wildlife,land,orenvironment.

III. StandardOperatingProcedureFollowingistheprocessthatODAfollowswhenapesticidecomplaintisreceivedfromacomplainantdirectly(viaphonecall,emailorother).ThePARCCoordinatorandother

agenciesalsomightprovidereferralstotheLeadInvestigatorwhenacomplaintisreceived.TheLeadInvestigatorassignsthereferraltoanInvestigator.Thegoalisforcasestobecompletedoranenforcementactionissuedwithin180daysorless.

I. IntroductionTheOregonDepartmentofFishandWildlife’s(ODFW)missionistoprotectandenhanceOregon'sfishandwildlifeandtheirhabitatsforuseandenjoymentbypresentandfuturegenerations.OnemeansofimplementingODFW’smissioninvolvescoordinatingwithotherStateandfederalagencies,includingthePesticideAnalyticalandResponseCenter(PARC),onpesticide-relatedincidentsinvolvingasuspectedfishorwildlifepoisoningoraspillthataffectsfishandwildlifeand/ortheirhabitat.PesticideincidentsmayinvolvereferralsfromPARCorotherentities,requirenotificationtoPARCand/ortheOregonEmergencyResponseSystem(OERS),andmayinvolveconcurrentinvestigationswithPARCmemberagenciesand/orthefederalgovernment.

TheseprocedureswillguideODFWstaffintheresponseandinvestigationofnaturalresourcelossesordestructionandinjuryoffish,wildlife,andtheirhabitatspecificallyrelatedtopesticideincidents.

III. ScopeTheseproceduresapplytopesticideincidentsthatcauseinjury,death,contamination,ordestructionoffishorotherwildlifeorinjuryordestructionoffishorwildlifehabitatorfromanyviolationoftheconditionssetforthinanypermit,order,orruleoftheEnvironmentalQualityCommissionthatcauseinjury,death,contamination,ordestructiontofish,wildlife,ortheirhabitat.SpecificresponsibilitiesofODFWpersonnelforincidentsoccurringinestuariesandthePacificOceanareoutlinedin“ProtocolandOrganizationalStructureforOilandHazardousSpillsinMarineandEstuarineWaters”,ODFWMarineRegion,December1992.MoredetailedinformationonODFW’sroleinpollutionorhazardousmaterialincidentscanbefoundinthe“DraftNaturalResourceLossesResponse,Investigation,andWildlifeRehabilitationProceduresManual”,ODFW,May2013.ODFWwillusetheproceduresdescribedinthesemanualstotheextentpracticalgivenavailablepersonnel,resources,andtechnicalandsafetyconsiderationsspecifictoeachincident.ODFWwillreviewandupdatetheseproceduresasneeded.

IV. StandardOperatingProceduresODFW’spolicytoinvestigatenaturalresourcelossesisgovernedbyOAR635-410(NaturalResourceLosses),withfurtherjurisdictiontoseekcompensationfordamagesprovidedunderORS468B.060(LiabilityforDamagetoFishorWildlifeorHabitat)andtheComprehensiveEnvironmentalResponse,CompensationandLiabilityActof1980(CERCLA).ODFWwillimplementOAR635-410-0015duringthefieldinvestigationofthenaturalresourcelossinorderto:

(1) Analyzeanddeterminethecause(s)oflosses;(2) Promptlyrespondtoandmitigatelosses;(3) Determinehabitatorfishandwildlifemanagementmeasuresthatmayberequiredtorestorefishandwildlifeproductioninaffectedareas;(4) Determineviolationsofstatute,ruleorpermits;and(5) InformDepartmentstaff,otheragencies,thegeneralpublicandnewsmediaontheextent,nature,andcausesofnaturalresourcelosses.

Toimplementthepoliciesandprocedures,ODFWwillperformthefollowingstepsupondiscoveringorreceivingareportofasuspectedpesticide-relatedincidentfromthepublicorotherentity(includingPARC):

Step1.DocumentInitialInformationODFWshallbenotifiedbyPARCassoonaspractical,withinonebusinessday,ofanysuspectedpesticide-relatedpoisoningoffishorwildlifeorimpacttohabitat.StaffwilldocumentincominginformationonODFW’sInitialReportForm(AppendixB).IfODFWdiscoversapesticide-relatedincidentorisnotifiedofsuchbyanothersource,staffwillfirstdocumentavailableinformationontheInitialReportForm.

Step2.NotifyResponseAgenciesODFWstaffreceivinganinitialreportwillnotifyotherappropriateODFWstaff,theStateofOregonEmergencyResponseSystem(OERS),andPARC,asapplicable.StaffwillshareinformationrecordedontheInitialReportForm(AppendixB).

Initialresponseconsistsofemergencyactionstakenbyagenciestogaincontrolofthesituationandtoprotectthepublicandtheenvironment.ODFW'sroleintheinitialresponsephaseistominimizeimpactstofishandwildlifeandtheirhabitat.ODFWmayassistinidentifyingpotentialfishandwildlifereceptors,habitats,andresourcesthatareatrisk,assessextentofdamagetotheresource(s),collectspecimensandsamples,andidentifylaboratoriesforanalysis.ODFWstaffarenotfirstresponders,butselectDistrictstaffhaverequiredbasictrainingunderFederalOSHAlaw29CFR1910.120(HazardousWasteOperationsandEmergencyResponse)andcanassistwithaninvestigationafterasiteisstabilizedanddeclaredsafetoenter.EachDistrictisequippedwitha“NaturalResourceLossesInvestigationKit,”whichissuppliedwithbasicequipmentforcollectionofspecimens/samplesanddamageassessment.ODFWwillcollectfish,aquaticorganisms,andwildlife(dependingontheseverityoftheincident),butgenerallyreliesontheDepartmentofEnvironmentalQualityortheDepartmentofAgriculturetotakewaterquality,vegetation,andsoilsamplesduringacoordinatedinvestigationbecauseODFWstaffdonothaveadequateexpertiseoradvancedequipmenttodoso.Advancedprocedurespertainingtoaninvestigationcanbefoundinthe“DraftNaturalResourceLossesResponse,Investigation,andWildlifeRehabilitationProceduresManual”.ODFWwill:◻ Determineifanon-siteresponseiswarrantedperOAR635-410-005(NaturalResource

Pesticide Analytical Response Center Standard Operating Procedure Relating to Pesticide Exposures

I. Introduction:

TheOregonPoisonCenter(OPC)isthedesignatedaccreditedregionalpoisoncenterservingthestateofOregon.TheOPCisacomprehensivetoxicologyresource,establishedbytheOregonStateLegislaturein1978.TherolesandresponsibilitiesoftheOPCestablishedinthislegislationinclude:

1. Emergencytelephonemanagementandtreatmentreferralofvictimsofpoisoningand

overdoseincidents;2. Informationtohealthprofessionalsinvolvedinmanagementofpoisoningandoverdose

victims;3. Communityeducationprogramsdesignedtoinformthepublicofpoisonprevention

methods.

TheOPCisaccreditedbytheAmericanAssociationofPoisonCenters(AAPCC)andoperatesaccordingtothesenationalaccreditationstandards.Accreditationstandardsdefinecriteriarelatedto:accessandservicesprovidedbythepoisoncenter,professionalexpertiseofstaff,datacollection,patientmanagementguidelines,emergencyplanningandpreparednessandongoingqualityreview.TheOPCisstaffedbyRegisteredNurseswithadvancedtrainingintoxicologyandEmergencyMedicinePhysicianswhoareboardcertifiedMedicalToxicologists.

TheOPCprovidesemergencyassistancetothepublic,callingregardingpotentialexposurestopoisoningandtoxicsubstances,andaswellasemergencyrespondersandhealthcareprovidersseekingconsultationregardingmanagementandtreatmentassistanceforpatientsexperiencingandtoxicexposure.ReferralstotheOPCarefacilitatedthroughanationalemergencynumber1(800)222-1222.Callstothisnumberarerouteddirectlytotheregionalpoisoncenterservingthecaller’slocation.

III. StandardOperatingProcedures:

PesticideExposures:TheOPCmanagesover42,000callsannuallyfromthroughoutOregonregardingpoisoningandtoxicexposurestoawidevarietyofsubstancesincludingpesticides,pharmaceuticals,envenomations,industrialchemicals,plantsandhouseholdproducts.

Thepoisoncenterutilizesstandardoperatingproceduresforassessmentandmanagementofallexposurecases.TheOPCstaffcollectsdetailedsubstanceexposuredataoneachcase,bututilizescarefulassessmentandevaluationtodeterminethesignificanceofthatparticularexposuretotheactualmedicalconditionofthepatient.Asaresult,OPCdatareflectlargenumbersofpotentialexposurestosubstanceswithvaryinglevelsofcausality.

Uponreceiptofacalltothepoisoncenter,theRNorPharmacistspecialistconductsanassessmentofpatientcondition,circumstancesofexposure,substanceofexposureaswellasapatienthealthhistory.TheRN/Pharmacistsworkcollaborativelywiththemedicalstaffofthepoisoncentertodetermineariskassessmentandtodevelopatreatmentplan.TheOPCstaffhasaccesstoanextensivevarietyofresourcesanddatabasestoassistinidentificationofproductingredientscontainedinthemultitudeofsubstancesimplicatedincasesreferredtothepoisoncenter.Manyexposurescanbesafelymanagedathome,withtheOPCstaffprovidingongoinginstructionsforhomecareandconductingaseriesoffollow-upcallstoensurethepatientisrespondingwelltotreatmentandnoadditionalproblemsarise.Morecomplexcasesarereferredtoahealthcarefacilityforadditionalassessmentandtreatment.Inthesecases,theOPCstaffcontactthereceivingfacilitytoletthemknowofthepatientreferralandtoprovidetreatmentrecommendationstothehealthcareprovider.TheOPCcontinuestomonitorthesepatientswhileinthehospitaltoprovideongoingassessmentandtreatmentguidancetothehealthcareteam.TheroleoftheOPCisnotaregulatoryone,buttoprovidemedicalguidanceandrecommendationsforeffectivepatienttreatment.TheOPCactivitiesandrecordsareconfidentialandprotectedbytheprovisionsoftheFederalHealthInformationPortabilityandAccountabilityAct.

TheOPCgeneratesanelectronicmedicalrecordforeachcall.Specificdataelementsarecollected,includingsubstance,timeandsiteofexposure,clinicaleffects,therapies,treatmentsiteandongoingpatientassessmentandtreatmentcriteria.De-identifieddatafromthepatientrecordiselectronicallytransmittedinreal-timetotheNationalPoisonDataSystem(NPDS).NPDSisutilizedforNationalToxicosurveillancebyCentersforDiseaseControltoidentifyandtrackpotentialsentineleventsrepresentingpotentialpublichealthconcerns.

IV. ReferralstoPARC:

ToassistinmeetingtherequirementsofORS413.042,433.004&433.006(thatpesticidepoisoningsdiagnosedorsuspectedbyahealthcareproviderbereportedtolocalorstatepublichealthauthorities),theOPCreportssignificantsymptomaticpesticideexposurestothePesticideExposureSafety&Tracking(PEST)programstaffoftheOregonHealthAuthority.Symptomatic

pesticideexposurecaserecordsarefaxedtoPESTatthetimethecaserecordisclosedandcomplete.PESTstaffevaluateseachcasetodeterminewhetheritmeetscriteriaforfollow-up.Contactinformationforthosecaseswhichmeetcriteriaforfollow-upandinwhichthepatient/guardianhasgivenpermission,isprovidedtoPARCforreview.

InadditiontotheprocessdescribedaboveforroutinereferralstoPARC,theremaybeisolatedinstancesinwhichtheOPCstaffmayconsultwithPARCandmemberagencies.Theserare,isolatedinstanceswouldbeintheeventadditionalinformationisneededregardingthecircumstancesorsubstancesofexposureforthepurposeofeffectivelymanagingthepatient’smedicalcondition.

OHA’sPESTprogramcollectsinformationaboutreportsofhumanacutepesticideillnessandinjury(APII).Itseekstounderstandtheconnectionbetweenreportedhealtheffects,thenatureoftheexposure,andthetoxicologyofpesticideproducts(ifknown)forindividualreportsofpesticideexposure.ThisallowsPESTtoassess,basedonavailableevidence,howAPIIis(andisnot)impactingthehealthofOregonians.Thisisapopulation-basedevaluationusingnationally-acceptedcriteriafromtheCDC’sSENSOR-Pesticidesprogram,notamedicalevaluationofindividuals.

StartingwithFY2016PARCcases,PARCwillbeabletointegratePEST’stwoevaluationsofhumanhealthimpacts(ClassificationCategoryandSeverityCategory)initsassessmentofContributingFactorsleadingtothoseexposures(asdeterminedbyPARCBoard).UnderstandingbothhumanhealthimpactsofpesticideincidentsandtheircausalfactorsmayleadtopolicymakinginOregonthattakesintoaccountthecomplexityofhumanAPII.

Program Overview

Emergency Response Branch Community Right to Know Program

The Oregon Community Right to Know and Protection Act (CR2K) was passed by the Oregon

Legislature in 1985 (ORS 453.307 to ORS 453.520). This law requires facilities with certain quantities

of hazardous substances to annually report information about those substances to the Oregon Office

of State Fire Marshal (OSFM), using the Oregon Hazardous Substance Information Survey (HSIS).

The intent of the law is to provide emergency responders, pre-planners, and the public with information

about hazardous substances in their communities so they may be better informed and prepared should

an incident involving hazardous substances occur. This law also requires emergency responders to

report incidents involving the release, or threatened release of hazardous substances to the OSFM.

The information collected on the HSIS includes the name of substance, the hazardous ingredient, the

amounts stored at the facility, the type of container it is stored in, its temperature and pressure, health

hazards associated with the substance, and if it contains an Extremely Hazardous Substance (EHS) as

defined by the Federal Environmental Protection Agency.

The CR2K program takes steps to validate the information reported and then disseminates it to the

Oregon Regional Hazardous Materials Response teams, each Oregon fire department, and other first

responder agencies in Oregon, emergency managers, and health professionals. The information is

also provided to the public and others when requested.

OHAinitiatesfollow-upactiononthoseincidentswhichmeetthecasecriteria,andinvolveadeath,or;A) hashealthcareprovider(HCP)involvement,or;B) isareportofwork-relatedAPII,or;C) involvestwoormorepeople,or;D) arereferredbyPARC

ThePESTProgramispassive,population-basedtrackingprogramthatissetuptodetectandbringforwardtoPARC,asappropriate,reportedcasesofAPIIinOregon.Withinthoseparameters,itconductsfollow-uponfiveprioritizedsubsetsofpesticidepoisoningcasesreportedtoOHA.PESTdoesnotconductfieldinvestigationsnordoesitoperateinaregulatorycapacity.Thecriteriaabovehavebeeninplacesince2006andhavebeenfoundtoallowforanaccuratecharacterizationoftheburdenofAPIIinOregon,asreportedtoOHAandconfirmedundernationally-usedcriteriaforAPIIfromSentinelEventNotificationSystemforOccupationalRisk(SENSOR)ProgramattheCentersforDiseaseControl&Prevention.ThePESTProgramatOHAstrivestoconductfollow-uponreportsofAPIIthatmeetcasecriteriaaccordingtothescheduleinthe“Procedures(withTimelines&Rationale)”sectionbelow:

ingredient(s)ofthepreliminarily-identifiedformulation(s)shouldbeprovidedtolocalemergencydepartmentsandhealthcareproviders;

c. Ensuretreatinghealthcareprovidersknowoftheoptionofgaining

pesticideuseinformation,withinonebusinessday,fromPARCformedicaltreatmentofthosereportingAPII.ThisisdonebythehealthcareproviderssubmittingarequestformtothePARCCoordinatoratmailto:[email protected]’swebsiteathttp://www.oregon.gov/ODA/programs/Pesticides/Pages/PARC.aspx.

d. RemindtreatinghealthcareprovidersofOregonPoisonCenter’sabilitytoprovidetoxicologicaladvice,diagnosticalgorithmsandtreatmentadvice24hoursaday,7daysaweek–800-222-1222;

e. Emergencydepartmentstaffandhealthcareprovidersshouldbedirected

toreport,perORS333-018-0015,suspectedorconfirmedpesticidepoisoningstothePESTProgrambyfaxwithin24hours.

B. Notifytoxicologicalresourcesofincident:PESTstaffwillnotifystaffatboth

NationalPesticideInformationCenter(800-858-7378)andOregonPoisonCenter(800-222-1222)ofthisincidentbyconveyingthedate,theapproximatelocationoftheincident,andtheincident’spreliminarilyidentifiedactiveingredients.Inthatcommunication,PESTstaffwill:

a. AskthatcallcenterstaffforbothtakenoteofcallerswhoarereportingsymptomsofAPIIafterspendingtimeneartheapproximatelocationoftheincidentatthereporteddate/timethepesticide(s)werereleased.CallcenterstaffshouldurgethosecallerstocontactPEST;

b. NationalPesticideInformationCentercallcenterstaffshouldbeaskedtocontactPESTregardingcasesittakesfromsuchcallers.

C. Asappropriate,PESTmayinitiateconferencecall(s)ormeeting(s)between

representativesofthelocalhealthdepartment,PARC,OregonDepartmentofAgriculture’sPesticidesDivision,andothersregardingcurrentsituationinEventareaandintendedagencyactions.

D. PESTwillprovidethefollowinginformationtothelocalhealthdepartment:that

residentsinthevicinityofEvent,whoarereportingadversehealtheffectswhichtheybelievearerelatedtothepesticideEvent,dothefollowing:a. Symptomaticresidentsencounteredbystateagencyorlocalhealth

departmentstaffshouldbeencouragedtoseetheirprimaryhealthcareproviderorcallOregonPoisonCenterat1-888-222-1222fortreatmentadvice;

b. ContactPARC,whichwillreferthemtoPEST(andemailtheircontactinformationtoPESTaswell);and

c. Contacttheirneighbors,whomayalsobereportingadversehealtheffects,andurgethemtocontactPARC,whichwillreferthemtoPEST(andemailcontactinformationtoPEST,aswell).

Stage3:ProvideTechnicalAssistance:Afterthefirsttwostageshavebeeninitiated,PESTmayassistwithorinitiatethefollowing:

theactiveingredientsreportedlyinvolved;b. MakeavailableActiveIngredientFactSheets,fromtheNationalPesticide

InformationCenter,;c. Adviseindividualstoconsulttheirownhealthcareproviderifconcerned

abouttheirownhealth;d. Directresidentsconcernedaboutchronicorlong-termeffectsofthe

reportedactiveingredientstocontactNationalPesticideInformationCenter([email protected]).

C. DirecthealthcareproviderswithtreatmentordiagnosticquestionstocontactOregonPoisonCenter:

a. Ensurelocalhealthdepartmentknowstodirectanyandallrequestsbytreatinghealthcareprovider,regardingmedicaltreatmentordiagnosis,toOregonPoisonCenter;

Goal:ToAssistOregonHealthAuthority(OHA)inconductingPublicHealthAssessments(PHA)orExposureInvestigations(EI).Background:Situationsinvolvingpesticideusemayrequirevariousstateagencies,suchasOregonDepartmentofAgriculture(ODA)andOregonDepartmentofForestry(ODF),throughthePesticideAnalyticalandResponseCenter(PARC),toprovideassistanceincollectingpesticideusedataforOregonHealthAuthority(OHA)toconductpublichealthassessments(PHA)orexposureinvestigations(EI).Aninitialmeetingamongtheinvolvedagencieswillbeheldtodiscussdataneedsandtoestablishacentralizedpointofcontact.Dataneedsmayincludethefollowing:
