THE INTENT OF THIS GRANT

The Dufferin NFP Support and Capacity Building Project is seeking grant applications from not-for-profit organizations within Dufferin County for projects that will:

1. Bridge organizations (or the sector) to new ways of operating; and/or,

2. Support the implementation of new approaches that result in more effective and sustainable service delivery

OUR PRIORITIES

Priority will be given to proposals that: Address a need within the social service sector Promise high impact in improving the vitality of an organization or the sector overall Help build capacity within an organization or the sector overall Foster greater collaboration between organizations Lead to more effective and sustainable service-delivery Support organization(s) to transition to new structures or methods of operating Are “grassroots” NFP organizations (NFPs with ongoing ministry funding would be eligible only

as part of a collaborative project)

Trillium Foundation Grant Writing Tips – This document, provided by the Ontario Trillium Foundation, outlines key questions that grant writers need to consider and answer in developing grant applications. It was provided to attendees at the recent grant-writing workshops we hosted in Dufferin County.

Ministry Grant Writing Presentation – This PowerPoint presentation was used at the recent grant writing workshops in Dufferin County. It was developed by the Ontario Ministries of Culture, Citizenship & Immigration; and, Health Promotion and provides grant writing tips and links to additional funding sources.

Sector Focus Group Summary – This document summarizes input from initial focus groups hosted with Dufferin’s NFP Sector in May 2010. Key issues and concerns of the sector as well as some initial ideas are included.

Charity Village - Funding Sources – Charity Village is a site dedicated to supporting registered Canadian charities and nonprofit organizations. 3. Details of fundraising activities planned for this year: Caledon Meals on Wheels (CMOW) Board of Directors Fundraising Committee is involved in a variety of fundraising initiatives. The organization has so far this year participated in several events to fundraise and to promote and encourage financial support of our services. If approved for this grant we would promote and bring exposure of this program to Dufferin communities to attract financial support of the program. We have been successful in attracting support from local service clubs as well as business groups, community groups and individual donors.

4. Outline the mission, purpose and objectives of your organization:

Mission Statement: Caledon Meals on Wheels is dedicated to ‘Feeding the Community Spirit’ by enabling individuals of all ages to live independent, healthy lives by providing nutritious meals, companionship, social programs and exercise for the mind and body.

5. How does Dufferin County benefit from your activities? Who do you serve? If possible, indicate service volumes over the past two years?

Caledon Meals on Wheels (CMOW) has been providing hot meal services to Orangeville and to underserviced areas in south Dufferin for 12 years. The agency initiated and established a frozen meal program in Orangeville and Dufferin and then partnered with Dufferin County Community Support Services to integrate that program into their services. We currently deliver on average of 30 – 35 hot meals daily, 3 days a week (over 4,600 per year) to seniors, the frail elderly, physically/mentally disabled and convalescents. The hot meal program is a shared partnership with DCCSS who deliver 2 days of the week. We also partner with DCCSS and Alzheimer Dufferin along with other Dufferin organizations for joint volunteer training and share a variety of supportive resources. We build community capacity by engaging Dufferin residents to volunteer in our program – we currently have 17 volunteers in the program.

6. Who takes part in your activities or makes use of your services (including ages if applicable)? All of the programs offered by Caledon Meals on Wheels are open to seniors, the frail elderly, mentally or physically disabled, and convalescents. We also, on occasion, provide services to new Moms and family caregivers. Our SMILE Exercise Program is designed to include gentle chair exercises for the less active: beginner clients; up to our ‘graduated classes’ - all designed to transition clients as they progress and improve their physical abilities. Our In-Home Program is designed to assist clients who are the more frail elderly and clients who have very specific medical or physical limitations. We currently have 13 volunteers who have taken training in the home support exercise program and who are or will be matched to assist an in-home client. So, in a sense, paying forward the benefits that they have received. The age range of the clients currently participating in our Caledon SMILE Program is from late 60’s into the late 80’s with 78 years old being the average age. We have clients

in our In-Home program in their 90’s and one delightful lady who is 99 years of age.

PROJECT DESCRIPTION

1. Describe the project, outlining how you determined the need, and the specific activities that will address these needs. (200 words or less) Caledon Meals on Wheels has been offering a seniors exercise program, now into its 8th year. The program is called Simple Movement to Enhance Independent Living for the Elderly – SMILE Program. The design of this program is based on the Home Support Exercise Program designed by the Canadian Centre for Activity and Aging – CCAA. The CCAA is connected to Western University and St. Joseph’s Health Centre in London, Ontario. This program was initially funded through a 3-year grant from the Ontario Trillium Foundation and then received permanent funding from United Way of Peel. We currently operate 11 weekly classes in Peel Living Seniors Apartment Buildings, Seniors Centers and Community Halls throughout Caledon as well as offering an in-home program for clients who are unable to attend group sessions. This program has a proven record of contributing to healthy aging and impacting the health system by reducing falls among seniors.Currently, there is no comparable exercise program for seniors in Dufferin. CMOW had been approached previously to expand this program into Dufferin but the funds have not been available. This pilot project would establish a SMILE Exercise class to operate out of a Dufferin County Seniors Apartment Building at 43 Bythia St. in Orangeville. The space would be provided free of charge by Dufferin County for the operation of the exercise classes. Even though the classes would operate within the Bythia building, seniors in the community would also be offered the opportunity and encouraged to attend.CMOW would provide the Certified Seniors Fitness Instructor who would conduct the classes. We would also provide the Coordinator of our SMILE Program for client assessment and program design. We would supply all the support materials and would be responsible for recording and documenting all client information.All activities related to the SMILE Program would be covered by CMOW Liability insurance.

2. How will you know the project has been successful? Identify specific outcome measures you will use. We will record attendance by participants, repeat attendance and growth of the

classes. We will do an initial assessment of each client and record progress We will record testimonials from the participants based on specific criteria We will employ the TUG system of evaluating client’s physical improvements. The

TUG system is a very simple and easy way of measuring progress. TUG stands for ‘Timed Up & Go’ and measures improvements in physical ability.

We will set goals for the classes and for the individual clients and evaluate outcomes on a quarterly basis.

Outcome indicators would focus on improved abilities in activities of daily living Outcome indicators would address improved balance, mobility and strength

3. Identify partners or other organizations with whom you will be working on this project. Are the other organizations registered charities? The County of Dufferin Housing Division has agreed to provide space for the operation of the SMILE Program at no cost . They are not a registered charity.

4. Should you be successful in this application how soon will you be able to implement the project? When do you anticipate it will be complete? CMOW has the infrastructure and the expertise in place to implement this program almost immediately upon receipt of funding. We have a proven program in place and trained staff to facilitate the program. The County of Dufferin Housing Division has the appropriate physical space available in which to conduct the classes. The Housing Services Worker with Dufferin County has spoken with the residents in the Bythia building to gage interest in the program. The feedback from previous tenant meetings has been very positive. We have budgeted for one year of operation, and would expect to attract sustainable funding prior to the end of the project to continue and expand the program to other seniors’ buildings, seniors’ gathering centers and eventually to home-bound clients.

5. Describe how this partnership will help the development of your organization. (200 words or less)Caledon Meals on Wheels has a long history and connection to Dufferin County through partnerships with various Dufferin organizations. We also have a connection through our similar challenges in the delivery of services within a large rural area. At Caledon Meals on Wheels, ‘Feeding the Community Spirit’ is the motto we work and live by and we strive to provide services that will enhance all aspects of the lives of our clients: nutrition; emotional and social; and physical well-being – all influences on healthy aging. We have first-hand experience that addressing client services in this holistic approach really can make a difference and we want to share this with other seniors in other areas. Caledon and Dufferin are both part of the Central West LHIN and integrating our services is also a way of building a better health system for both of our communities.

6. Will this project allow your organization to build capacity within the sector? Please describe (200 words or less)The SMILE program has a proven success record. Documentation, statistics and most of all, personal testimonials, show the extremely positive benefits and outcomes resulting from this program. Clients of the program testify that the improvement in their physical abilities have enabled them to support general activities of daily living, but also that they have regained confidence to participate in community social activities, resulting in renewed friendships and social supports. The exercise classes have proven to be a social venue and the participants encourage and mentor each other on their endeavours and successes. This program has shown that it can contribute to a more vibrant senior community – seniors become more involved in community life even to the point of entering into volunteer service. This program has also connected to the business community and has attracted sizable donations over the past two years from a private company, Amgen Canada Inc. This company has given support to the SMILE Program because the program aligns with their philanthropic philosophy of supporting healthy families and healthy communities. So, building capacity within the sector, but also building capacity within the municipality itself and to the business community.

If the project is anticipated to extend past next year, how will you fund it?If the program is approved, we plan to very early on search out options for sustainable funding to continue and expand the program. As previously mentioned, United Way of Peel has been a supporter of the SMILE Program in Caledon for the past 5 years and this might be a good reference to support funding from United Way of Guelph Wellington. We would also look to funding from the business community.

8. Are there other aspects of this project with which you require assistance where volunteer support might be helpful? What skills might you require? For example:Caledon Meals on Wheels are able to support this program with an already established infrastructure and do not anticipate a need for assistance in any of the example areas.
