The Council on Senior Services (COSS) is a community coun-cil of professionals who work for and volunteer for agencies and businesses that serve senior citizens. The goal of COSS is to improve access to services for seniors in Allen County. This directory has been developed to educate senior citizens and professionals about the numerous services that exist.

Proceeds from the advertising sales in this directory pay for printing of the book and for grants to organizations that submit projects created for the health, education and/or welfare of seniors in our community. Grants awarded in 2012 included Community Transportation Network, Allen County Council on Aging, A Day Away Adult Daycare, Allen County TRIAD, Aging & In-Home Services, The Volunteer Center @ RSVP, Turnstone Center for Children and Adults with Disabilities, Northeast In-diana Radio Reading Service, Long Term Care Ombudsman, Hope Alive, Health Visions, Heartland Hospice, Parkview Se-niors Club and Trails Edge Resident Activity Committee.

In 2011, over 41,000 older adults and persons with disabilities, as well as their families and supporters, received services from or through Aging & In-Home Services of Northeast Indiana (AIHS). As the Area III Area Agency on Aging as designated by the U.S. Administration on Ag-ing and the State of Indiana, AIHS is the primary resource for aging and disabled populations and the largest funder of services includ-ing support for the Council on Aging in the following counties: Al-len, Adams, DeKalb, Huntington, LaGrange, Noble, Steuben, Wells, Whitley. Each family is assigned to a pantry based on their address. There are 28 locations throughout Allen County. To discover which food pantry to visit, call Associated Churches at 422-3528. When visiting a pantry, individuals will need to show a photo I.D. If current residency is different than the address on the photo I.D., then a utility bill, phone bill or rental lease needs to be presented for current proof of residency.

Generations is an inpatient psychiatric program designed to meet the emotional and medical needs of older adults suffering from acute psychiatric problems. Customized treatment is delivered by a caring and experienced multidisciplinary team of psychiatrists, nurses, technicians, therapists and activity coordinators.

Patients with dementia with complications, disruptive behaviors, confusion, delirium, depression and/or severe agitation are treated at Generations. The ultimate goal is to return patients to their optimal level of functioning after treatment.

Veterans’ Benefi ts for Long Term CareThe VA provides home and community-based services to veterans. These medically necessary services include home care, hospice, re-spite care, assisted living, domiciliary care, geriatric assessments and nursing home care. The veteran must be enrolled in VA’s health care system. Some of these services may be offered to veterans who do not have service-connected disabilities but who may qualify be-cause of low income or because they are receiving pension income from VA. Call the VA Northern Indiana Health Care System at 426-5431.

Aid and Attendance is an under used benefi t. It is a special pension benefi t for wartime veterans and their wives. Some who are not eli-gible for VA benefi ts because of income, may still be eligible for this benefi t if they have large medical expenses. The benefi t is for those receiving in-home care, inability to take care of bodily needs, or bedridden. They may also qualify because of mental injury or illness that requires regular assistance to protect them from the hazards in their daily environment. Long Term CareLong term care is a variety of services that include medical and non-medical care to people who have a chronic illness or disability. It helps the individuals meet their health or personal needs. It often as-sists people with support services such as activities of daily living like dressing, bathing, and using the bathroom. These services can be provided in the home, in the community, in assisted livings, or in nurs-ing facilities. Generally, Medicare does not pay for these long term care services, referred to as “custodial care.” Medicare pays only for medically necessary skilled nursing and therapies in the home and for skilled care in nursing facilities.

Medicaid and Long Term CareMedicaid is a combined State and Federal Government program that pays for certain health services and nursing home care for older people with low incomes and limited assets. It is an entitlement pro-gram which means that all who qualify for it will receive it. For infor-mation and to apply, call 1-800-403-0864 or access the internet at www.in.gov/fssa or www.ifcem.com.Medicaid Waiver is the Medicaid program that pays for in-home long term care services. Income allowed in order to be eligible for this program is three times the amount allowed for the regular Med-icaid program. This is not an entitlement program so there may be a wait.

“Care Transitions” originally described a joint initiative of the Admin-istration on Aging (AOA) and Centers for Medicare and Medicaid (CMS) to fund programs related to improving the coordination of the transitions through the network of Aging & Disability Resource Centers (ADRC) they had created within Area Agencies on Aging. ADRCs have been working to assist individuals in “critical pathways,” which is defi ned as the times or places when people make impor-tant decisions about long-term care. This work included several inno-vative interventions to facilitate the hospital discharge process and to help nursing facility residents return to the community. Since that time, models have expanded around the country, most notably, CMS’ Community-based Care Transitions Program (CCTP). The term is now applied to CMS’ CCTP partners as well as those created un-der other auspices. The hallmarks of any “Care Transition” program should be and evidence-based intervention, a person-centered ap-proach to support patients and their caregivers, support across care

Safe & Independent at HomeFor over 46 years, the U.S. Administration on Aging (AoA) has pro-vided home and community-based services to millions of older per-sons through the programs funded under the Older Americans Act. President Lyndon Johnson on August 14, 1965, signed into law the Older Americans Act (OAA). The OAA set out specifi c objectives for the maintaining the dignity and welfare of older individuals and cre-ated the primary vehicle for organizing, coordinating and providing community-based services and opportunities for older Americans and their families.

The original legislation established authority for grants to states for community planning and social services, research and develop-ment projects, and personnel training in the fi eld of aging. The law also established the Administration on Aging (AoA) to administer the newly created grant programs and to serve as the Federal focal point on matters concerning older persons. The mission was later ex-panded to include those individuals with disabilities.

Although older individuals and individuals with disabilities may re-ceive services under many other Federal programs, today the OAA is considered to be the major vehicle for the organization and de-livery of social and nutrition services to this group and their caregiv-ers. It authorizes a aide array of service programs through a nation network of 50 states agencies, 629 Area Agencies on Aging, nearby a national network of providers and a number of Tribal and Native Hawaiian organizations.As the Area III Area Agency on Aging as designated by the U.S. Ad-ministration on Aging and Indiana Family and Social Services Ad-ministration, Division of Aging, Aging & In-Home Services of North-east Indiana (AIHS) is the primary resource for aging and disabled populations and the largest funder of services including support for the Council on Aging in the following nine counties: Allen, Adams, DeKalb, Huntington, LaGrange, Noble, Steuben, Wells, and Whitley.

Each year, AIHS completes an Area Plan with information on the needs and preferences of older individuals and persons with disabili-ties and their family caregivers in relation to a comprehensive, coor-dinated system for providing long-term care in home and commu-nity-based settings in AIHS’ nine county region. The AIHS Area Plan is part of the overall State Plan on Aging which is submitted to the Administration on Aging in accordance with provisions of the Older Americans Act.

Services under that plan include Meals on Wheels home-delivered meals programs, Senior Dining in congregate settings, transporta-tion, adult day care, legal assistance, health promotion or chronic disease self-management programs, among others.

Nationally, information on services is available from the Eldercare Lo-cator, a public service of the U.S. Administration on Aging, at 1-800-677-1116, or from the National Department of Health and Human Services - www.longtermcare.org. Clearinghouse for Long-Term Care Information website developed by the U.S.

Locally, access to information and resources on services you need now or in the future, and how to pay for services, is available through Aging & In-Home Services of Northeast Indiana’s Aging and Disabil-ity Resource Center. Aging and Disability Resource Centers (ADRCs) were created by AoA, in partnership with Centers for Medicare & Medicaid (CMS), to provide consumers and caregivers information on home and community-based long-term care services. ADRCs provide consumers information, options counseling, referral, assess-ment, educational and assistance in planning for the future needs. AoA is emphasizing the importance of integration of proven evi-dence-based health promotion interventions, which can lessen dis-ability related to chronic illnesses, prevent falls, and reduce the bur-den experienced by family caregivers of individuals who are older and/or disabled.

The ADRC operated by AIHS receives a wide range of calls. The most requested services are: Case Management; Personal Attendant Care/Home Health Aide; Homemaker/Chore; Meals on Wheels; Re-spite Care; Adult Day Services; Emergency Response Systems; Assis-tive Technology; Assisted Living; Home & Vehicle Modifi cation; Adult Foster Care.

The challenge to the individual and family is often in funding those services. ADRC staff are able to assist in informing about and apply-ing for a variety of federal, state, local programs that assist with pay-ing for in-home services, such as Medicaid Aged & Disabled Waiver,

THE PROBLEMEvery year emergency personnel respond to hundreds of telephone calls to homes where there is a possible emergency situation, but are unable to gain immediate access. It may be a concerned neighbor or a family mem-ber who has a fear for their safety or a 9-1-1 call that is answered, the caller is unable to speak.

THE SOLUTIONRegistering in the Safe at Home Emergency Registration System will provide important contact information to emergency personnel before an emer-gency occurs. The information you give is strictly voluntary, confi dential and could save your life. Safe at Home information is entered into the computer system shared by police, fi re and EMS services throughout Allen County.

The program request must follow the Mission Statement of the Council on Senior Ser-vices…To promote the service and benefi ts of seniors in Northeast Indiana through a network of individuals and organizations providing some type of service to our maturing population.A maximum total amount will be awarded annually via the COSS grant process. The amount requested and the annual maximum award per organization is not guar-anteed. The awarded amount will be determined by the COSS Board of Directors.The focus of each request should mirror the purposes of the Council on Senior Ser-vices. These purposes include providing education, information and advocating for seniors. These purposes are to be accomplished using a grassroots approach. Grant recipients are notifi ed and checks are awarded at the November member-ship meeting.In order to continue to qualify for subsequent grant awards, a written report evaluat-ing the project will be required by the COSS Board of Directors by May 1st. There will also be an opportunity to provide an oral report to the general COSS membership at the May meeting.

Activities of Daily Living (ADL’s) - Bathing, dressing, eating, walking and all necessary activities required to function on a daily basis.

Advance Directives - Legal documents outlining how a person’s medical decisions are to be handled including living will, power of attorney for health care and health care representative.

Adult Protective Services (APS) - The offi cial agency mandated to receive, investigate, and respond to complaints of neglect and abuse of the elderly.

Ambulatory - The ability to walk freely and independently.

Assisted Living (AL) - Housing for an elderly or disabled person that provides room, meals and various levels of care.

Assessment - An evaluation performed of a person’s mental, emotional and social capabilities.

Do Not Resuscitate (DNR) - A written and legal order that a pa-tient does not want to be resuscitated.

Dementia - A group of symptoms that accompanies certain dis-eases. A loss of thinking, remembering, and reasoning. May also include behavioral changes and moods.

Durable Medical Equipment (DME) - Reusable medical equip-ment such as walkers, wheelchairs and hospital beds.

Durable Power of Attorney (DPOA) - A legal document giving an-other person authority to make fi nancial and legal decisions if a person becomes physically or mentally incapable.

Geriatrics - Medical specialty of treating the problems and dis-eases of old age.

Hospice - A way of providing care, support, and comfort to peo-ple approaching end of life.

Medicaid - A jointly funded state and federal program to assist those with a fi nancial need with medical care.

Medicare - A federal insurance program for those age 65 and over or disabled.

Memory Care - Special programs dedicated to caring for those with Alzheimer’s Disease, dementia or other cognitive impair-ments. Usually services include group activities, socialization and assistance with ADL’s.

Nurse Practitioner (NP) - A professional registered nurse who has special training to provide primary healthcare.

Palliative Care - Care provided to manage pain and make the patient as comfortable as possible.

Support Group - A group, usually family members or caregivers that meet for mutual support and to discuss practical means of addressing their loved one’s needs.

Long Term Care (LTC) - A term used to describe care given in the form of medical and support services to people who have lost some or all of their capacity to function without assistance.
