COVID-19 Reporting Options in Therap
Please report any COVID-19 related issues including suspected illness for clients by completing a special incident report in Therap
or report a support staff illness send to [email protected]
In response to new guidelines released by DDS, and Pursuant to W&I Code section 4639.6, to identify high risk events and incident trends related to the COVID-19 pandemic and to swiftly implement risk mitigation activities, the Director of the Department is directing regional centers and all providers to complete a special incident report (SIR) for any of the following events, even if the incident does not otherwise meet California Code of Regulations, Title 17 reporting criteria:
Expected Reporting
An individual has had direct contact with any person who has tested positive for COVID-19, including provider staff, family, and other individuals;
An individual is exhibiting COVID-19 symptoms and has visited a hospital for any related symptoms or has been tested for COVID-19;
An individual was recommended to be tested for COVID-19; An individual tested positive for COVID-19; or An individual is exhibiting COVID-19 symptoms and was instructed to quarantine.
This document provides additional guidance on how KERN REGIONAL CENTER would like Vendors to enter incidents related to COVID-19 into Therap. In coordination with KERN REGIONAL CENTER, Therap has added the following incident type: Event- Other/ Event Type- Communicable Disease/ Sub-type- COVID-19 As well as a Status field with the following check box options:
o Contact/Exposure o Negative o Positive o Symptoms o Tested

Multiple Events in One GER
The incidents you will enter may have
multiple events. For a single individual
please enter as many of the above events
as are relevant in one incident, if you
know about them at the same time. For
example, you would create an event
stating that an individual tested positive,
add another event that they visited the
ER, and one more event stating they were
relocated. All 3 events should be listed as
separate events with a High notification level
The window for reporting is still 24 hours, so if a consumer has additional items after 24 hours, please
enter it in a new GER.
High - Medium - Low
For COVID-19 related events, we are using notifications levels as noted in the guidance below:
HIGH = An individual tested positive for COVID-19;
HIGH = An individual is exhibiting COVID-19 symptoms and has visited a hospital for any related
symptoms or has been tested for COVID-19;
HIGH = An individual was recommended to be tested for COVID-19;
MEDIUM = An individual has had direct contact with any person who has tested positive for
COVID-19, including provider staff, family, and other individuals; or
MEDIUM = An individual is exhibiting COVID-19 symptoms and was instructed to quarantine.
If a consumer has multiple of these categories, the highest notification one should be used.
