Registration & Withdrawal Process Effective Aug. 2015, Creekside H.S. Student Services Office will enroll new students on the

following days and times below:

Mondays: 8:30 a.m. – 1:00 p.m.

Tuesdays: 8:30 a.m. – 11:30 a.m. and 1:00 p.m. – 3:00 p.m.

Wednesdays: 8:30 a.m. – 1:00 p.m.

Thursdays: 8:30 a.m. – 11:30 a.m. and 1:00 p.m. – 3:00 p.m.

Fridays: 8:30 a.m. – 1:00 p.m.

Parents/Guardians will be greeted by Mrs. L. Smith, Guidance Clerk. If they are there to

register, Mrs. Smith will check the address to make sure they are zoned for our school.

Parents can complete paper/online registration forms while they wait for the registrar

to start the enrollment process. Mrs. Smith will make copies for registrar.

If the parents/guardians indicate that the student has an Individual Education Program

(IEP) on the registration paperwork, they will be directed to our IST, Mrs. K. Williams. If

the IST is available, she will take the parents/guardians to her office for more

information. The IST will fill out the IEP scheduling form that will indicate the type of

classes the counselor should schedule the student for (ex. 00 – General, 70 –

Collaborative, 80 – Small Group, and 78 – Consultative). If the IST is not available to

meet with the parents/guardians, they will have to return the following day.

Students who register with all documentation after 10:30 a.m. will start school the

following day.

Please be aware if the caseload counselor is not available for scheduling a new student

due to other duties, the Counselor of the Day will assist the parent and student with a

new schedule.

Once the new student receives his/her schedule, bell schedule and map of the school, a

Guidance Office Aide will give the student a tour of the school.

Only the parents/guardians on record can withdraw students (identification is needed).

The registrar will compile a weekly enrollment and withdrawal report and email the

counselors every Monday.

ALL WITHDRAWALS REQUIRE A 24 HOUR PROCESSING TIME. BOOKS MUST BE

TURNED INTO COMPLETE THE WITHDRAWAL PROCESS

Transfer Grades & Transcriptions

When scheduling new students, the Guidance Clerk will make a copy of the new student’s

previous transcript and 2 copies of the transfer grades. This information will be given to

the counselor to schedule the new student. Counselors WILL NOT keep enrollment

folders in their office for any reason. The counselor will file the transcript in a

Transcriptions folder/binder to transcribe.

A copy of the transfer grades will be given to the student to show his/her teachers and

the Registrar/Guidance Clerk will email the transfer grades to the new teachers as well.

During the first 2-3 weeks of school, counselors will file transcriptions in their

folder/binder to transcribe during their scheduled Counselor of the Day or any given free

time.

All transcriptions for new students should be transcribed within a 4-week time frame from

their entry date and given to Ms. G. Wyatte for data entry. Ms. Wyatte, will complete

transcripts within one week from the time they were submitted. Completing

transcriptions in a timely matter will reduce students taking classes that aren’t needed

and it will reflect the correct amount of credits on their Creekside H. S. transcript.

ALL 1ST SEMESTER TRANSCRIPTS MUST BE TRANSCRIBED BY THE FIRST WEEK OF DEC.

2015, UNLESS THE STUDENT ENROLLS AFTER THE FIRST WEEK OF DEC. 2015.

ALL 2ND SEMESTER TRANSCRIPTS MUST BE TRANSCRIBED BY THE FIRST WEEK OF MAY

Counselor of the Day & Hall Duty Transitions Counselor of the Day is used to give counselors time to do things such as transcriptions,

organize, file, plan events, register new student if the caseload counselor is unavailable,

return calls and emails, and be available for Crisis.

During Senior/Junior Conferences, the Counselor of the Day will NOT schedule any

conferences on their scheduled day.

During 9th & 10th grade classroom guidance lessons, GCIC/Bridge Law Activities, and

Signs of Suicide, the Counselor of the Day will be AVAILABLE to the guidance office.

Therefore, one counselor will be available every day.

We currently have five counselors. The Counselor of the Day & Hallway Duty for

Transition for 1st and 2nd semester is scheduled below:

Counselor’s Corner (Lunchroom Resources)

Counselors will set up in the lunchroom on Wednesdays to offer

resources, disseminate important information, and help monitor

lunchroom activity. Counselor will work on a rotation schedule: (See

Student Passes

All students must present a pass to Mrs. Smith, Guidance Clerk, when

visiting the Counseling Office. Mrs. Smith, will check to see if the counselor

of record is available.

1. If the counselor is available, the student will sign in using the Electronic

Google Doc and wait for the counselor to bring them to their office.

2. If the counselor is unavailable, the student can fill out a Request to see a

Counselor form and Mrs. Smith will place the form in the counselors

drop box located on the outside of their door.

3. If the counselor is unavailable and it’s an emergency (i.e. Suicide

Ideations, Crying/Upset, Altercation, or Mediation) Mrs. Smith will give

the student to the School Social Worker. If the School Social Worker is

unavailable, the Counselor of The Day will see the student.

4. College Bound Picture Passes - Theses passes will be given to students

who has turned in an acceptance letter to Mrs. Smith, Guidance Clerk.

5. Special Colored Passes (RED) – Will be given to students with special and

specific needs. These passes will be a certain color. If this pass is shown

to a faculty member, please bring or allow that student to come to the

counseling office ASAP!

Appointments & Parent Teacher Conferences Parents will have to make an appointment to see the caseload counselor of record for their

student. We will no longer take walk-ins in order to maintain our schedule and timeline. If the

parent walks in and wants to see a counselor, Mrs. Smith will ask the following questions before

making an appointment with the counselor or scheduling a Parent/Teacher Conference (PTC).

Parent/Teacher Conference Times

Mornings 7:50 a.m. – 8:10 a.m.

After School 3:35 p.m. – 3:55 p.m.

1. What is the nature of your request/concern? (Many times these request can be

handled before they meet with a counselor).

a. Parent/Teacher Conferences (PTC) – If current grades is the concern, ask the

parent if they are on the Parent Connect/Parent Portal? If not direct, them

to our Parent Liaison, Mrs. Jennifer Jones. Mrs. Smith will let the parent

know that they can get grades as well as communicate with the teachers

through this portal. This is a crucial step before scheduling a PTC. If they

don’t have internet access, Mrs. Smith can print out the grade report with

proper identification. Mr. Kellam caseload (Si –Z) will be the counselor on

late duty, which is 5:00 p.m.

b. Schedule Changes – There is a specific time period for schedule change

request. Any request after that specific time period would need an

Administrative approval. Mrs. Smith will get the parents information for the

counselor to contact them at a later time.

c. Personal Issues – Mrs. Smith will get the parents information for the

counselor to get more information and possibly schedule an appointment, if

needed. VERY IMPORTANT!! If it’s bullying or harm to self/others (Suicide)

information, Mrs. Smith will see if the counselor can see them right away. If

the counselor is not available, Mrs. Smith will see if Mr. Grady Davis, School

Social Worker, can see the parent. If Mr. Davis is unavailable, the Counselor

of the Day will see the parent.

All classroom guidance will be done through the English classes

due to every student taking English.

Small group guidance (2 – or more classes at a time) will utilize

the media center and/or auditorium for these guidance lessons.

Suicide Protocol

If a school employee has reason to believe that a student is at risk for harm to self or

others, the employee should immediately refer the student to the School Counselor

and/or School Administrator. The School Counselor/School Administrator should use

the 4 Action Steps below as helpful reference and guide for the students who are in

crisis.

1. School Counselor/School Administrator should involve any necessary staff and

immediately:

*Supervise and monitor the student

*Call 911 for any medical emergency situation or imminent danger

*Notify the School Principal or Designated Site Administrator

*Contact the Parent/Guardian/Enrolling Person/Emergency Contact regardless

of the student’s age or emancipation and ask him or her to come to the school

campus immediately

2. While Notifications are being made an additional staff member (i.e. Counselor, Social

Worker, School Psychologist or other as designated by the School Administrator) may

privately discuss the student’s state of being with student. Discussion points can include

questions about what the student is feeling or thinking.

3. When the parent\guardian or emergency contact arrives, the staff:

*Conduct and document a conference with the parent/guardian or emergency

contact.

*Provide mental health resources to parent/guardian or emergency contact(s)

(i.e. FCS Immediate Mental Health Resources). The parent /guardian or

emergency contact(s) should be strongly encouraged to take the student from

campus to the facility/treatment provider of his or her choice for mental health

risk assessment. Please remember that neither the student, nor parent, should

be forced to seek treatment, nor should enrollment in school be denied if

parent/guardian or emergency contact(s) refuses to seek treatment for student.

4. Conduct and document a re-entry conference with parent/guardian and student when

student returns to school.

All necessary documents must be completed and give a copy to the parent/guardian.

Secretary, Registrar, IST, Special Ed. Dept. Chair, and School Social Worker will be invited

as needed.
