Biomedical Imaging Research DevelopmentSingaporeI was tasked by the School of Medicine and the Office of Life Sciences at the National University of Singapore to investigate and help develop the requirements for a leading programme in biomedical imaging research for Singapore.Subsequently I developed a proposal to develop national resources for both small animal and clinical translational research imaging research programs. These proposals included:• Understanding the space, personnel, equipment and scientific requirements for a

successful imaging research program• Recommending some technologies for the biomedical imaging research in Singapore,

in particular MRI and molecular imaging• Recommending the type and level of key scientific personnel for such programs• Proposing an initial budget for such programs

These proposals were used as the basis for a formal submission to the Biomedical Research Council by the Singapore Bioimaging Consortium (SBIC) to develop laboratories for animal and human research imaging, the former at the Biopolis and the latter to be housed next to the National University Hospital in a new Translational Medicine building.The national Clinical Imaging Research Centre (CIRC) was envisioned as a national resource, to focus on clinical trial and related research for quantitative and qualitative imaging analysis of human subjects. MRI and molecular imaging using PET and SPECT have been specified as requirements for this centre. I was closely involved in negotiations over the course of a year to find a suitable imaging technology and research partner for the CIRC, and this culminated in a final agreement to partner with Siemens in late 2007. second phase of the CIRC, including all the relevant space planning, structural and magnetic requirements, transportation and logistics, patient and staff management, equipment proposals and detailed specifications, as well as budgetary planning for the first 5 years of operation. Sydney UniversityAfter returning to Australia, I am leading the planning and development of a research imaging facility to be based in a new $400 million building, the Centre for Obesity, Diabetes and Cardiovascular Disease (CODCD), to be based at the University of Sydney adjacent to Royal Prince Alfred Hospital. The facility will include multimodality small animal imaging and a multimodality state of the art clinical imaging facility designed specifically for quantitative imaging research.The imaging program will aim to be research-intensive and hypothesis-driven, will be led by scientists and clinician-scientists, and will aim to develop novel methods for quantitative and functional imaging of the heart and cardiovascular system, monitoring treatment of metabolic diseases, and regional body fat composition in humans, large animals and small animals.

3. Leadership in Radiology Educationa. Undergraduate

In 1996-97, I was involved in the planning for the IT transmission and development of new learning materials for the new postgraduate medical curriculum for Sydney University. At that time I was also involved in the process to appoint clinical academic radiologists across various clinical departments in the University of Sydney’s various medical schools.Since 1998 I have been involved in planning medical student teaching programs for in-hospital postings to clinical radiology departments while in Singapore.I also developed a learning syllabus in medical imaging for medical undergraduates from the National University of Singapore in 2001, and instigated a formal program to develop online teaching resources in Radiology for medical undergraduates.From 2009 onwards I led the development of education and assessment in medical imaging for the graduate program for the University of Sydney, culminating in the completion of the undergraduate medical imaging curriculum at the end of 2009. In 2010 I was appointed Head of the Discipline of Imaging for the Sydney Medical School at the University of Sydney, and continue to progress development of vertically integrated teaching and learning in medical imaging across the 4 years of the Sydney Medical Program, with the aim of ensuring that students have access to consistent resources and undergo appropriate assessment of medical imaging knowledge in their training.

b.PostgraduateSydneySince 1990, I have been conducting informal mock examination teaching sessions for Radiology trainees preparing for their final written and oral examinations for the FRANZCR and FRCR qualifications. I commenced this at my own initiative in Sydney, Australia, and continued this in Singapore. formally underwent mock film reading and/or viva voce tutorials once a week under different instructors/tutors in the months leading up to their FRCR 2B examinations.In 2004, I was appointed chairman of the national Specialist Training Committee (STC) for Diagnostic Radiology, which oversees postgraduate training in Radiology for all basic and advanced trainees within Singapore, and also acts as the advisory committee for Radiology qualifications for the Ministry of Health’s Specialist Accreditation Board. Under my leadership, the supervisory and evaluation framework for trainees has been revamped, and since 2006 the STC developed a seamless 5-year training plan for Radiology trainees that was launched in May 2007. I developed new tools for progress evaluation and training assessment as part of this shift to seamless training. The STC also ran the Master of Medicine examinations in Diagnostic Radiology at National University of Singapore, as well as the Exit Certification examinations for all Radiology trainees. In 2007, the College of Radiologists Singapore was formed, and I was elected as Chairman of the Education Board, which focussed on post-accreditation CME, CPD and training program development.

RANZCRIn 2001, I successfully obtained accreditation for the Department of Diagnostic Imaging at the National University Hospital in Singapore as a training centre for the FRANZCR qualification. NUH remains the only hospital outside Australia and New Zealand to receive this accreditation. Since that time, a number of trainees have been appointed and have successfully passed examinations conducted both in Singapore and Australia. In September 2008 I joined the NSW Greater Metropolitan Clinical Taskforce (GMCT) as an academic representative, as well as the NSW Government Institute for Medical Education and Training (IMET) Radiology Training Implementation Group, to help improve and expand Radiology training across NSW.In January 2009 I took up the position of Chief Censor for the Royal Australian and New Zealand College of Radiologists, resulting in the completion of a new curriculum and assessment program for professional Radiology training for our College trainees in Australia, New Zealand and Singapore. I continue to lead this curriculum rollout and am leading changes in the RANZCR Training Centre Accreditation framework, reforming the structure and conduct of our College examinations, and planning the transition to a digitally based examination system.

← RADIOLOGY CLINICAL EXPERTISE

1. MRII worked in the first public hospital in Australia to have a high field (1.5 Tesla) MRI system in 1986, and I worked with this system for over 10 years. I trained in all aspects of MRI, including physics, instrumentation, pulse sequence optimization, pharmacology of MR contrast agents, and the broad range of rapidly expanding applications in MRI. I have taught extensively on fundamental aspects as well as clinical MRI of most body regions. I became experienced in diagnostic neuroradiology, abdominal radiology and musculoskeletal radiology, particularly in CT and MRI.

I have been a member of the International Society of Magnetic Resonance in Medicine, the premier scientific society in the field, since 1988, and was invited to sit on their Strategic Planning Committee in 2004. I have also sat on other committees of the society — Global Development, Workshops, Governance, and I chaired the Young Investigator Award Committee in 2009. My links with the Society enabled NUS to organize the first ISMRM Outreach Course to be conducted in Asia. This program has proven extremely successful and has been replicated around the world in many countries.

2. Breast ImagingI was trained at the Sydney Square Diagnostic Breast Centre, and the Northern Sydney Breast Screening Centre in breast screening and diagnosis using mammography, ultrasound and various breast interventional techniques including needle biopsy and localization. I also established the breast MRI service at Royal North Shore Hospital, and again at National University Hospital, including MR-guided breast localization and biopsy. In National University Hospital, I helped to establish a quality assurance system for mammography, and introduced image-guided breast needle biopsy and mammotome biopsy services.I developed the case to justify the establishment of the National Breast Screening Programme in Singapore, and chaired the Quality Assurance Committee of BreastScreen Singapore from 2002 to 2008. I have taught widely on breast imaging, screening and image-guided biopsy in Asia.I was recently appointed as a Visiting Medical Officer and Head of Breast Radiology for the Westmead Breast Cancer Institute, and have also joined the Clinical Advisory Board of this organisation.

3. Musculoskeletal ImagingIn Sydney, I trained in musculoskeletal imaging with a strong musculoskeletal radiology and orthopaedic team, learning musculoskeletal interventions including biopsy, and introduced a variety of nerve and joint injection techniques in the spine, as well as training myself in musculoskeletal MRI and ultrasound in the early 1990’s. I was a member of the Sydney Bone Club, “Innominates”, who met regularly to share cases, techniques and knowledge.In Singapore, I helped to popularize and teach musculoskeletal ultrasound locally and in Southeast Asia, to expand the use of multidetector CT in musculoskeletal injury, and to introduce MR arthography and various musculoskeletal intervention techniques in NUH, including image-guided tendon, nerve, disk and spinal joint injections. I am also trained in cement vertebroplasty and bone tumour radiofrequency and laser interstitial thermal ablation (with Prof. Afshin Gangi at the University of Strasbourg).I have an established international reputation as a musculoskeletal radiologist, being a founding member of the Asian Musculoskeletal Society, a multidisciplinary society registered in Hong Kong. I served as the first Honorary Secretary in this society and subsequently served on its Executive Committee until 2005. I have lectured as a member of Faculty in this society’s annual meetings held across Asia.I have been very active in musculoskeletal ultrasound training. I helped to organize the Annual Musculoskeletal Ultrasound Society Course in Singapore in 2000; this is

the premier course in MSK ultrasound. I am a regular member of this Society, having taught in this course in Singapore, the USA and Europe as a member of Faculty. I also co-organized the first Asian MSK Ultrasound course at National University Hospital in Singapore, and have also conducted hands-on ultrasound training workshops in Malaysia, the Philippines, Sri Lanka, India and Thailand since 1999. I was invited to teach musculoskeletal ultrasound for the International Skeletal Society’s Annual Refresher Course in Singapore in September 2005. Normally, only ISS members are permitted to teach in this course.

4. Interventional ProceduresI trained in vascular and interventional radiology in Sydney, at one of Australia’s best centres for neurointervention and vascular interventions. I learned a wide range of endoluminal vascular, gastrointestinal and hepatobiliary interventions, including dilatation and stenting techniques, as well as thrombolysis, embolisation and vasodilatation methods. I trained in all areas of image-guided tissue biopsy, drainage and injection. In Singapore, I introduced image-guided automated core needle biopsy throughout the body to NUH, as well as a wide array of ultrasound guided interventional techniques, and developed a central vascular line access programme in Radiology. I also introduced radiofrequency tumour ablation of the liver to NUH from 1998 onwards, and developed the programme for Yttrium-90 SIR-sphere liver tumour therapy starting in 1999-2000. From 2000 to 2007, NUH was the only institution in Southeast Asia performing this procedure regularly for patients with unresectable liver cancer.I have trained other radiologists to perform RF ablation and Yttrium-90 SIRT in Singapore and throughout Southeast Asia through in-house training programmes, as well as by conducting workshops on RF ablation. I have been invited to speak on RF ablation in workshops conducted in Malaysia and Vietnam, and to train and present on Y90 SIRT regionally and in Australia. SIRtex Medical has formally recognised me as one of their expert trainers and proctors. In 2007 and 2008 I organised and ran training workshops on Y90 SIRT for interventionists from Asian countries (Korea, Taiwan, Malaysia, Singapore and Hong Kong), and have proctored the first live cases of Y90 SIRT in a number of hospitals in various countries (Malaysia, Taiwan, Hong Kong, Korea and New Zealand).

Experiments performed on rabbits using stereotactic placement of cerebral electrodes, neurochemical and electrical stimulation of the temporal lobe (particularly the basal amygdaloid nucleus) and histochemical distribution of acetylcholinesterase within the telencephalon. 2003–2006 Virtual Spine WorkstationPrincipal Investigator: Teoh Swee Hin, Mechanical Engineering, National University of SingaporeThis project aims to develop computer-based models of the human spine using a combination of clinical-level CT and MRI and micro-CT to develop meshing models of bone, to develop computer simulations of bone architectural strength and behaviour. These meshing models will be used to model and simulate response of osteoporotic bone to the addition of bone cement used during the procedure of vertebroplasty. They will also be used to simulate mechanical changes of the spine during various motions, and to develop a simulator for surgical training. This was a broadly multidisciplinary program involving Engineering, Radiology and Orthopaedic Surgery. I am co-supervising one PhD student who is working on meshing models and cement vertebroplasty simulations.
