Course information: One of the required clinical rotations that 4th year pharmacy students complete where they work as a part of a health care team in a primary care outpatient clinic. The types of activities that the students perform with my supervision include medication reconciliation, optimization of medication therapy, medication counseling and provision of answers to medication questions from physicians, nurses, medical assistants and patients. Setting
CHI Health Clinic – Old Market Family Medicine Clinic
Outpatient primary care clinic serving approximately 3,500 patients
Other members of the healthcare team include physicians, medical assistants, nurses,
radiology technician, psychologist, social worker and dietician
Role & Clinical Practice Activities
Clinical pharmacist contributing to patient care approximately 32 hours per week
Contributions include:
o Providing consults and answering medication-related questions from other
members of the healthcare team (15-20 times per day)
o Answering questions and providing education to patients being seen at the clinic
(3-5 times per day)
o Providing medication disease management services directly to the patients (3-5
scheduled patient visits per day)
o Assist patients in ensuring they have access to all needed medications by
addressing potential changes in medications and/or navigating programs that
provide access at a lower cost or no charge (2-3 consults per day)
Activities impacting CHI Health Clinic healthcare system
o Leading an initiative to improve the screening practices for hepatitis C virus
throughout all 35 primary care clinics
Including development of electronic health record tools, screening
protocols, documentation tools and educational resources
o Leading a team of primary care physicians in piloting a project to initiate
treatment for hepatitis C virus in a primary care setting
