HHQI’s Progressive Cardiovascular Learning & Action Network
In support of your cardiovascular health improvement efforts, HHQI invites home health agencies and
other stakeholders to join the new Progressive Cardiovascular Learning & Action Network (CardioLAN).
The CardioLAN is a diverse group of individuals from various healthcare organizations – such as home
health agencies, professional associations, and QIN-QIOs – who share a commitment to improving
preventive cardiovascular care.
When you join the CardioLAN, your organization will be recognized on the HHQI website for your
commitment to improving your patients’ cardiovascular health. Joining the CardioLAN also provides
increased support from HHQI and networking opportunities through free monthly teleconferences and
webinars that focus on cardiovascular topics and resources.
The CardioLAN is an exclusive benefit of HHQI campaign participants, so you must be registered with
HHQI in order to join the CardioLAN. Registration is quick and free, and participation requires only as
much time as you can spare. Just follow these easy steps.
If you’re new to the HHQI National Campaign:
Step 1: Visit www.HomeHealthQuality.org.
Step 2: Click on the Registration tab in the menu across the top of the page and
complete the form. If you are a CMS-reporting home health agency, you’ll need
your Medicare provider number or CCN. When you click Submit, you’ll be asked if
you’d like to join the CardioLAN.
If you’re already participating in the HHQI National Campaign:
Step 1: Log in at www.HomeHealthQuality.org with the username and password that you
use to access the Best Practice Intervention Packages (BPIPs).
Step 2: Click on the CardioLAN page under the Members tab in the menu across the top
of the page to be directed to the one-click sign-up page for the CardioLAN.
