Disclaimer for Minors and Registration: Minors under the age of eighteen (18) years of age are not permitted to register without the permission of a parent or guardian. By registering for Hot Shots 2015 on the dates of August 29 and 30, user agrees and warrants to being eighteen (18) years of age or to having parental consent.

Hot Shots Release & Liability Waiver: I recognize and acknowledge that there are certain risks of physical injury to participants in the Hot Shots 3-on-3 Tourney. In consideration of participation, I assume on my own behalf and on behalf of my minor child/ward all risks associated with playing in this tournament, including, but not limited to falls, contact with other players, weather hazards, playing surface conditions, etc. On behalf of my minor child/ward and on my own behalf I agree not to sue, and waive, release, discharge and hold harmless Yakima Valley Sports Commission, the Yakima Valley Visitors and Convention Bureau and Yakima Valley Tourism, all sponsors, all charities benefiting from the event, and the City of Yakima, WA, together with each of their respective affiliates, officers, partners, members, board members, agents, employees and all committee members and other volunteers, from any and all claims of liability for death, personal injury, or property damage of any kind or nature whatsoever arising out of or in the course of participating in this event. In signing this document I intend and do bind heirs, executors, administrators, or assigns or anyone acting on my behalf or on the behalf of the named minor participant. This waiver extends to all claims of every kind or nature whatsoever, foreseen or unforeseen, known or unknown. I am solely responsible for determining if I or my minor child/ward is physically fit and/or skilled for activities contemplated by this event. I hereby grant full permission to the Yakima Valley Sports Commission and the Yakima Valley Visitors & Convention Bureau, to use photographs, videotapes, motion pictures, recordings, or other records of me or my likeness and that of my minor child/ward from this event for any purpose. As the parent/legal guardian of the named minor participant, I understand the above listed points and agree to allow the minor to participate. The undersigned has read the foregoing waiver and release, understands that s/he has given up substantial rights by signing, and signs it voluntarily. Every participant must sign a waiver. The participant’s parent/guardian must sign the waiver if under 18 years of age. PARTICIPATION WILL BE DENIED, if the signature of an adult participant or parent/guardian is not provided. When registering online, my online signature shall substitute for and have the same legal effect as an original form signature.

Hot Shots Promise: I will be a good sport at all time during the Hot Shots Basketball Tournament. I understand that if I exhibit improper behavior, both my team and I may be disqualified from the tournament.
