    14b WAREWASHING AND SANITIZING PROCEDURES

    OBSERVATIONS:Dish machine observed with highest temperature of 147F after 3 cycles. Machine was actively in use during

    inspection. Use sink to sanitize dishes until repaired. REPEAT VIOLATION

    CALCODE DESCRIPTION:Food-contact surfaces and multiservice utensils shall be effectively washed, rinsed, and sanitized to remove or

    completely loosen soils by use of manual or mechanical methods. The temperature of the washing solution shall be maintained at least

    100F or at the temperature specified by the manufacturer's label. Manual sanitation shall be accomplished during the final rinse by contact

    with solution containing 100 ppm available chlorine or 200 ppm quaternary ammonium. Mechanical sanitization shall be accomplished in

    the final rinse by achieving a utensil surface temperature of 160F or by contact with solution containing 50 ppm available chlorine. After

    cleaning and sanitizing, equipment and utensils shall be air dried. (114097, 114099.1, 114099.2, 114099.4, 114099.6, 114099.7, 114101

    23 VERMIN AND ANIMAL CONTAMINATION

    OBSERVATIONS:~20 live cockroaches observed at valve below long stretch of dirty dish table in warewashing area.

    ~20 live cockroaches observed around/behind thin plumbing that runs below dirty dish table from machine to end of table.

    2 live cockroaches observed at temperature dial of unused steam table at front service line.

    1 live cockroach observed below dry goods table at end of cook's line.

    Cockroaches appear to be German, multiple life stages observed below dish table. Facility is closed & health permit suspended

    until cockroach infestation is abated. Contact EMD for reinspection after pest control all serviced to eliminate the infestation.Remove all unused equipment that provides hiding areas for pests, clean entire facility including hard to reach places &

    dishes/utensils as they have been subject to pests. Seal all areas of walls/ceiling where pests may enter facility.

    Perform self inspection with a light throughout facility to ensure no live activity of cockroaches is observed prior to calling for

    reinspection. See comments at end of report for additional information.

    CALCODE DESCRIPTION:All poisonous substances, detergents, sanitizers, and cleaning compounds shall be stored to prevent

    contamination of food, equipment, utensils, linens, and single-use articles. Only pesticides that are necessary and specifically approved for

    use in a food facility may be used. Containers of poisonous or toxic materials shall have a legible manufacturer's label or if taken from bulk

    supplies shall be clearly and individually identified with the common name of the material. (114254, 114254.1, 114254.2)

    33 NONFOOD-CONTACT SURFACES CLEAN

    OBSERVATIONS:Plumbing & storage below front line observed with heavy grease accumulation. Clean & maintain clean prior to

    reinspection.

    Hood at charbroiler in kitchen observed unclean. Clean & maintain clean prior to reinspection.

    41 PLUMBING FIXTURES AND BACKFLOW PREVENTION

    OBSERVATIONS:Leak observed at left side faucet of 3-compartment sink. Repair within 7 days.

    Leak observed at left side of steam bath in kitchen. Repair within 7 days. REPEAT VIOLATION

    Ice machine is draining to floor that is not sloped to drain. Provide plumbing to direct drain to floor sink/drain with pipe ending 1

    inch above the drain.

    CALCODE DESCRIPTION:Steam tables, ice machines, ice bins, food preparation sinks, display cases, walk-in refrigeration units, and

    other similar equipment that discharge liquid waste shall drain into a floor sink with an air gap or other approved type of receptor. Plumbing

    and plumbing fixtures shall be maintained so as to prevent contamination, shall be kept clean, fully operative, and in good repair. The

    potable water supply shall be protected with a backflow or back siphonage protection device. Any hose used for potable water shall be of

    44a PREMISES KEPT FREE OF UNUSED EQUIPMENT

    OBSERVATIONS:Excessive amounts of equipment for catering & unused items for kitchen observed throughout facility. Remove

    unused items so facility may be better monitored for pests/vermin & maintained clean in hard to reach places. Correct within 30

    45 FLOOR, WALL, AND CEILING FINISHES

    OBSERVATIONS:Grout observed deteriorated throughout kitchen. Repair/replace within 90 days.

    Ceiling panels at front line observed propped up. Secure to eliminate gaps.

    Broken wall/grout at cove base in chemical room storage. Repair to eliminate gap to inside of wall.

    CALCODE DESCRIPTION:Food facilities shall be fully enclosed in a building consisting of permanent floors, walls, and an overhead

    structure that meets minimum standards. The walls and ceiling shall be smooth, durable, nonabsorbent, and easily cleanable. Flooring and

    base coving shall be smooth, durable, and made of approved nonabsorbent material that is easily cleanable. Floor surfaces shall be coved

    at the juncture of the floor and wall with a 3/8 inch minimum radius and shall extend up the wall at least 4 inches. (114143 (d), 114266,

    NOTICE OF TEMPORARY SUSPENSION OF HEALTH PERMIT AND IMMEDIATE CLOSURE OF FOOD FACILITY

    This notice is to inform you that as of this date the Environmental Health Permit for the above mentioned food facility is hereby suspended

    and all operations therewith are ordered to cease. This action is taken in accordance with Section 114409 of the California Health and

    Safety Code which states If any imminent health hazard is found, unless the hazard is immediately corrected, an enforcement officer

    may temporarily suspend the permit and order the food facility immediately closed.

    The violations for which this action is taken and associated sections of the Health and Safety Code (H&SC) are as follows:

    Section 113980 H&SC: Failure to protect food from contamination.Section 114259.1 H&SC: Food is subject to contamination from vermin.

    You have the right to make a written request for a hearing to the Environmental Health Division located at 10590 Armstrong Ste B.,

    Mather, CA 95655, within 15 days after receipt of this notice to show just cause why the permit suspension is not warranted. Failure to

    request such a hearing within the 15-day period shall be deemed a waiver of the right to a hearing. After these violations have been
