• Employees on an unpaid Leave of Absence (LOA) during AnnualEnrollment, will be given an enrollment opportunity uponreturn to Active status

• Employees are granted a 30-day window to make changes tocoverages for the new Plan Year on the GaBreeze web site or bycalling the GaBreeze Call Center at 1-877-342-7339

• If employees paid premiums while on LOA, and make nochanges during their enrollment period upon return to ActiveStatus, benefits will rollover into the new Plan Year with theexception of Flexible Spending Accounts

• If employees failed to pay premiums, no coverage will show onGaBreeze web site. An enrollment opportunity will be allowedto make selections for the new Plan Year, subject to applicablepenalties. Benefits from the current Plan Year will not rolloverinto the new Plan Year
