A. GENERAL REQUIREMENTS The Testing Firm must adhere to the following in testing equipment and systems at the House of Correction:

1. Infrared thermographic inspection services shall be conducted during normal business hours of 8:00 am to 5:00 pm Monday through Friday. All circuitry that is inspected using infrared technology shall be running at not less than 60% of normal load. Cleaning, testing, and related activities requiring outages must be performed during hours as follows: 6:00 pm to 6:00 am Monday through Friday. Schedule all work with House of Correction Staff to accommodate adjustments of time or work areas. Provide a minimum 48 hour notice to HOC Staff prior to any outages. Contractor shall use Owner’s outage notification form as applicable. Testing Firm Staff & Subcontractors will be escorted by HOC Staff to all areas and equipment requiring scanning, cleaning, and testing. 2. Infrared thermographic scanning shall be conducted by a licensed electrician that is at least a Level 2 Certified Infrared Thermographer as specified in the American Society of Nondestructive Testing (ASNT) SNT-TC-1A guidelines. 3. Testing and related activities shall be performed according to the most recent edition of the “Maintenance Testing Specifications for Electrical Power Distribution Equipment and Systems”, published by the InterNational Electrical Testing Association Inc. (NETA); National Electric Code (NEC); National Fire Protection Association 70B-Electrical Equipment Maintenance (NFPA 70B); Institute of Electrical and Electronic Engineers (IEEE); American Society for Testing and Materials (ASTM); and National Electrical Manufacturer’s Association (NEMA). Tests are to include NETA recommended testing, maintenance and inspection for switchgear, breakers, switches, and transformers; in addition to any other specific tests outlined by this RFP. 4. At the conclusion of tests and inspections, the Testing Firm’s licensed Professional Engineer shall analyze the test results and shall then forward a report to Milwaukee County DAS Architecture, Engineering, and Environmental Services Section, located at 633 West Wisconsin Avenue, with specific recommendations. The professional engineer will certify that the tests performed are in accordance with recognized standards for such procedures. Equipment used in making the test measurements shall be accurate and bear recent traceability to the National Institute of Standards. 5. The Testing Firm shall be an independent, third party entity that can function as an unbiased testing authority, professionally independent of the manufactures, suppliers, and installers of equipment or systems being evaluated. It shall be regularly engaged for the past five years in the testing of electrical equipment devices, installations and systems and shall use electrical engineers and technicians who are regularly employed for testing services. 6. Engineers and technicians performing these electrical tests and inspections shall be trained and experienced concerning the apparatus and systems being evaluated. These individuals shall be capable of conducting the tests in a safe manner and with complete knowledge of the hazards involved. They must evaluate the test data and make a judgment on the continued serviceability or non-serviceability of the specific equipment. Engineers and technicians shall provide maintenance recommendations.

7. Engineers and technicians shall be certified in accordance with ANSI/NETA ETT-2000, Standard for Certification of Electrical Testing Personnel. Each on-site crew leader shall hold a current certification, Level III or higher, in electrical testing. The Testing Firm’s engineer shall have a P.E. license. 8. Technicians shall be State Indentured Journeyman Wireman, responsible for their own lock-out/ tag-out and grounding. Coordinate with HOC Electricians. 9. The testing organization shall submit appropriate documentation to demonstrate that it satisfactorily complies with these requirements. 10. The testing organization shall have permanent local (Milwaukee County and the four border counties) representation while performing work throughout the term of the Agreement for Professional Services. 11. The Testing Firm’s employees shall be required to report in and check out as directed by HOC staff. 12. Milwaukee County shall have the right to determine the method to bring performance into compliance with the Agreement. 13. The Testing Firm's employees shall be trained and have experience in the testing and proper maintenance, and repair of electrical distribution equipment and systems. All parties involved shall follow industry standard safety procedures. A licensed Professional Engineer shall review all tests and written reports. 14. The Testing Firm’s employees are to become familiar with conditions relating to execution of electrical power distribution equipment and systems at HOC. Neglect of this requirement will not be accepted as cause for additional compensation. 15. Milwaukee County assumes no responsibility for Testing Firm's materials or equipment. The Testing Firm assumes full responsibility for damage or loss due to storing of materials and equipment. 16. The Testing Firm shall not use existing facilities for convenience, such as lunch rooms, storage rooms, etc. Violators will be warned and may be requested to leave the job-site. 17. The Testing Firm shall utilize designated on-site parking. 18. All HOC rules and regulations governing security and safety shall be enforced. All Testing Firm’s personnel shall be properly badged when within the facility. 19. Failure to observe security and safety requirements will be reason to remove Testing Firm or Sub-Contractors or their personnel or material from the site. 20. Smoking on the House of Correction property is prohibited, except where clearly marked.

21. Prior to first day of work, Testing Firm, shall provide a list of all personnel who will be involved in project, to the Milwaukee County Project Manager. This list shall contain 24 hour telephone numbers of those persons who should be called in the event of an emergency during day-time hours, evenings, and weekends. 22. Milwaukee County HOC Electrical Services Staff will be on-site during all scanning, cleaning, maintenance, and testing services; and will provide all replacement equipment and parts as required due to testing failures or equipment malfunctions.

B. BASIC SERVICES Base Fee ‘A’ Lump Sum Services: Base Fee ‘A’ Lump Sum Services shall include the infrared scanning, cleaning, maintenance, and testing of portions of the electrical power distribution system in the building as follows:

1. Infrared thermographic scanning of the interior of one (1) primary switchgear, five (5) transformers, five (5) main switchboards, and five (5) distribution panels noted below. 2. Cleaning, including wiping down and vacuuming, of the interior of one (1) primary switchgear, five (5) transformers, five (5) main switchboards, and five (5) distribution panels noted below. 3. Testing, including oil quality tests, turns ratio tests, and coil integrity tests on the four (4) oil-filled transformers noted below. 4. Testing, including injection tests and contact resistance tests, of the eight (8) fused switches/disconnects in the primary switchgear and main switchboards noted below. 5. Testing, including injection tests and contact resistance tests, of the forty (40) breakers in the main switchboards and distribution panels noted below.

Unit Price Services: Unit Price Services shall include the testing of portions of the electrical power distribution system as follows:

1. Testing, including injection tests and contact resistance tests, of a 100A breaker in the panels noted above under Base Fee ‘A’ Lump Sum Services, Alternate Fee ‘B’ Lump Sum Services, and Alternate Fee ‘C’ Lump Sum Services. (Testing of 100 amp breakers, if awarded, will occur while the panels are open for cleaning and other breaker tests).

Testing Firm’s Responsibilities: The Testing Firm shall provide, as a minimum, the following services in conjunction with the cleaning, maintenance, and testing requirements per NETA Standards:

1. Provide temporary power and lighting for testing and cleaning purposes. 2. Coordinate de-energizing of equipment with HOC Electrical Staff and WE Energies. 3. Provide lock-out and tag-out services. 4. Confirm equipment is de-energized. 5. Open equipment, and close equipment after cleaning and testing. 6. Install temporary grounding, and remove temporary grounding after cleaning and testing. 7. Remove breakers, and reinstall breakers after cleaning and testing. 8. Make equipment maintenance adjustments, including tightening loose parts and torqueing connections per manufacturer’s specifications. 9. Notify HOC Electrical Staff immediately of components which are recommended to be replaced, via verbal communication and written email correspondence. 10. Coordinate the re-energizing of the equipment with HOC Electrical Staff and WE Energies.

C. PROJECT PHASING

1. Phase 1 – Complete infrared thermographic scanning of all switchgear, switchboards,

transformers, and panels; as noted under Base Fee ‘A’ Lump Sum Services above. And, if awarded, complete infrared thermographic scanning of all transformers and panels; as noted under Alternate Fee ‘B’ Lump Sum Services above. And, if awarded, complete infrared thermographic scanning of all panels; as noted under Alternate Fee ‘C’ Lump Sum Services above

a. Complete all infrared thermographic scanning between 8:00 am and 5:00 pm, prior to

Phases 2 through 6.

2. Phase 2 – Complete cleaning, maintenance, and testing of Primary Switchgear, including switches and fuses, as noted under Base Fee ‘A’ Lump Sum Services above; and complete cleaning, maintenance, and testing of Transformers T-1, T-2, T-3, and T-4 as noted under Base Fee ‘A’ Lump Sum Services above; all between 6:00 pm and 6:00 am.

a. Maximum shut-down time: 12 hours b. Coordinate main electrical service disconnection and lock-out/tag-out with WE

Energies, and HOC Electrical Staff. c. Emergency generator to provide power to emergency circuits.

3. Phase 3 – Complete cleaning, maintenance, and testing of two (2) main switchboards, one (1) transformer, and five (5) distribution panels; including breakers, switches and fuses; (excluding Main Generator Switchboard MSB/EM); in Building 590-Dormitory Building Boiler House, Building 592-Surges Multi-Purpose, and Building 610-Pump House as noted under Base Fee ‘A’ Lump Sum Services above; all between 6:00 pm and 6:00 am. And, if awarded, complete cleaning, maintenance, and testing of three (3) transformers and two (2) distribution panels; including breakers; in Building 590-Dormitory Building Boiler House and Building 592-Surges Multi-Purpose, as noted under Alternate Fee ‘B’ Lump Sum Services above; all between 6:00 pm and 6:00 am. And, if awarded, complete testing of 100A breakers; in panels noted to be cleaned in Building 590-Dormitory Building Boiler House, Building 592-Surges Multi-Purpose, and Building 610-Pump House as noted under Unit Price Services above; all between 6:00 pm and 6:00 am

a. Maximum shut-down time: 24 hours (non-concurrent) b. Coordinate service disconnection and lock-out/tag-out to Building 590-Dormitory

Building Boiler House, Building 592-Surges Multi-Purpose, and Building 610-Pump House at Primary Switchgear with HOC Electrical Staff.

c. Emergency generators to provide power to emergency circuits.

4. Phase 4 – Complete cleaning, maintenance, and testing of one (1) Main Switchboard MSB/EM; in Building 590-Dormitory Building Boiler House, as noted under Base Fee ‘A’ Lump Sum Services above, all between 8:00 am and 5:00 pm. And, if awarded, complete testing of 100A breaker; in Main Switchboard MSB/EM in Building 590-Dormitory Building Boiler House, as noted under Unit Price Services above; all between 8:00 am and 5:00 pm

a. Maximum shut-down time: 8 hours b. Coordinate lock-out/tag-out of Emergency Generator at Building 590-Dormitory

1. A project overview, including procedures, appraisal, and recommendations.

2. Infrared thermographic inspection test results, including thermographic images of all tested

equipment.

3. Equipment test results, for each breaker, switch and transformer tested.

4. A summary of recommendations for equipment, including but not limited to: repair, replace, or monitor for possible replacement in the future.

5. A list of any code violations.

6. An Excel spreadsheet listing each piece of scanned, cleaned and/or tested equipment; and including the following information:

1. Milwaukee County reserves the right to request partial or full reimbursement from Testing Firm for change orders resulting from errors and omissions in the services they are contracted to provide.

VI. RELATED WORK BY OTHERS

Milwaukee County is providing current one-line electrical diagrams of the House of Correction facilities, as Attachment 4 to this RFP.

VII. SUBMISSION REQUIREMENTS The Proposal shall conform to Milwaukee County's Proposal Preparation, Submission and Evaluation Guidelines (see Attachment 1). The proposal shall include the Consultant Proposal Form (see Attachment 2) and the following information: A. Cover Page: Include project number and name, project location, Testing Firm's name,

address, telephone number, e-mail address, and proposal date.

B. Table of Contents: Include an identification of the material by section and page number.

C. Letter of Transmittal: The name and description of the organization submitting the proposal briefly stating the proposer’s understanding of the services to be provided.

D. Description of the Organization: A description of the organization submitting the proposal. Include the name, size, legal status (corporation, partnership, etc.), professional registration/certification, and major type of activity or areas of consulting.

E. Description of the Organization’s Experience: Include a list of similar projects that the

organization has participated on during the past five (5) years. Attach a separate sheet for each project, up to five (5) maximum, giving a brief description of each project, the organizations participation, and a client contact reference and phone number. Provide a list of three (3) references that can be contacted with questions regarding your past work.

F. Description of Project Team/Resumes: Include an organizational structure of the project team, including any sub-consultants to be used for this project. Include the name of the Principal-in-Charge of this project along with the name, occupation and title of the Project Manager who will be in charge of this project. Provide a one-page resume' for each individual involved in the project, and include their name, title, qualifications, duties for the project, professional registration, relevant certifications, and a brief description of related experience including time contribution in this capacity to past projects.

G. Sub-Consultants: Indicate the names and addresses of any sub-consultants and/or associates proposed to be used in this project. State the capacity they would be used in and the approximate percentage of the total services they would provide. Also state their past experience in the field.

H. Project Approach: Provide a description of challenges you anticipate in this project and how you propose to overcome them. Discuss how you plan to staff the project to efficiently complete the work effort.

I. Project Schedule to Completion: Provide a timetable and relationship of tasks which are necessary to complete this project as noted in the “Project Schedule" section of this RFP.

J. Constant Effort: Include a spreadsheet/matrix listing the names, classifications, hourly rates and hours to be spent by each required task to complete the project as described in this RFP.

K. TBE Firm Goals: The Targeted Business Enterprise (TBE) participation goal for this project/contract is 17%. (Include Commitment to Contract with TBE (TBE-14) form(s) with the

proposal).

i. Community Business Development Partners (CBDP) is responsible for monitoring and enforcing Milwaukee County's Targeted Business Enterprise (TBE) Program for inclusion of small business. Targeted firms include DBE firms certified under the Unified Certification Program following Federal regulations, and WBE and MBE certifications from the State of Wisconsin DOA.

For a non-certified firm bidding or proposing as Prime, participation may be obtained utilizing a TBE firm, whether DBE, MBE, or WBE. This allows for increased participation by providing opportunities for multiple certifications to be included in the project.

For a TBE firm bidding or proposing as Prime, the goal must be satisfied using DBE

subcontractors or sub-consultants. MBE and WBE certifications count as additional participation once the goal is achieved through DBE participation. Any work a TBE Prime self-performs would be counted as additional participation in excess of the participation provided by subcontractors or sub-consultants.

Compliance reporting is accomplished from collection of data in the Diversity Management and Compliance System, utilizing B2GNow software. Prime contractors are required to report payments received from the County and amounts paid to subcontractors in the system. Subcontractors will receive an automated email requesting them to confirm the amounts and whether the terms of the prompt payments policy were followed. There is no cost to the Prime or any subcontractor, the only requirement is to become a registered user and complete the one hour webinar training.

L. Quality Control: Submit a contract document quality control plan. Quality control is to be performed by individuals not assigned to the project on an ongoing basis.

M. Fee Proposal: The fees for this project shall be clearly stated as lump sum not-to-exceed fees for the services requested.

VIII. TESTING FIRM SELECTION

Proposers must recognize this is not a bid procedure, and a Professional Services agreement will not be awarded solely on the basis of the low fee proposal. Milwaukee County reserves the right to accept or reject any and all proposals, issue addenda, request clarification, waive technicalities, alter the nature and/or scope of the proposed project, request additional submittals, and/or discontinue this process. The proposal evaluation team will be made up of three to five individuals with technical knowledge of the requirements and familiarity with the project. The project manager will post this RFP, addendums and other information related to this project to the Milwaukee County Construction RFP website:

To allow time for proposal preparation, Milwaukee County will not post anything new within two days of the proposal due date.

IX. GENERAL REQUIREMENTS

1. With the signing and submission of a proposal, the submitting Testing Firm certifies that the

standard terms and conditions of the Agreement for Professional Services (that will be used to contract with the selected Testing Firm) has been read and understood, and that the submitting

Testing Firm is ready, willing and able to sign the agreement when requested without making any substantive changes.

2. Proposers shall follow Milwaukee County Code of Ethics as follows: No person(s) with a personal financial interest in the approval or denial of a Contract being considered by a County department or with an agency funded and regulated by a County department, may make a campaign contribution to any County official who has approval authority over that Contract during its consideration. Contract consideration shall begin when a Contract is submitted directly to a County department or to an agency until the Contract has reached final disposition, including adoption, County Executive action, proceeding on veto (if necessary) or departmental approval.

3. The successful Testing Firm must be an Equal Opportunity Employer.

4. The proposal shall conform with all attached documents. All proposals should use this RFP and its attachments as the sole basis for the proposal. The issuance of a written addendum is the only official method through which interpretation, clarification or additional information will be given.

5. All costs for preparing a proposal, or supplying additional information requested by Milwaukee County, is the sole responsibility of the submitting party. Material submitted will not be returned.

Attachment 1: Proposal Preparation, Submission and Evaluation Guidelines I. Explanation to Prospective Offerors

Any prospective offeror desiring an explanation or interpretation of this RFP must request it in writing no later than seven business days before the last date for submission of proposals. Requests should be directed to the individual in charge at the address listed in the RFP. Any information given to a prospective offeror concerning a solicitation will be furnished promptly to all other prospective offerors as an amendment to the RFP, if that information is necessary in submitting offers or if the lack of it would be prejudicial to any other prospective offerors.

II. Complete Proposals

Proposals shall represent the best efforts of the offerors and will be evaluated as such. Proposals must set forth full, accurate, and complete information as required by this section and other sections of this RFP.

III. Unnecessarily Elaborate Proposals

Brochures or other presentations beyond those sufficient to present a complete and effective response to this solicitation are not desired and may be construed as unnecessarily elaborate and an indication of the offeror's lack of cost consciousness. Elaborate art work, expensive paper and binding, and expensive visual and other presentation aids are neither necessary nor desired. Concise and clear proposals are sought.

IV. Retention of Proposals

All proposal documents shall be retained by the County and therefore, will not be returned to the offerors. The County will not pay for preparation of proposals or for proposals that are retained by the County.

V. Examination of Proposals

Offerors are expected to examine the Site, Statement of Work and all instructions and attachments in this RFP. Failure to do so will be at the offeror's risk.

VI. Legal Status of Offeror

Each offeror must provide the following information in its proposal:

A. Name of the offeror;

B. Whether offeror is a corporation, joint venture, partnership (including type of partnership), or individual;

C. Copy of any current license, registration, or certification to transact business in the State of Wisconsin if required by law to obtain such license, registration, or certification. If the offeror is a corporation or limited partnership and does not provide a copy of its license registration, or certification to transact business in the State of Wisconsin, the offeror shall certify its intent to obtain the necessary license, registration or certification prior to contract award or its exemption from such requirements; and

D. Copies of any current license, registration or certification required in RFP;

E. If the offeror is a partnership of joint venture, names of general partners or joint

venturers. VII. Organization of Offeror

Each proposal must further contain a chart showing the internal organization of the offeror and the numbers of regular personnel in each organizational unit.

VIII. Offerors Authorized Agent

Each proposal shall set forth the name, title, telephone number, and address of the person authorized to negotiate in behalf of the offeror and contractually bind the offeror, if other than the person signing the proposal.

IX. Price Schedule Submission

Offerors are to submit prices for each item identified in the Proposal. Offers for services other than those specified will not be considered. The prices set forth in the schedule will be used for evaluation purposes and for establishing a contract price. Milwaukee County reserves the right to accept or reject any and all Proposals.

X. Certification and Representations

Offerors shall return with their proposal resumes and any other documents as may be requested in the RFP.

XI. Signing of Offers

The offeror shall sign the proposal and print or type its name on the form. Erasures or other changes must be initialed by the person signing the offer. Offers signed by an agent shall be accompanied by evidence of that agent's authority, unless that evidence has been previously furnished to the County.

XII. Proposal Guarantee

By submitting a proposal the offeror guarantees that it will keep its initial offer open for at least 60 days.

XIII. Acknowledgement of Amendments

Offerors shall acknowledge receipt of any amendment to this solicitation by signing and returning the amendment or by letter or telegram including mailgrams. The County must receive the acknowledgment by the date and time specified for receipt of offers. Offeror's failure to acknowledge an amendment may result in rejection of the offer.

XIV. Late Proposals and Modifications and Withdrawals of Proposals

Any proposal received at the office designated in the solicitation after the exact time specified for receipt will not be considered.

XV. Proposal Evaluation Criteria

Following is a list of general criteria which will be used to evaluate the proposals:

a. Quality and Responsiveness to the RFP. Weight: 20%

b. Project Approach and Understanding, including strategy to perform requested work and time schedule. Weight: 30%

c. Qualifications and Experience. Weight: 35%

d. Fee and Hourly Rates. Weight: 15%

XVI. Staffing

Consultant shall provide, at its own expense, all personnel required in performing the services under this agreement. Such personnel shall not be employees of Owner.

The offeror must describe his or her qualifications and experience to perform the work described in this RFP. Information about experience should include direct experience with the specific matter and similar facilities. Areas of expertise of each proposed staff member shall be provided (i.e., engineering, economics, architecture, planning). Specific examples of similar or related projects previously conducted shall cite:

- Name of client organization

- Name, address, and current telephone number of client contact person

- Contract number and inclusive dates

- Contract amount

Offeror shall provide the following information for every resume:

- Full name

- Title and areas of specialty

- Affiliation (that is, staff of offeror or Sub-consultant)
