1Statement from the Chair
The past year has presented numerous administrative challenges. From October 2012 through September 2013, we had an interim business director. Sharing this person with the department of surgery helped to keep things operative. However; this resulted in some strategic mistakes that will impede our growth in the coming year.
First, the renegotiation of the Atrium lease resulted in a buyout of the general surgery portion. The tradeoff being, Orthopaedics received four additional years of an unfavorable lease. Second, a renewal of a physical therapy contract with NovaCare was executed, again placing the practice in an unfavorable position. Finally, a contract with Premier physical therapy was signed with no business plan for recovering the cost of the position.
The above mentioned has placed the department in an unfavorable financial position as well. In start of the new service line of physical therapy at the Health Center, our department has born all of the startup costs. Coupling this with the Wright State Physicians overhead applied to clinical earnings makes it very difficult to make physical therapy profitable. Right now we are covering cost of the division: however, at our current volumes it may take years to pay off the deficit.
The questions we raise are in the viability of any ancillary service when the Wright State Physicians overheads are taken right off the top. This business model will make it difficult to expand to the levels needed to make it profitable. In addition, we have not received any “buy in” from the athletic trainers that work for WSU athletics. In the original planning, they were going to be credentialed and would provide FTEs as PT extenders. Our attempt to get this relationship going has stalled, primarily because Tony Ortiz and the Provost have both moved on to other positions. Our relationship with Athletic training will need to be critically evaluated as we move forward.
The other challenge that will become larger in the coming year is the rent and business plan for the new Health Center. Athletic training occupies half of the first floor, yet contributes no revenue. In order for the Health Center to become successful their relationship will need to change, otherwise the rent will increase for other occupants, resulting in the risk of losing the rent of paying tenants. We will need to aggressively pursue other revenue producing sources for the first floor.
Within the infrastructure of our department, we have had numerous personnel changes which have produced some adjustments. Our new business director, Pam Kadrovach has been a very positive influence. She has the experience of running an orthopaedic practice and has greatly helped our billing and office process. We also hired an accountant, a shared position with general surgery. Amanda Carson has made great strides in making sense of our overhead both within the department and within WSP. Unfortunately, the financial management we get from WSP is deficient. Amanda has uncovered discrepancies in the department fund that have gone on for years, never being corrected by corporate despite an internal audit and two external audits in the end of 2012. Our current challenge is to pay the incentive reconciliation to our high earning faculty while rebuilding our department fund.
The final area of challenge that warrants addressing is our ongoing relationship with Premier as they build their orthopaedic service line. We have gained much influence in this because our group is the easiest to deal with and has the highest skill level in the system. This provides us with much bargaining power. The challenge is managing our own interest and balancing those of Premier. Having our lease agreement and our own PT and X-Ray restricts us from branching out in geographic areas where Premier wants to expand. The challenge or opportunity lies in becoming more nimble and mobile which may involve selling off PT and X-Ray to allow them to expand.
In the area of faculty recruitment and retention, we are recruiting a spine surgeon, Mark Kayanja, MD. PhD., who is close to signing. There is a large need in Premier for a dedicated orthopaedic spine surgeon and we should be able to make his practice successful in a short time period. We hope to have him start September 2014.
On the other hand, our affiliation with Greg Stover has been difficult and deteriorating. Due to a poorly written contract, his assimilation into the group has been strained. He had a successful profitable practice in an area where we did not have anyone working. Through mismanagement his continued relationship with WSP is in jeopardy, likely to end at the end of 2014 and likely to result in a payout.
In Summary, 2013 has been a very challenging year. I think we have weathered much of the storm that occurred with the dismissal of our business director and I think we are headed in the right direction.
