Scope of the ProblemLifetime risk of a person with diabetes developing an ulceration as high as 25%

108,000 lower extremity amputations performed secondary to diabetes complications in 2014. This is the most common cause of non-traumatic lower extremity amputations.

▪Demonstrate movement of the toe “up” and “down”

▪Have the patient close their eyes and tell you which direction you are moving the toe

▪Failure to identify the correct direction indicates a loss of proprioception

◦ Foot is dorsiflexed at the ankle

◦ Hammer strikes the stretched Achilles tendon just proximal to the calcaneus

◦ Normal elicits a small amount of plantarflexion

Patellar Reflex◦ Knee hangs with gravity flexion off table

◦ Hammer strikes patellar tendon between patella and tibial tuberosity

Autonomic Neuropathy

Denervation of dermal structures leads to decreased sweating causing dry skin, cracking, thick and brittle toenails, dysregulation of temperature

Structural Deformity and Limited Joint MobilityLead to focal areas of high pressure

Muscle Wasting – “Intrinsic Minus Foot”Foot deformities are more common in diabetic patients due to atrophy of the intrinsic musculature responsible for stabilizing the toes
