The essential elements of the digital health revolution include wireless devices, hardware sensors and software sensing technologies, microprocessors and integrated circuits, the Internet, social networking, mobile/cellular networks and body area networks, Health IT (aka health information technology), genomics, and personal genetic information.

The lexicon of the emerging Digital Health market is extensive and includes all or elements of mHealth (aka mobile health and includes Apps), Wireless Health, Health 2.0, eHealth, e‐Patient(s), Healthcare IT / Health IT (information technology), Big Data, Health Data, Cloud Computing, Quantified Self, Wearable Computing, Gamification, TeleHealth / TeleCare / Telemedicine, Precision and Personalised Medicine, plus Connected Health.

Considering how we can treat cardiac events at point of care, We need to evidence it works

A DHI ‘Embedding New Practice’ project is underway in parallel with clinical and laboratory staff within NHS Grampian involved in the service integration of the new technology. 

A second line of academic enquiry will support this work by demonstrating how new point of care diagnostics may affect the clinical assessment of risk, and ultimately effect decisions as to where patients are best treated.

Early findings from these two activities will inform a second phase, in which the project team will attempt to develop the care pathways for applicable patients (avoiding A&E) on the strength of the point of care testing capability.

• Its not just elderly people though that will be using digital health, so we must ensure that we are looking to future proofing our services, preparing for the service demands that are coming from this next generation of digital natives

Scottish Parliament Motion: 5th March 2014

ExploratoryDigital Social Care 

DHI Capabilities Involved: Exploratory

External Participants: NHS24, the Scottish Centre for Telehealth & Telecare, the Association of Directors of Social Work, Turning Point Scotland, Renfrew Council, Edinburgh City Council, East Lothian Council, Midlothian Council, North Lanarkshire Council, The University of the West of Scotland.This Exploratory involves a series of high level, multi partner civic workshops. 

These consider the challenges facing social care and the emerging digital opportunities in the context of increasingly integrated health and social care services.

The first workshop was held in January 2014, and agreed the following themes for exploration and further discussion:

• Coordination • Community Capacity Building• Self Service & Accessibility• Ablement First

The group is about to undertake a mapping exercise to plot what is needed, what is already being done and what technologies are available in this context.

Experience LabNext Generation of Ambulance (Active) 

DHI Capabilities Involved: Exploratory, Laboratory and Factory

External Participants: The Scottish Ambulance Service, NHS Grampian, the Centre for Rural Health, Glasgow School of Art

The additional and changing nature of demand on Health & Care services has continued to accelerate in recent years. 

unscheduled care needs are changing, with a focus on their evolving needs and how they choose to access services. 

The Scottish Ambulance Service (SAS) has seen a steady shift in service demand, from an emergency focus to a more complex variety of demands more closely aligned with primary, community and social care services.

A major overhaul of the technical specifications for the design of ambulances is due to come into effect in 2016. 

This represents an ideal opportunity to take a step back and consider how the public and SAS staff have changed

Experience Lab: Next Generation AmbulanceWhat is the Challenge?

• 3 experience laboratory workshops are complete 

• These workshops involved a mix of group work, using product design methods, clinical scenario role playing, interviews and prototyping exercises.

What is the result?

• A set of recommendations bringing the user needs sharply into focus and strengthening confidence in this procurement programme

• Participating Clinicians felt valued and empowered

Factory: Atrial Fibrillation (AF) August ‘14

What is AF?

• AF is the most prevalent sustained heart rhythm disorder

• Just under 100k Scottish Citizens are diagnosed with AF 

• Estimated 30k citizens with AF have yet to be detected 

Who is in the DHI Collaboration?• AliveCor and their technology

• Five NHS boards in Scotland

• Twenty GP Practices 

• Two Scottish Universities

• Up to 10,000 Patients

• AF increases an individual’s risk of suffering a stroke by five times

• Millions of pounds of avoidable expenditure in the NHS in Scotland

• In 20 years there has been a 60% increase in hospital admissions due to  AF

What are we going to do?• Evaluate whether you can take accurate ECGs anywhere at low cost

• Determine the percentage of undetected AF 

• Conduct an academic & health economic analysis on long term consequences of undetected AF and stroke high prevalence
