• Rental Subsidy: Covers difference between 30% of income and 50% rent limit
• Target Population: Extremely low-income (<30% AMI) individuals with disabilities
• Support Services: Provided by HOME Choice or other Medicaid waiver program
• Unit Creation: Designated through LIHTC program or eligible existing units
• Unit Goal: 485 units by 2023, as noted in the Cooperative Agreement
• Tenant Matching: Referral process through SocialServe database

• Tenants are supplied to you—no need to seek individuals from target population

• Undergo a Phase I ESA and OHFA Environmental Review (unless already HUD assisted)
• Cannot already be 100% subsidized. Partially-subsidized projects do work.
• Cannot be 100% use restricted for disabled or senior (age 62+) individuals
• Cannot designate more than 25% of units for individuals with disabilities

• Must sign a 20-year Rental Assistance Contract (RAC)
• Must sign a 30-year Use Agreement for 811-assisted units
• Must use the 811 Tenant Selection Plan for 811 units
• Must fill next available unit with an 811 resident until the total units in RAC are met
• Must use HUD’s TRACS system for requisition of funds from OHFA

Referral Process - SocialServe
• Centralized database managed by Emphasys Software
• Emphasys will periodically check in with property management staff
regarding unit availability
• OHFA staff will input property information into the system 90 days prior
to placing in service
• Referral Agents need to complete SocialServe training prior to referring
eligible participants. Training TODAY

Referral Process – Tenant Selection Plan
• Specific to 811/ODMSD units, must not conflict with property TSP
• Only send one eligible tenant per available unit
• Up to two months of vacancy payments to ensure enough time for
participant transition
• If a prospective tenant cannot be matched with a unit in two months,
the available unit can be leased to a tax credit qualifying household
