permanent voluntary surrender of his certificate and license to practice as a certified public accountant in
the Commonwealth of Pennsylvania because he was found guilty, pled guilty or pled nolo contendere to
any felony, any crime an element of which is dishonesty or fraud, and/or any federal or state revenue law
or the revenue law of any foreign jurisdiction and he has been convicted of any felony. (04/21/16)
PA Architects Licensure Board
Bucks County Richard Edward Brown, Jr., registration no. RA006822X, of Doylestown, Bucks County, was ordered to
pay a civil penalty of $4,000 and to complete three hours of remedial education on the topic of Ethics for
Professional Architects because he engaged in the practice, or offered to engage in the practice of
architecture in this state, or used the title of architect or used any sign, card or device that implied that
such he is an architect, or is competent to engage in the practice of architecture, without holding a
currently valid certificate issued pursuant to this act. (05/12/16)
Philadelphia County George J. Baker, registration no. RA010583X, of Philadelphia, Philadelphia County, had a public
reprimand placed on its board record and was ordered to pay a civil penalty of $3,200 because he
practiced architecture in the Commonwealth of Pennsylvania without holding a current license/certificate
issued under the Act and affixed his seal on design and project-related documents during a time in which

Baker Architects, unlicensed, of Philadelphia, Philadelphia County, was ordered to pay a civil penalty of
$2,800, a public reprimand was placed on its board record, and it was ordered to cease and desist from the
practice of architecture until such time as the firm becomes licensed, because it engaged in the practice of

Out of State Donald Allen Deal, registration no. RA010058B, of Middletown, Delaware, was ordered to pay a civil
penalty of $3,600 and a public reprimand was placed on his permanent disciplinary record with the board
because he engaged in the practice, or offered to engage in the practice of architecture in this state, or

civil penalty and to cease and desist from practicing cosmetology and/or limited cosmetology on a license
that is lapsed, expired, or otherwise not current with the board because she practiced nail technology

Luzerne County Dejavu Beauty Center, Inc., license no. CB123925, of Hazleton, Luzerne County, was ordered to pay a
$1,100 civil penalty and costs of investigation of $51.25 and to cease and desist from operating without a
designated licensee in charge or owner present, from failing to maintain all areas of the salon in a safe and
sanitary condition, from failing to properly sanitize equipment, from failing to utilize a closed container
waste bin, and from possessing and using metal credo tools in the practice of cosmetology because they
operated without an owner or designated licensee in charge present; failed to have a garbage can with a
lid; failed to maintain all areas of the salon in a safe, orderly, and sanitary condition; failed to sanitize
tools, instruments, and utensils that come into contact with a client immediately after each use; and
operated as a cosmetology salon in a grossly incompetent and unethical manner through possession and

Philadelphia County Lee Lucien Fowlkes t/d/b/a Tha Hair Mechanics, license no. CB124382, of Philadelphia, Philadelphia
County, had a public reprimand placed on the salon’s permanent disciplinary record with the board, was
ordered to pay a $4,250 civil penalty as well as the previously assessed unpaid civil penalty in the amount
of $100, and to cease and desist from violating lawful board orders, operating on a suspended salon
license, employing unlicensed individuals, aiding and abetting unlicensed practice activity, and failing to
have an owner or designated licensee in charge present on its premises during inspection because the
salon violated lawful disciplinary orders of the board, operated a cosmetology salon in a grossly
incompetent, dishonest, and unethical manner by employing unlicensed employees, aided and abetted the
unlicensed practice of cosmetology, and operated without the salon owner or designated licensee in

reprimand placed on their permanent disciplinary record with the board, was ordered to pay a $2,250 civil
penalty, and was suspended for a period of three months with such suspension immediately stayed in
favor of probation because they operated as a salon in a grossly incompetent and unethical manner by
employing an unlicensed employee and by utilizing wax applicator sticks more than once, aided and
abetted unlicensed practice activity in the profession of cosmetology, failed to properly sterilize a piece of
equipment that has been rendered unsanitary, and failed to keep all areas of the salon in a sanitary

permits authorizing him to practice as a notary, because he failed to maintain custody of an accurate
chronological register of all official notary acts and failed to be familiar with the duties and
responsibilities of a notary public. (05/24/16)
Philadelphia County Jeannine M. Fitzgerald, notary commission no. 1190451, of Philadelphia, Philadelphia County, had her
notary commission suspended for a period of six (6) months, three (3) months of active suspension with
the remaining period of suspension to be stayed in favor of probation, ordered to attend and successfully
complete a six (6) hour notary practice and procedure course approved by the Secretary or his designee,
and pay a civil penalty of $500 and a probation assessment of $50 per month or $300 for the costs of
continued monitoring because she failed to be familiar with the duties and responsibilities of a notary
public by failing to include the required language for a verification, affidavit, acknowledgement and/or
acknowledgement by an attorney at law on the Assignment of Bail; in the alternative, she failed to know
through personal knowledge or have satisfactory evidence that the person appearing before her was the
party who executed the agreement; and she failed to maintain an accurate chronological register of all

30 hours of remedial education on the topic of Professional Personal Experience, focusing on obstetrics in
dogs, prior to the expiration of the probationary period because she engaged in incompetence, gross
negligence, or other malpractice, or the departure from, or failure to conform to, the standards of
acceptable and prevailing veterinary medical practice and violated board regulations by neglecting an

procedure logs and/or medical records; violated the board’s regulations with regard to responsibility to
clients and patients; and violated the board’s drug regulations which require that veterinarians only
dispense or administer drugs, including prescription drugs, that are within the expiration date specified by
