2. The SHS-DREC shall:

a. Conduct on-site validation as to whether or not the existing school buildings/classrooms or plans for classrooms to be constructed are in accordance with the technical specificationsstipulated in the latest DepED Educational Facilities Manual, the pertinent provisions of theNational Building Code and this DepED Order. A letter of notification shall be provided bythe Office of the SDS to the applicants of the action taken on their requests within fifteen (15)days upon receipt of such request;

b. Include in its evaluation the Staffing Complement/Position Requirement Analysis, forpurposes of determining the required number of teaching and non-teaching positions as wellas the funding requirements for PS, MOOE and CO, on multi-year basis (e.g. over a five-yearperiod), without prejudice to consideration of such application for the next/succeeding schoolyears; and

c. Submit recommendation, through the SOS, to the Office of the RD, through the AssistantRegional Director (ARD), on each proposal upon the completion of all requirements. Thedeadline for submission of recommendation shall be on June 30 of the current year. TheDREC, through the SOS, shall disapprove all applications/proposals that fail to comply withthe requirements; copy furnished the Planning and Research Section.

3. Moreover, for a more efficient preparation and submission of required documents, theUnnumbered Regional Memorandum dated January 16, 2020 re: Online Submission of DocumentaryRequirements for the Issuance of Permit to Offer the Senior High School Pro1,'fam or Additional SHSCourses in Public and Private Schools is hereto attached.

4. All pending applications and future applications shall be submitted to Eric N. Quillip,Education Program Supervisor, Division Senior High School Focal Person, who shall conductpreliminary perusal and evaluation of the documents and whenever necessary refer to appropriateunit/section/division for further technical assistance.

5. Focal person for Private Schools shall closely coordinate with the SHS-DREC in so far ascurricular offering and curricular requirements for SHS private school applications to streamline theapplication process for both private and public schools in the Schools Division.

6. No school is allowed to start offering the SHS Program without the approval of applicationfrom the Regional Office. It is therefore advised that all applications shall be submitted ahead of timeprior to the year of opening .

7. Travelling expenses of the SHS-DREC during the conduct of the on-site validation and ocularinspection shall be charged to Division MOOE/local funds subject to the usual accounting andauditing rules and regulations.

8. Immediate and wide dissemination and compliance of this Memorandum is desired.

THE ISSUANCE OF PERMIT TO OFFER THE SENIOR HIGH

SCHOOL (SHS) PROGRAM OR ADDITIONAL SHS COURSES IN PUBLIC AND PRIVATE SCHOOLS

For a more efficient system of submitting application documents to the Regional Office, the Online Submission of Requirements for the Issuance of Permit to Offer the Senior High School Program or Additional SHS Courses in Public and Private Schools shall be implemented to accommodate applications for SY 2021-2022.

However, the evaluation of documents for SY 2020-2021 has indicated poor compliance with the provisions of DepEd Order No. 51, s. 2015 for Public Schools and DepEd Memorandum No. 4, s. 2014 for Private Schools by most of the school applicants.

In view of this, all Schools Divisions are enjoined to call upon all public and private schools applying for SY 2020-2021 to resubmit their applications using the online system which included compliance indicators of the required documents.

Google Forms will be used as the platform to host the application for the Online Submission and can be accessed by the schools through their respective Uniform Resource Locators (URLs).

For the step by step procedure, from submission of documents to issuance of Permits, please refer to Attachment 1. Attachments 2 and 3 contain the templates for the Letter of Intent (School) and Endorsement Letter (SDO) respectively.

Attachment 1 - Step by Step Procedure

ONLINE SUBMISSION OF DOCUMENTARY REQIDREMENTS FOR THE ISSUANCE OF PERMIT TO OFFER THE SENIOR HIGH SCHOOL PROGRAM OR ADDITIONAL SHS COURSES IN PUBLIC AND PRIVATE SCHOOLS

Step 1. School accomplishes template for Letter of Intent and should observe the following: a. Only the prescribed template and file format should be used.b. No modifications should be made on the templatec. A school should submit all documents of more than one page in one continuous

document of multiple pages in PDF format. In case the file exceeded the allotted filesize, please contact the Division IT Officer (ITO).

d. All documents should be signed by the School Head.

Supply all required information such as Name of School, School Address, etc. and upload all documents indicated in the form. Uploading will fail if these documents are not in PDF format. Moreover, the school should take note of and strictly follow the indicators on how the documents should be prepared.

Step 3, As collaborators, the ITO and the SHS point person (SHSPP) of the Division has the same access rights for this particular Google form as its administrator. ITOs will assist

the SHSPPs in accessing the form. This would require submission of a Gmail account to the Quality Assurance Division (QAD), larry.samala@depedi:9V.ph, on or before January 20, 2020 which will be utilized for inviting the ITO and SHSPP as collaborators. This may require creating a new Gmail account. However, caution should be taken as collaborators may inadvertently delete attached files.

Step 4, The SHSPP evaluates all the documents and checks if: a. all documents required had been submitted (a PDF file may have been attached but is

not the required document);b. all documents were fully accomplished (some information may have been omitted);c. all documents conform with their respective indicators.

NB: (Please be guided by DepEd Order No. 51, s. 2015 for Public School applications and

DepEd Order No. 4, s. 2014 for Private School applications)

Steps. If everything is in order, the SHSPP and other members of the Division Review and Evaluation Committee conduct on-site validation of the school. Use the following links to download the requirements on SHS facilities, equipment, tools and materials:

included in bit/y/shsreql - requirements are located anywhere near theend of the document)

The SHSPP should keep notes and other supporting documents (checklists, inventories, tables, records, etc.) and prepare a short summary of the status of the school relative to its readiness to offer SHS course/s. These would be utilized during the validation of the school by the Regional Office.

Step 6. If the school has fully complied with all the physical requirements of the course, the SHSPP prepares an Endorsement Letter (Attachment 3) for signature by the SDS or his/her representative. Applications should not be endorsed to the Regional Office unless all documentary and physical requirements had been complied with.

Step 7. Once signed, the IT Officer scans the Endorsement Letter and uploads the scanned copy to Google Drive. He/she shares the scanned copy using the link sharing, "Anyone at Department ofEducation with the link". He/she then copies the link to this file and creates a link in the Google forms spreadsheet where the responses of the schools are kept. The link for the Endorsement letter should occupy the cell next to where the last attached document is located. Otherwise, this cell should contain the words "For Compliance, (indicate due date)".

Step 8. The QAO validates all documents. If everything is in order and the application had been endorsed by the SDO, the QAD appoints the schedule for validation of the school. The QAD and SDO SHSPPs conduct on-site validation before the end of the calendar year.

Step 9. If on-site validation yielded positive results, the QAD prepares the permit and approval letter and forwards the same to the Office of the Regional Director.

Step 10. The Regional Director signs the permit and approval letter and return it to the QAD.

Step 11. The QAD sends the signed permit, approval letter and all pertinent documents to the Records Section of the Regional Office and emails scanned copies of the permit and approval letter to the school. However, the school still needs to secure the original copies of these documents from the SDO.
