Also documented is the conversation I had with you, lvr Haga, In gaid conversation dated1Zg2A12,we walked down the hospital hall so that my dfive/s license could be examined. I allowed the youto hold and examine the lD but not io make copis citing lD theft concems. You appeared satisfied with the lDthen asked me to sign a fofm. I indicted that he had aiready signed this tofm.

Yo!,(Mr. Haga) said, "You made many strike-throughts to the form you signed." I said, "l did not agree wrth alloithe terms / conditions on the form so I struck through them." You requested that a form without stfikes besigned by me. I then indicate lwill not sign this form. You then ask me, "Does lredcted] have medicaid?" Irespond, "No'. You(Mr. Haga) then indicated lredactedl may be eligible for medicaid. I indicated, "l amresponsible for all of lredacted]'s obligations. Medicaid is not an option. In fact, medicaid would be a noraldi lemma for me."

Did you(Mr. Haga), director of patient access for the hospital, fail to convey to medical staff that Medjcaid i6 notan option? lf not, then why is medical staff curently billing Medicaid?

Documentation regarding this conversation and all related statements of non-consent will be put into anaffidavit.
