Does Convergence of 4Ps and PhilHealth Sponsored Program Induce Outpatient Care Utilization
For Illness/Injury?

I. ABSTRACT
Given the binding resource and administrative constraints, the government often faces trade-off between
depth and breadth of social protection programs coverage and intended impact. On one hand, the
government may opt to have comprehensive well-complemented/converged programs and have deeper
impact but only for a limited coverage/number of beneficiaries. On the other hand, more people would
have been reached if the programs are not converged/complemented but covering only limited aspect of
vulnerability. This study investigates whether there’s considerable impact from complemented/converge
4Ps and PhilHealth Sponsored Program (SP) than compared to having 4Ps alone or SP alone. Applying
propensity score matching methods to address the program selection bias and using the Family Health
Survey 2011 data, this study finds statistically significant positive impact on outpatient utilization (due to
illness/injury) from the convergence of 4Ps and SP. This confirms that there’s income effect from 4Ps,
price effect from SP, and information advantage from the Family Development Sessions requirement in
4Ps for the beneficiaries to utilize outpatient care (resulting from illness/injury).
II. INTRODUCTION: STATEMENT OF THE PROBLEM
During the Arroyo administration, there was explosion of disparate numerous and small patches of social
protection programs that seek to reduce vulnerability and impoverishment of the people. During the 2009
review of NAPC, a total of 66 identified social protection programs/projects were being undertaken by 18
government agencies and attached agencies. The NAPC National Anti-Poverty Program (2010-2016)
report describes these social protection programs having lack of coordination, small coverage, and heavily
controlled access by local politicians and service providers. (National Anti-Poverty Program (2010-2016)

The Aquino administration sieved through these social protection programs and scaled up several of
them. In the Philippine Development Plan 2011-2016, the Aquino administration explicitly declared
policy of “converging social protection programs for priority beneficiaries and target areas”, further
stating:
“Social protection programs shall converge in terms of strategies, target beneficiaries and
geographic areas, to fully maximize resources and create more impact to beneficiaries. The
Pantawid Pamilyang Pilipino Program shall form the backbone of this convergence, with
complementation from other social protection programs that ensure job generation,
livelihood/microfinance, CDD and asset reform.”

Given the political, administrative, and resource constraints, there is pronounced need to design the web
of social protection programs to efficiently achieve reduction of vulnerability and ultimately poverty and
inequality. Unfortunately, there’s dearth of impact evaluation about the convergence of these social
protection programs that can guide the policy-makers in recalibratings them.
Two of the most prominent and heavily funded social protection programs of the Aquino administration
are the conditional cash transfer program Pantawid Pamilya Pilipino Program (more commonly known
as 4Ps) and the social health insurance Sponsored Program of Philhealth (more commonly known as SP).
Pantawid Pamilya Pilipino Program (henceforth, 4Ps in the rest of this paper) is the flagship conditional
cash transfer program that aims to break the intergenerational dimension of poverty by investing in the
education and health of the household members. It started in 2008 with only 6,000 household
beneficiaries in four municipalities and two cities. The coverage was scaled up to 3 million households by
May 2012.
Since 1995, the Sponsored Program (as SP in the rest of this paper) of Philippine Health Insurance
Corporation (PhilHealth) is a free health insurance coverage of the poor households, mainly funded by
Local Government Units (LGUs) or co-financed by national government for some low-income LGUs.
Prior to 2011, poor households were mainly identified and selected by LGUs. Many conducted
evaluations found that there was high inclusion and exclusion errors in the identification of the poor.
(Chakraborty, 2012) In 2011, the national government addressed this leakage problem by financing only
the premiums of poor households identified by the nationally administered National Household Targeting
System for Poverty Reduction (NHTS-PR). Consequently, this implies the convergence of 4Ps and SP
since 4Ps beneficiaries are also in NHTS-PR. The actual implementation of automatic SP coverage of 4Ps
households started only on May 29, 2012 after the issuance of PhilHealth Circular No. 24 s. 2012 and
budget allotment in GAA for fiscal year 2012.
This study tries to pin down the impact of the convergence of 4Ps and SP on the outpatient care utilization
for illness/injury. We used outpatient utilization for illness/injury as main outcome variable to take
advantage of the short-term variability that’s possibly caused by annual coverage from SP and newly
scaled up 4Ps in 2011. The data used in this study is the Family Health Survey conducted in 2011
because, as earlier mentioned, 4Ps and SP were fully converged from 2012 onwards; hence, there’s no
longer control group composed of households that have 4Ps alone or SP alone post-2012.
III. IDENTIFICATION, PROGRAM BENEFITS DESIGN, AND BENEFITS DELIVERY
Pantawid Pamilya Pilipino Program (4Ps)
After selecting municipalities, 4Ps household beneficiaries are identified using Proxy Means Test (PMT)
based on the provincial poverty threshold. Eligible households are those with pregnant women and/or
with children between zero to 14 years old.
Cash transfers are based on two criteria. First is called the health grant which includes PhP 500 per
household every month. Health grant requires the households that: (a) children aged below five regularly
visit health center for DOH protocol checkups; (b) pregnant women regularly visit health center for
prenatal checkups; (c) all school-aged children undergo deworming at schools; (d) households with 0-14
years old attend the Family Development Sessions at least once a month.
The second type of cash transfer is the education grant which is PhP 300 per child per month (up to 10
school months a year) for maximum of three children. The condition is that the children should have at
least 85% of school days attendance every month.
The conditions are being monitored using the Compliance Verification System (CVS). CVS forms are
kept by school teachers and health facilities. These forms are verified bimonthly prior to each release of
cash. Cash is delivered through ATM cash cards through rural banks, over-the-counter LandBank
branches, or Globe G-cash remit outlets.

Prior to 2012, the premium is PhP 1,200 per identified household. The national government co-finances
90% of premium for five years with poor LGUs (4th, 5th, and 6th class) for the Sponsored Program
beneficiaries.
Starting 2009, the national government started to subsidize the premium of Sponsored Program household
beneficiaries which are identified poor by the National Household Targeting System for Poverty
Reduction (NHTS-PR). In 2010, the final list of 5.2 million NHTS-PR-identified households came out
and only 15% (or 800,000 members) of them are covered by Sponsored Program. (Chakraborty, 2012)
The automatic coverage of NHTS-PR identified households, which include 4Ps beneficiaries, was first
instituted through PhilHealth Board Resolution No 1571 s. 2011 and was given PhP 12 Billion from the
National Government budget in GAA 2012. The first wave of implementation happened in May 29, 2012
after the issuance of PhilHealth Circular No. 24 s. 2012.

Using the conventional choice model in economics, the increase in income from 4Ps is represented by
parallel shift in budget line upwards. The social insurance Sponsored Program (SP) benefit is represented
by decrease in effective price of outpatient care (outward pivot of budget line shown in the diagram
above) because the social insurance basically lifts the fees of outpatient consultation for the beneficiaries.
Note that the pivoted budget line is still downward sloping because there are still costs to the SP
beneficiaries of utilizing outpatient care like fare of going to the nearest PhilHealth-accredited public
health facility, and not to mention the forgone income. SP benefits are reflected as price because the
benefits, when availed, cannot be used to buy other goods somewhere else.
Depending on the nature of utility function of the selected beneficiaries, the conventional choice model
can give us intuition on what will be the overall effect from having both the income and effective price
changes.
If we assume that outpatient care for illness/injury is a normal and ordinary good, we expect increase in
outpatient care utilization for illness/injury from the increase in income from 4Ps and decrease in
effective price of outpatient care (as shown in graph below). In this way, the income effect surpasses the
substitution effect.
However, we cannot rule out the possibility when the substitution effect just cancels out the income
effect, making the outpatient utilization level equal as before. One possible scenario is that the income
effect from 4Ps is just enough to finance the requirements like sending the children to school,
transportation and forgone income costs in attending prenatal check-ups, Family Development Sessions,
deworming of children. Even with the increase in demand for these requirements (because they are
required in the first place), there may be no change in demand for outpatient care for illness/injury.
Another possibility is that program beneficiaries would substitute outpatient care for other goods as their
income increases and as effective price of outpatient care decreases. For one, the beneficiaries may
valuate outpatient care as inferior good. There are many possible scenarios that can fit in this case,
ranging from increase in consumption of vices to increase in demand for lumpy capital/heavy equipment
for family farm, all of these possible scenarios that lead to sacrificing of outpatient care for illness/injury.
Other important aspects that we cannot ignore in this study are the program specific specification, actual
implementation, and partial equilibrium effects (specifically the supply side ones). These other pathways
affect outpatient care utilization for illness/injury other than the income and price effects from 4Ps and
SP:

These listed other pathways can affect both the treatment and the main outcome so that the estimated
effect of the treatment may not be solely due to the treatment per se but from these confounding factors.
Hence, this study tries to address these sources of bias in the evaluation method.
V. EVALUATION DESIGN
The main evaluation method used in this study is the Propensity Score Matching to compute for the
Average Treatment Effect on the Treated. Matching treated and untreated samples based on exact
observed characteristics can be very tedious and almost impossible to implement because of limited
sample. This dimensionality curse of covariate matching is solved by Propensity Score Matching by
finding untreated matches to the treated sample based on probability of getting the treatment. (Rosenbaum
& Rubin, 1984)
Note that in principle, average treatment effect on the treated is just the difference between the expected
outcomes with and without treatment among those who actually participated in treatment (Caliendo &
Kopeinig, 2008):

Note that the subtrahend is not observable because we cannot have same person subjected to two different
treatments at the same time and observe different outcomes. What is observable is the outcome among
those who did not get the treatment. Transposing the subtrahend to the left hand side and subtracting both
sides the observable 𝐸(𝑌(0)|𝑇 = 0), we get

The probability of getting treated, after being conditioned on list of observed covariates X, should
be orthogonal (independent) to the potential outcomes

The probability of the samples of getting treatment should be positive and the probabilities of
treated and untreated samples should overlap in order to have matches
We only need the weaker assumptions of in computing the Average Treatment Effects on the Treated.
(Caliendo & Kopeinig, 2008)

Bias from variation of supply of outpatient care is minimized by including the province/city and
urban/rural dummy variables since primary health care is devolved to the LGUs.
The selection process of SP or 4Ps is controlled by including most of wealth asset variables used in the
proxy means test. Note that we cannot have all the variables used in proxy means tests because of data
limitation.
To control for the double endogenetity arising from having two program selection process, this study run
the evaluation methods in three different samples. In the first model, we use only households with SP and
compared households with both SP and 4Ps against those with SP alone. In this way, we are able to pin
down the wealth effect among households that experienced price effect from SP. In the second model, we
use households with 4Ps and compared those with SP and 4Ps against those with 4Ps alone. In this way,
we are able to pin down the price effect among households that experienced wealth effect from 4Ps.

VII. CONCLUSION
Having both 4Ps and SP sample against having SP alone increases outpatient utilization for illness/injury
by 4 percentage points. Having both 4Ps and SP against having 4Ps alone increase outpatient utilization
for illness/injury by 3 percentage points.
