Improve the effectiveness of transitions of care to reduce readmissions focusing on Heart Failure, Pneumonia, AMI, CABG, COPD and Hip/Knee through the appropriate use of emergency departments, referring hospitals, skilled nursing facilities, clinics and hospice.
