Crazy, Not CrazyEvaluating a Patient's Mental Status
“Altered Mental Status” tends to be a frequent complaint to EMS providers. Butwhat is normal, what is altered and how do EMS providers differentiate betweenthe two during the patient assessment. This course is designed to teach theparticipant proven accepted methods of evaluating mental competency andbeing able to incorporate this new method into a part of everyday treatment.
1. Review the brain and its structures2. Differentiate normal and abnormal in patients3. Discuss competency versus capacity in patients4. Discuss various forms in assisting in mental evaluations5. Review mental illnesses encountered in the field

Ben Hearn is twenty-two-year veteran paramedic with 12 years as an adjunct instructor for local community colleges and a state evaluator for NREMT testing. A native of West Tennessee, his passion is for the next generation of EMS providers to be better than he. Having studied and acquiring a Master’s degree, he has a desire to educate and encourage all in the EMS profession. He is also passionate about promotion of the advancement of his profession as he serves as the Tennessee Membership Coordinator for the National Association of EMTs.
Married to his wife, Cheryl, for 23 years; his daughter is a sophomore studying music at Bethel University and his son is a 6th grader in Medina.

The American Melting Pot-Discussing Cultural Compentency
Description:
America has always been a country of open arms to welcome those who wish to come here to her shores. It is however; inappropriate to conclude that these people groups should abandon their traditions and cultural values when they arrive. EMS personnel are often their first encounter with medical personnel and have the opportunity to establish the tone for their continued care. EMS personnel should be able to treat patients from other cultures without personal biases or any type of discrimination.
Objectives:
• Define “culture” and the need for cultural competence in EMS • Evaluate the change of immigration in the US and the local state • Discuss how to investigate cultural differences within an individual area • Discuss how those cultural differences might affect medical care • Analyze methods of overcoming possible impediments to delivering emergency care

Therapeutic Communication
Description:
Communication is an important component of the EMS system. Without it, all systems would breakdown, from dispatching information, to patient interviewing, to patient handoff to definitive care. As providers, one must recognize that there are bindings that limit, such as ethics, autonomy, understanding, active listening and then execution of what is understood. EMS practitioners need to understand and gain rapport with patients in order effectively initiate treatment. But to be a good speaker, being an active, effective listener is a must. Reviewing communication techniques, how to become an active listener and effectively communicate with various ages of patients is necessary
Objectives:
1. Discuss the ethical aspect of communication 2. Review the interrelation of empathy and communication 3. Discuss importance of gaining the patient’s confidence and barriers to that effect 4. Differentiate effective and ineffective communication 5. Interpret nonverbal patient behaviors 6. Distinguish common field interview mistakes

Documenting Patient Refusals
Description:
Refusals are a part of EMS care, whether it is a refusal of a procedure or complete refusal of care and/or transport. As providers, our focus should be on quality patient evaluation and assessment and be able to best assist the patient in determining if EMS services are in their best interest. In a crowded room, a thorough assessment of the patient, their condition and needs along with the advice of the practitioner is impossible. Having the ability to correctly document the scene, patient condition, assessment and recommendations for transport are a must in today’s quality improvement process, not to mention, keeping the provider out of attorney’s crosshairs. A thorough discussion of assessment, and documentation will be examined during this session.
Objectives:
1. Discuss the duty to act of the EMS practitioner 2. Discuss types of consent and the legal aspects 3. Discuss the patient’s capacity to make decisions and the legal aspects 4. Define the art of a patient assessment for refusals 5. Define the art of legal and accurate documentation for refusals

Opioids…America in Crisis
Description:
America’s love for opioids has been decades in the making. From injured soldiers dealing with pain to America’s workforce getting their life back, opioid medications have made an indelible mark. Although a new push from the federal government examining the opioid use, President’s since the 1970s have been dealing with the over-use of not only prescription opioids but illegally produced ones as well. Of late, the increased opioid use has created a need to address over-medicated patients; this has triggered the increased usage of Naloxone by lay-people and law enforcement personnel. This increased use has created additional concerns for EMS personnel. In addition to opioids, a brief look at new illegal medications on the horizon will be reviewed.
Objectives:
1. Define the history of opioids in America and the current causation of the overdose epidemic. 2. Describe briefly the basic properties of naloxone. 3. Demonstrate the effectiveness of law enforcement’s use of naloxone. 4. Critique law enforcements’ use of naloxone. 5. Examine personal protective measures needed. 6. Examine new illicit synthetic narcotics being created.
