BY SWITCHING TO THE FHO MODEL, YOU CAN EXPECT:
If you are already in a Patient Enrolment Model: If you have already enrolled your patients then you won’t have to repeat this process. Cirrus will manage
the batch roster transfer with the MOHLTC on your behalf, and support you with transitioning all previously-enrolled patients from one model to another.
If you are not working in a Patient Enrolment Model:Once you join the FHO, you may begin enrolling patients to your practice.
WILL MY TRANSITION TO THE FHO DISRUPT MY PRACTICE? No. All necessary services to transition to the FHO will be performed by Cirrus with minimal disruption to your work fl ow or practice.
HOW WILL THE TRANSITION AFFECT MY PATIENTS?

Capitation Payments for each enrolled patient, adjusted for age and sex
Shadow Billing (15% of the fee code value) on the 126 comprehensive care “in-basket” codes provided to enrolled patients
Fee-For-Service Payments (100% of the fee code value) for complex care ‘out-of-basket’ codes to enrolled patients
Fee-For-Service payments (100% of the fee code value) for all services provided to non-enrolled patients
Special Premiums and Bonuses

WHO CAN FORM OR JOIN THE FHO?The minimum requirement to form a FHO is a group of three or more family physicians who may work under the same roof, or, may be geographically separated. The individual FHO physicians work together as a group to provide comprehensive primary care, extended offi ce hours, and on-call coverage to the group’s enrolled patients.
At present, the MOHLTC is restricting FHO entry per the conditions below:
1) New FHOs may be formed in areas defi ned as under-serviced by the MOHLTC only.
2) A physician may join an existing FHO in any area only if they replace a departing FHO physician and takeover their spot.

WHAT WILL MY REVENUE IN A FHO LOOK LIKE?After transitioning to a FHO, your revenue will consist of fi xed payments, Capitation Payments, and variable income from billings and bonuses.
