Coalitions – key points
✔Use an existing team if you have one; form a team if you don’t
✔Engage someone from target audience✔Where folks are
• 43% closer to existing partners• 29% no changes• 24% at least 1 new partner

Coalition ?s in Report
✔Did you create a new coalition or utilize an existing coalition?
✔ Is your coalition missing any key partners?✔Does your coalition include a community
member?✔Provide a brief success story (4-5 sentences)
describing specifically how a coalition member helped in the process.

Tips to Answering ?s
✔Be honest – in the long run it will help you✔“Include a community member”----not you
and professional colleagues; you need to include the population of focus
✔Tell a story – more powerful than you can imagine

Target Population ?s in Report

✔Who (individuals or institutions) or what influenced your target audience?
✔Describe any populations or demographics unique to your community.

Tips to Answering ?s
Our target population is 3rd graders. We chose this narrow focus because we have another project with 3rd graders in the county and have good partnership with entities that influence this population. Also from this needs assessment we learned that this is a common age for parents to send their children to after-school programs vs neighbors or friends. Over past 5 years we’ve had a growing number of refugee families move to our county.

Tips to Answering ?s
✔ Don’t say “We used our 2017 CHA data”…the end✔ Do say, “We used our 2017 CHA data plus
additional data relevant to food, nutrition and physical activity from County Health Rankings, Kids County Data, and YRBS. The additional data indicated a high proportion of children on food assistance programs and a very low proportion of adolescents eating the recommended amount of fruits and vegetables.”

Next I looked at information that was missing like citizen opinion and developed a survey. Some of the questions were about what prevents citizens from walking or buying fresh fruits and vegetables. Surveys are handed out at the front desk of partners (hospital, extension, YMCA, WIC, health center, etc.)
