What’s in it for you?

Are you eligible?

Our goal is to help you recover after your hospital stay.

If you can answer “yes” to these questions, this program may be for you:

1. Are you experiencing homeless-ness?

2. Are you 18+? 3. Are you recovering from a

post-acute medical issue that requires bed rest?

4. Can you safely manage your own activities of daily living (ADLs)?

5. Can you administer medication on your own?

6. Are your mental health symp-toms stable enough for you to exist in a group living environ-ment without creating risk or disruption?

7. Do you have no need for oxy-gen- or IV-maintenance?

The Convalescent Care Program helps people who left the

hospital and are expe-riencing homelessness; our program gives you a place to stay. We give you a place to rest. A place to get better. As a guest in the Con-valescent Care Pro-

gram, you will receive medication education.

Care coordination. Wound care. You will be linked to housing

and income resources. You will be linked to behavioral health and primary care services.

If you are a patient:

• Your medical provider needs to enroll you.

If you are a medical provider:

1. Call our Nurse Care Coordinator (Nurse): 410-598-6758.

2. The Nurse will tell you if beds are available.

3. Prepare referral information: bit.ly/ConCareReferrals. Fax to 410-837-8020. The Nurse will review and determine if your patient is eligible.

Anthony’s story

Anthony was staying at a residen-tial employment program and actively seeking a job when he suffered a stroke that left him per-manently disabled in his mid-40s. He lost his spot in the program due to his inability to work, and was referred by a hospital to the Convalescent Care Program. Here, nurses helped him manage his new medical conditions and a social worker helped him adjust psychologically to being disabled at such a young age. Staff also helped him secure insurance. An-thony left the Convalescent Care Program for a transitional hous-ing program for individuals with disabilities.

Joseph’s story

Joseph was living under a bridge and working outside until pro-longed exposure to the winter cold resulted in frostbite on his hands and feet. He was referred by a local hospital to the Convalescent Care Program, where he was able to stay while getting the care he needed to avoid amputation and heal. Program staff referred Joseph to a transitional housing program and connected him to primary medical care at Health Care for the Homeless.

After you join our program, 1. You will be assigned a bed;

you can rest there 24 hours a day.

2. You will receive nursing services; these services are 12-hour-a-day.

3. Depending on your needs, you will be linked to housing and income resources. We will link you to mental health services and/or addiction services. We will link you to primary care.

4. You will recover from your medical condition. Your team will let you know when you can move to the shelter or other housing. You will complete the program!
