    Creating the Quilt of Continuity

    MILWAUKEE (MPA-PPR) Creating the QUILT of Continuity requires

    communication practices that are regular, respectful, engaging, recruiting, decision making,educational, innovative, hands-on, principle-centered, transparent and accountable for

    compliance at the census tract and neighborhood level. It is part of the All Hands on Deck, WE

    CAN initiative that demands re-defining, re-branding and un-trapping hidden talent and vetted

    businesses in the city for improved quality of life and economic development.

    MPA LLCs best practices compliance include being Culturally-Sensitive. No one is Exempt.

    Additionally, review of beneficiaries who have had an insurance plan but have NEW options for

    WELLNESS, require update services. This would include incentivizing and rewards for all

    beneficiaries. This would indicate a difference in overall knowledge of insurance carriers and

    insurance benefits. Therefore, a One-size fit all model is not acceptable.

    GROUND ZERO

    MPA LLC is looking at Ground Zeromodels in and out of the health care industry with mixed

    service delivery customers of implementation for metric review and unit of analysis in other

    fields to help ensure a positive launching for preparedness of the People Protection and

    Affordable Care Act (PPACA), October 1, 2013 and January 1, 2014.

    Wellness programs recognized by the PPACA are:

    DEMONSTRATIONMPA LLC Demonstration period is June 1, 2013 March 31, 2014 (305 days). The target groups

    are in the 7 neighborhoods (Amani, Halyard, Harambee, Hillside, Midtown, Riverwest,

    WAICO/YMCA), approximately 44,000 residential and commercial beneficiaries in

    Milwaukee. It also includes:

    practices of health care providers hospitals-clinics-home care and health practitioners doctors-nurses-specialists suppliers pharmaceutical organizations and insurance payers (private and public).
