for a period of three months subject to the terms and conditions specified by the Secretary; assessed a civil
penalty of $250, stayed in favor of successful completion of a live, in-person, six-hour notary practice and
procedure course during her suspension because she failed to maintain an accurate chronological registry
of all official notary acts, to be familiar with the duties and responsibilities of a notary public; and to be of

for a period of three months, with such suspension immediately stayed in favor of probation, subject to

assessment of $900 for costs of continued monitoring, and must successfully complete a live, in-person,
six-hour notary practice and procedure course approved by the Secretary because he failed to know
through personal knowledge or have satisfactory evidence that the individuals appearing before him were
the parties who signed the document; to maintain an accurate chronological registry of all official notary

Susquehanna County The Secretary of the Commonwealth accepted a Consent Agreement in which Michael Brainard of
Jackson, Susquehanna County, permanently and voluntarily surrendered his notary commission I.D. No.
1241365, along with any other licenses, registrations certificates or permits authorizing him to practice as
a notary because he failed to require personal appearance; to be of good character, integrity, and ability;
and to be familiar with the duties and responsibilities of a notary public. (08/11/16)
Out of State Carolyn A. McMahon, notary commission I.D. No. 1208251, of Sewell, New Jersey, permanently and
voluntarily surrendered her notary commission because she failed to take and subscribe the constitutional
oath of office, give a surety bond and record the bond, commission and oath of office in the office of the
recorder of deeds for the county in which the notary maintains an office at appointment or reappointment;

PA Board of Nursing Allegheny County Megan Elizabeth Wegley, license no. RN557576, of Coraopolis, Allegheny County, was indefinitely
suspended for no less than three years retroactive to June 10, 2016, based on violating the terms of a
