Introducción
• Objetivos terapéuticos en IAM:– tratar de alcanzar una reperfusión completa,
rápida y sostenida de la arteria relacionada haciendo hincapié en lograr una reperfusión a nivel tisular miocárdico.
• Estrategias : reperfusión farmacológica con drogas fibrinolíticas y la reperfusión mecánica mediante angioplastia coronaria.

Grading flow – TIMI system
TIMI 2 – partial reperfusion
The contrast material passes across the obstruction and opacifies the coronary bed distal to the obstruction. However, the rate of entry of contrast into the vessel distal to the obstruction and/or its rate of clearance from the distal bed are perceptibly slower than its entry into and/or clearance from comparable areas not perfused by the culprit vessel (e.g., the opposite coronary artery or coronary bed proximal to the obstruction).
TIMI 3 – complete perfusion
Antegrade flow into the bed distal to the obstruction occurs as promptly as into the bed proximal to the obstruction and clearance of contrast material from the involved bed is as rapid as from an uninvolved bed in the same vessel or the opposite artery.

Depende • disponibilidad de una sala de cateterismo y de un
equipo experimentado en el tratamiento percutáneo del IAM ;
• tiempo de evolución desde el inicio de los síntomas hasta la presentación en el centro de atención;
• la capacidad y rapidez para lograr un traslado desde centros de baja complejidad a centros de alta complejidad;
• Extensión del IAM ; • Compromiso hemodinámico del paciente.

• Trombolisis: reduce en forma significativa la mortalidad en el IAM
• Limitaciones: 1. Algunos pacientes no son candidatos por el alto riesgo de
complicaciones hemorrágicas– no tener EKG conclusivos del diagnostico2.Permeabilidad coronaria <70% .3 Complicaciones : ancianos ECV hemorrágico.
Efectividad de angioplastia primaria en las primeras seis horas de este.
Éxito: 80 y 97% (mayoría > 90% ).

PCI + STENT
Estudios: GRAMI, el FRESCO, el ZWOLLE, el PASTA, el PAMI-STENT trial y el STENTIM-2.
• Conclusiones:– mayor porcentaje de pacientes con flujo final timi 3,– menor incidencia de eventos isquémicos adversos, – mejor sobrevida libre de eventos a largo plazo,– menor necesidad de una nueva revascularización del vaso
tratado y disminución en la rata de reestenosis angiográfica.
R/ siempre y cuando la anatomía lo permita, se deberá utilizar un stent para tratar el vaso relacionado.

tengan síntomas de isquemia miocárdica.b. Tengan indicación para terapia trombolítica y vayan a ser

Complicaciones inherentes 1. Infarto agudo que persista con isquemia o tenga
recurrencia tornándose refractaria al tratamiento.2. Presencia de un defecto septal ventricular o de una insuficiencia valvular mitral.3. Cuando se sospecha una formación de pseudoaneurisma. 4. Falla cardíaca congestiva o hipotensión, o ambas, durante el tratamiento médico intensivo.5. TVR o FV.

Claves para la realización de una PCI primaria optima:
1. Disponibilidad permanente de laboratorio y personal entrenado de cardiología intervencionista.
2. Excelente coordinación entre los equipos médicos y paramédicos de los servicios de urgencias y cardiología intervencionista.
3. Utilización de terapia farmacológica adecuada, desde urgencias, como por ejemplo ASA, BB, heparina e inhibidores de gp IIb/IIIa.
4. Realizar una técnica angiográfica rápida. (tiempo puerta-balón menor de 90 minutos).
5. Mantener un tiempo de coagulación activado (ACT) entre 200 y 300 segundos.
6. Utilizar, siempre que la anatomía lo permita stent en el vaso responsable.
7. Sólo tratar el vaso relacionado con el IAM.
