HIGH POTENTIAL ACCIDENT DESCRIPTIONS -
Description: (What, when, where, how)
Electrician was fatally electrocuted when he came into contact with obscured live PT connections
Root cause:
PT installed in cable compartment and fed from Bus bars
Corrective Action Taken:
Install PT on Buss bar compartment
Preventative Action Taken: (action taken to stop this type of incident from happening again)
• Ensure visibility of all live parts in a breaker• Audit all switchgear to engineer out similar
installation• Implemented SURETECH personal alerts
DetailsALL people can now SEE whether electrical equipment is live or dead
• Use on any voltage from 110Vac to 760kVac
• The Personal Alert is worn on the wrist like a watch, fastened with Velcro straps, OR mounted on
telescopic link stick with
standard crown adapter
• No user controls, automatically switches ON or OFF, entering or leaving the electric field
• Buzzers beep and ultra bright LED flashes at a rate that increases from 0.7Hz when approaching live
equipment, to
20Hz near live conductor
• Filters for corona, partial discharge and electrical noise make the HV/PA stable under wide range of
electrical noise
• Types available:
• H – High skill, higher voltages (white)
• M – medium skill, med voltages (grey)
• L – Low skill, lower voltages (black)

HIGH POTENTIAL ACCIDENT DESCRIPTIONS -

Electrician was exposed to an ARC flash when he withdrew a MV breaker that was still supplying load to a faulty transformer
Root cause:
Breaker failed to trip – Breaker stickySome of the breakers have not been maintained
Corrective Action Taken:
Servicing of all BreakersSwitching done under supervisionInstall cable live indicatorsInstall remote open close
Preventative Action Taken: (action taken to stop this type of incident from happening again)
Install mechanical interlock to prevent withdrawal of breaker if not fully open
Roll out new type of Arc clothing to the group

Other initiatives• Revision of Group Electrical COP and associated Standards for:
• Supply of Equipment standards (Transformers, Switchgear, Motors, Cables Etc• Repair of Equipment standards• Installation and operating standards
• More frequent transformer oil testing • Aging transformers• On degradation of oil on previous tests
• Improving transformer protection requirements
• Switchgear maintenance to increase in line with application and operations
• Thermal imaging scans for Hot connections being doing now for a number of Years
• Color coding of Reticulation drawings is very useful on busy drawings
• Energy Efficiency has high weighting currently looking at Photo Voltaic generation opportunities
