Read 19 new articles per day which appear in medical journals
19 x 2 hrs (Critical Appraisal) = 38 hrs per day
Davidoff F et al. (1995)EBM; A new journal to help doctors identifythe information they need. BMJ 310:1085-86.
For General Physicians to keep current:

Evidence is anything used to determine or demonstrate the truth of an assertion.
Scientific evidence is evidence which serves to either support or counter a scientific theory or hypothesis.
In scientific research evidence is accumulated through observations of phenomena occur in the natural world, or created as experiments in a laboratory
What is Evidence?
![Page 8: Evidence Based Dentistry EBD](https://reader033.vdocument.in/reader033/viewer/2022061517/56815a03550346895dc7506c/html5/thumbnails/8.jpg)
The extent to which one can be confident that an estimate of effect or association is correct (unbiased).
What is ‘level of evidence’?
![Page 9: Evidence Based Dentistry EBD](https://reader033.vdocument.in/reader033/viewer/2022061517/56815a03550346895dc7506c/html5/thumbnails/9.jpg)
Level of Evidence Type of Study
1a Systematic reviews of randomized clinical trials (RCTs)
1b Individual RCTs
2a Systematic reviews of cohort studies
2b Individual cohort studies and low-quality RCTs
3a Systematic reviews of case-controlled studies
3b Individual case-controlled studies
4 Case series and poor-quality cohort and case-control studies
5 Expert opinion based on clinical experience

“Evidence-based medicine is the integration of best research evidence with clinical expertise and patient values”
- Sackett & Straus
What is Evidence-Based Medicine?
![Page 11: Evidence Based Dentistry EBD](https://reader033.vdocument.in/reader033/viewer/2022061517/56815a03550346895dc7506c/html5/thumbnails/11.jpg)
The conscientious, explicit & judicious use of current best evidence in making decisions about the care of individual patients.
In other words; EBD is sometimes described as doing the right thing, for the right patient, at the right time
What is EBD?
![Page 12: Evidence Based Dentistry EBD](https://reader033.vdocument.in/reader033/viewer/2022061517/56815a03550346895dc7506c/html5/thumbnails/12.jpg)
clinically relevant research Unbiased research Reproducible research often from the basic sciences of medicine especially from patient centered clinical
research
By best research evidence
![Page 13: Evidence Based Dentistry EBD](https://reader033.vdocument.in/reader033/viewer/2022061517/56815a03550346895dc7506c/html5/thumbnails/13.jpg)
We mean the ability to use our clinical skills and past experience to rapidly identify each patient’s unique health state and diagnosis, their individual risks and benefits of potential interventions, and their personal values and expectations.
By clinical expertise
![Page 14: Evidence Based Dentistry EBD](https://reader033.vdocument.in/reader033/viewer/2022061517/56815a03550346895dc7506c/html5/thumbnails/14.jpg)
We mean the unique preferences, concerns and expectations each patient brings to a clinical encounter and which must be integrated into clinical decisions if they are to serve the patient.
By patient values
![Page 15: Evidence Based Dentistry EBD](https://reader033.vdocument.in/reader033/viewer/2022061517/56815a03550346895dc7506c/html5/thumbnails/15.jpg)
When these three elements are integrated, clinicians and patients form a diagnostic and therapeutic alliance which optimizes clinical outcomes and quality of life
![Page 16: Evidence Based Dentistry EBD](https://reader033.vdocument.in/reader033/viewer/2022061517/56815a03550346895dc7506c/html5/thumbnails/16.jpg)

1. Clinical skills & experience2. Integrating evidence with patient
values
EBP vs traditional practice
Similarities:
![Page 19: Evidence Based Dentistry EBD](https://reader033.vdocument.in/reader033/viewer/2022061517/56815a03550346895dc7506c/html5/thumbnails/19.jpg)
EBP vs traditional practicedifferences
EBP traditional practice
Uses best evidence available
Unclear basis of evidence
Systematic appraisal of quality of evidence
Unclear or absent appraisal of quality of evidence
More objective ,transparent & less biased
More subjective,opaque & biased
Greater acceptance of uncertainty
Greater tendency to black & white conclusion
![Page 20: Evidence Based Dentistry EBD](https://reader033.vdocument.in/reader033/viewer/2022061517/56815a03550346895dc7506c/html5/thumbnails/20.jpg)
Advantages & disadvantages of EBD
![Page 21: Evidence Based Dentistry EBD](https://reader033.vdocument.in/reader033/viewer/2022061517/56815a03550346895dc7506c/html5/thumbnails/21.jpg)
1. Treatments & diagnostic test are being used at a time when their effectiveness is approved
2. EBD prevent using ineffective treatment methods, so it will probably decrease charges
The usefulness of EBD
![Page 22: Evidence Based Dentistry EBD](https://reader033.vdocument.in/reader033/viewer/2022061517/56815a03550346895dc7506c/html5/thumbnails/22.jpg)
3. It help us updating our knowledge continuously instead of reading lots of irrelevancy & unreliable literature, so time saving.
4. It helps policy makers through development of clinical guidelines', providing them with enough documents & evidence
The usefulness of EBD
![Page 23: Evidence Based Dentistry EBD](https://reader033.vdocument.in/reader033/viewer/2022061517/56815a03550346895dc7506c/html5/thumbnails/23.jpg)
5. Instead of teaching students current standard treatment method, it teach them how to find the best current therapy for their disease
6. EBM promote evidence instead of persons authority
7. It decreases medical errors
The usefulness of EBD
![Page 24: Evidence Based Dentistry EBD](https://reader033.vdocument.in/reader033/viewer/2022061517/56815a03550346895dc7506c/html5/thumbnails/24.jpg)
First, the need to develop new skills in searching and critical appraisal can be daunting, although (as we pointed out above) evidence-based care can still be applied if only the former has been mastered and directed toward pre-appraised resources
What are the limitations of EBD?
![Page 25: Evidence Based Dentistry EBD](https://reader033.vdocument.in/reader033/viewer/2022061517/56815a03550346895dc7506c/html5/thumbnails/25.jpg)
Second, busy clinicians have limited time to master and apply these new skills, and the resources required for instant access to evidence are often woefully inadequate in clinical setting.
![Page 26: Evidence Based Dentistry EBD](https://reader033.vdocument.in/reader033/viewer/2022061517/56815a03550346895dc7506c/html5/thumbnails/26.jpg)
