A Tie-up Model Incorporating Medical Care, Nursing Care, and Social Welfare to Cope with Super-Aging
Advantages of the current project
This project begins with a survey of currentconditions experienced by the elderly peopleof the area. It will be implemented based onthe survey as follows, respecting to thegreatest possible extent the opinions andmotivation of local medical personnel.• Devise a medical care model featuring tie-
ups between visiting medical care, nursingcare, rehabilitation services, and treatmentclinics, in support of life at home for theelderly.
• Establishing medical care and socialwelfare information networks usingadvanced ICT technology (promotingremote treatment/ “cloud” files/ automationof hospital services, etc.).
• Survey and assess advanced medical andsocial welfare initiatives in Japan andoverseas, and study feasibility in theregion
