Diffuse large B cell lymphoma
Lymphoma defined by a neoplasm of large
lymphoid B-cells with nuclear size equal to or
exceeding normal macrophage nuclei or more than
twice the size of a normal lymphocyte that has a
diffuse growth pattern
Diff diagnosis : Large B-cell lymphoma with a follicular
pattern : Follicular 3B
Prov Entity : large B cell Lymphoma with IRF4 break

» Le Grade III est considéré comme une
variante de lymphome diffus à grandes
cellules B
– Evolution en 2 à 5 ans du grade I au grade III.
L ’Alpha-interferon pourrait contrôler les
grades I et II.

• Peut être introduit dans le compte rendu mais pas une entité à part (20 à 35% des cas)
• Prédominance dans les DLBCL activés B par rapport aux DLBCL GC
• A ne pas confondre avec les DLBCL double hit qui sont caractérisés par une translocation des gènes MYC et BCL2et/ou BCL6, qui de plus sont observés dans les DLBCL GC
• Pas de corrélation entre MYC à plus de 40% en IHC et réarrangement de MYC

– Ne répondent pas au traitement classique de type
R-CHOP
