Par-ents or guardians (or students eighteen years of age or older) who do not want their student involved in all, or some, of the classes may obtain an opt-out request form from the principal. Immunizations/Physicals Students in Kindergarten through 6th grade must have 5 DTaP (diphtheria, pertussis and tetanus). 4 Polio, 2 MMR (measles, mumps, and rubella), 3 Hepatitis B Vaccines and 2 Varicella Vac-cines. The fi rst MMR/Varicella must be on or after the child’s fi rst birthday. One dose of polio must be after the child’s fourth birthday. All 7th-12th grade students are required to have one dose of Tdap regardless of the interval since the last dose of Dtap or Td. All kindergarten students must present a signed physical exam form at the time of enrollment. Physicals are also required for students involved in sports and for those students new to the district that are 8 years of age or younger. all fi rearms sales.1. All long gun buyers must be 18 years of age.2. All hand gun and home protection guns must be 21 years of
age.3. All buyers must fi ll out form 4473, day of the auction.4. All buyers must pay in full, day of the auction for all purchases.5. Please read the form 4473 carefully, there have been changes.6. Please answer all questions thoroughly, write legibly while
fi lling out the form (or we will have to ask that you re-do the form).
7. All Firearms will have a form 4473 (except black powder guns)
When calling FBI NICS for checks one of the following will happen:
1. Proceed, you are approved and can take the fi rearms with you.
2. Delayed, you cannot have your fi rearm until approved (within 3 business days) - Write phone number at top of form 4473.
3. Denied, if you are denied, all your money will be refunded less a $100.00 processing fee.
4. Firearms left after 30 calendar days will be re-sold for storage.
NON-RESIDENT BUYERSMissouri and Other bordering states must furnish a licensed
fi rearms dealer in your state, shipping will be C.O.D. to that dealer.
