I. Water Quality Monitoring .
A. Influent Monitoring
In accordance with the discharger's Time Schedule Order (TSO) No. R4-2002-0l09, the dailyand monthly wastewater flow can be found in the Maintenance Log located in Appendix E. l
B. Effluent Monitoring .
Wastewater effluent samples were taken by the Treatment Systems Operator, Barton Slutske andsubmitted to .the Regional Board. The monthly sample results are shown in Appendix B. Atabuiar summary of this data can be found on page 5.
D. Groundwater-Monitoring
Appendix A contains 'a, site 'plan shOWing the location ofall the groundwater monitoring wells:The original field reports, lab analyses and chain--of-custody forms for these samples are includedas Appendix C ofthis report. Per the request of the Regional Water Quality VV:ater Board twoadditional monitoring wells were installed, MW14A and IvfW14B. The shallow well (Mw14B),
, was found to be dry.
Groundwater monitoriIig was performed in accordance with Stone Environmental Inc. StandardOperating Protocol (SOP) 6.27.2: Wells were purg~dusing.a variable-speed Grundfos pump or adisposable bailer. Between three and five well volumes were purged. Grab samples were takenwith a disposl:!-ble bailer within 24 hours ofthe completion ofpurging, within the industry

The new treatment plant continues to run in startup condition. The Disinfection System continuesto be mqdified in order to brin~ the effluent into compliance.
One fan creating air to a portion ofthe treatment media failed creating elevated BOD sampleresults in the August sample. This fan was replaced allowing proper aeration to the entiretreatment system. The Pur-o-Tech disinfection system is being modified by the manufacturer toensure proper functioning oftheir system. Specific information can be found in The QuarterlyMaintenance Report ,found in Appendix E. , .

I. Water Quality Monitoring
A. Influent Monitoring
In accordance with the 'discharger's Time Schedule Order (1'SO) No. R4-2002.:01 09; the dailyand monthly Y'ast~water flow can be found in the. Maintenanee Log located in Appendix E.
B. Effluent Monitoring
Wastewater effluent samples weretaken by the-Treatment Sy~tems Operator: Barton Slutske andsubmitted to the Regional Board. The monthly sample results Were asked for numerous times but
.were not provided 'bythe reporting.
D..Groundwater Monitoring
. Appendix A contams' a site plan showing the l~cationofall the groundwater monitoring wells.The original field reports, lab analyses and chain-of-custody forms for these samples are included'as Appendix C ofthis report. Per the request ofthe Regional Water Quality Water Board twoadditional momtoring wells were installed, MW14A and MWI4B.· The shallow well (MW14B),was found to be dry.
Groundwater monitoring was performed in accordance with Stone Environmental Inc. Standar4Operating Protocol (SOP) 6.27.2. Wells were purged·using.a variable-speed Grundfhs pump or adisposable bailer. Between three and five well volumes were purged. Grab samples were takenwith a disposable bailer within 24 'hours ofthe completion ofpurging, within the industrystandard sampling window. A tabular summary of this data can be found on page 6.

This report provides infonnation to fulfill the monitoring and reporting provisions of thedischarger's Monitoring and Reporting Program CI-8342, a part of the currently issued WasteDischarge Requirements (vVDR) Order No. R4-2002-01 08.

I. 'Vater Qualitv Monitoring
A. Influent Monitoring
In accordance with the discharger's Time Schedule Order (TSO) No. R4-2002-01 09,the dailyand monthly wastewater flow can be found in the Maintenance Log located in Appendix E.
B. Effluent MonitOl'ing
Wastewater effluent samples were taken by the Treatment Systems Operator, Barton Slutske andsubmitted to the Re~onalBoard. The monthly sample results aTe shown in Appendix B. Atabular summary of this data can befou~d on page 5.
D. Groundwater Moni'toring
Appendix A contains a site plan showing the location of alI the ,groundwater. monitoring wells.The original field reports, lab analyses and chain':'of-custody forms for these samples are includedas Appendix C of this report. Per the ,request of the Regional Water Quality 'Water Board twoadditional monitoring wens wer~ installed, MWl4A and MW14B. The shalIow well (MW14B),was found to be dry.
, .GrOl.mdwater monitoring was performed in accordance with Stone Environmental lJ.1C. StandardOperating Protocol (SOP) 6.27.2. Wells were purg.ed using a variable-speed Grundfcis ,pump or adisposable bailer. Between tlJ.I:.ee and five well volumes were purged. Grab samples were taken,witb a disposable bailer "vith1J1 24 hours of the completion of purging, within the industrystandard sampling window. A tabular summary of this data can be found on page 6.
As acknowledged in the TSO, actua'l vertical separation to groui1dwater wi11 not be analyzed orconsidered in violation until the treatment facility has achieved full upgrade as outlined in theWDR.

III. General Provisions For Reporting
The new treatment plant continues to run in startup condition. The Disinfection System continuesto be modified in order to bring t~le effluent into compliance.
A Work Plan for future improvements has been submitted. Thy work plan can be fOlmd inAppendix E. .

This report provides infonnatj.on to fulfill the'monitoring and reporting provisions of thedischarger's Monitoring and Reporting Program CI-8342, a part of the currently issued WasteDischarge Requirements (WDR) Order No. R4~2002-0108. '

I. Water Quality Monitoring
A. Influent Monitoring
In accordance withthe discharger's Time Schedule Order (TSO) No. R4-2002-0109, the dailyand mon~lywastewater j10w can be found in the Maintenance Log located in Appendix E.
B. Effluent Monitoring
The monthly sample results were asked for numerous times but were not provided in time for thesubmitta'r of the 1st quarter report. 2nd
, 3rd and 4th quarter wastewater effluent samples 'were takenby the Treatment Systems Operator, Barton Slutske and submitted to the Regional Board. Themonthly sample results are shown in Appendix B. A tabular summary of this data can be found
,on page 5.
D. Groundwater Monitoring
Appendix A contains a site plan showing the location of all the groundwater monitoring wells.The original field reports, lab analyses and chain-of-custody forms for these samples are includedas Appendix C'ofthis report. Per the request of the Regional Water Quality Wat~rBoa,rd twoadditional monitoring wells were installed"MW14A and MWI4B. The shallow well (MW14B),was found to be dry. ' ,
Groundwater monitoring was performed in accordance with Stone Environmental Inc. StandardOperating Protocol (SOP) 6.27.2. Wells were purged using a variable-speed'Grundfos pump or adisposable bailer. Between three and five well volumes were purged. Grab samples were takenwith a disposable bailer within 24 hours of the ,completion ofpurging, within the industry'standard'sampling window. A tabular summary of this data can be found on page 6.

II. General Provisions For Sampling and Analvsis

All chemical, bacteriological, and toxicity analyses were conduded by Pat-Chern Laboratories ofMoorpark, CA, a facility certified by the State Department ofHealth Services EnvironmentalLaboratory Accreditation Program (ELAP). Laboratory analyses followed United StatesEnvironmental Protection Agency (USEPA) standard methods as noted on the lab reports in theAppendices.
III. General Provisions For Reporting
A Wor;k. Plan for future improvements has been submitted. The work plan can be found inAppendixE.
