is a personalized transplant learning
and resource platform.
This step-by-step guide will help you
learn the main features of MTP
and how to get started in minutes.
Here are the 5 steps towards
successfully using MTP:
Step 1: Register for your MTP account | 3

Go to: http://www.mytransplantplace.com/Click on the Register New User button.

MTP Registration Form
Fill out the MTP registration form with your First Name,Middle Name, Last Name, Email Address and Password.
Click on the Register button when you have completedthe form.

Once you have completed the registration form, you willbe sent an email to the email address that you provided.
Click on the activation link in your email or paste the linkinto your web browser bar. If you haven’t received your
email veri�cation, please check your junk/spam folder.

Return to: http://www.mytransplantplace.com/Enter your Email Address and Password youused during registration. Then click on the Sign In button.

To customize your My Transplant Place experience complete your MTP account pro�le. Start by selecting
who you are. Every user must have their own account. Please fully complete the form.

Select your MTP age group
Your options are Adult (>18) and Pediatric (<18).

Select your MTP language
Your options for Adult are English, Arabic, Hmong,Spanish and Somali. Your option for Pediatrics is Englishwith more languages coming soon!

Select your Transplant Organ
Your options are Heart, Intestinal, Islet, Kidney, Liver, Lung, Pancreas and Pancreas & Islet. *You may select multiple organs if applicable.

If you know your speci�c transplant information;Transplant Evaluation Start Date, Transplant Surgery
Date,Transplant Doctor, Transplant Surgeon andTransplant Coordinator, please complete. *You may select multiple Transplant Doctors if applicable.

Agree to the Terms and Conditions
Select I Agree to the Terms and Conditions.To view the Terms, click on the Click here text.
Click on the Save button to complete your pro�le.
