CONTRACTOR will be audited by COMPANY at the end of yard WORKSITE activities. CONTRACTOR will be paid if audit score target fixed by COMPANY is achieved. The audit will take into account the SITE installation work preparation.

1. SHOC form should be socialized in every TBM/Project Mass Meeting by Safetyman/Safety Officer/HSE Coordinator (HSE Personnel).
2. The minimum SHOC form are 25 copy sheets per HSE Personnel and it should be given for all of worker per work area.
3. SHOC form should be collected base on minimum target 100 SHOC in every week and every HSE Personnel.
4. SHOC Analyzed should be prepare daily SHOC form minimum are ± 300 copy per day.
