3. ALL ELEVATIONS ARE BASED ON NAVD-88.
4. THE CONTRACT PLANS PROVIDE A GENERAL DEPICTION OF THE EXISTING CONDITIONS
THAT EXIST WITHIN THE WORK AREA. PRIOR TO BIDDING, THE CONTRACTOR SHALL
CONDUCT ALL SUCH SITE INVESTIGATIONS AND FIELD VISITS NECESSARY TO
DETERMINE THE ACTUAL FIELD CONDITIONS THAT MAY IMPACT HIS WORK PLAN AND
COMPLETION OF THE WORK.
5. THE CONTRACTOR IS ADVISED THAT THERE ARE LIKELY TO BE UTILITY CONFLICTS TO
BE RESOLVED DURING CONSTRUCTION OF THE WORK. UTILITY CONFLICT RESOLUTION
IS A PART OF THE BASIC WORK OF THE CONTRACT. THE CONTRACTOR SHALL BE
RESPONSIBLE FOR LOCATING AND PROTECTING ALL EXISTING UTILITIES THAT MAY BE
IMPACTED BY NEW CONSTRUCTION.
6. FIELD CONDITIONS MAY NECESSITATE CHANGES TO THE HORIZONTAL ALIGNMENT AND
VERTICAL GRADE OF PROPOSED UTILITIES. THIS MAY BE REQUIRED TO
ACCOMMODATE ACTUAL EXISTING UTILITY LOCATIONS OR OTHER LOCALIZED FIELD
CONDITIONS. THE CONTRACTOR SHALL PRE-PLAN THE INSTALLATION OF NEW UTILITY
LINES AND NOTIFY THE ENGINEER OF RECORD OF ANY REQUIRED REALIGNMENT
PRIOR TO THE WORK BEING COMPLETED.
7. THE CONTRACTOR SHALL PROVIDE AT LEAST 48 HOURS NOTICE TO THE VARIOUS
UTILITIES COMPANIES IN ORDER TO PERMIT THE LOCATION OF EXISTING
UNDERGROUND UTILITIES IN ADVANCE OF CONSTRUCTION. CONTACT UTILITIES
NOTIFICATION CENTER AT "CALL SUNSHINE" (800) 442-4770.
8. THE CONTRACTOR SHALL REPLACE ALL EXISTING PAVING, STABILIZED EARTH, CURBS,
DRIVEWAYS, SIDEWALKS, FENCES, MAILBOXES, IRRIGATION LINES, SIGNS, AND OTHER
IMPROVEMENTS WITH THE SAME TYPE OF MATERIAL THAT WAS DISTURBED DURING
CONSTRUCTION OR AS DIRECTED BY THE ENGINEER, TO EQUAL OR BETTER THAN THE
CONDITION THAT EXISTED PRIOR TO THE START OF CONSTRUCTION.
9. CONTRACTOR SHALL MAINTAIN A CLEAR PATH FOR ALL SURFACE WATER DRAINAGE
STRUCTURES AND DITCHES DURING ALL PHASES OF CONSTRUCTION, AND ENSURE
THAT CONSTRUCTION/RESTORATION HAS NOT CHANGED THE CURRENT DRAINAGE
FLOW PATTERNS.
10. CONTRACTOR SHALL PROVIDE ALL WARNING SIGNALS, SIGNS, LIGHTS, AND FLAG MAN
AS REQUIRED IN THE PROJECT SPECIFICATIONS AND STATED IN THE F.D.O.T. "MANUAL
ON TRAFFIC CONTROL AND SAFE PRACTICES."
11. CONTRACTOR SHALL MAINTAIN A SET OF PLANS WITH CURRENT FIELD CHANGES
MARKED THEREON AND SHALL DELIVER THESE PLANS TO THE ENGINEER UPON
COMPLETION OF CONSTRUCTION. FINAL PAYMENT SHALL NOT BE MADE UNTIL THIS
REQUIREMENT HAS BEEN SATISFIED
12. THE CONTRACTOR SHALL NOTIFY THE ENGINEER IMMEDIATELY WHEN CONFLICTS
BETWEEN DRAWINGS AND ACTUAL CONDITIONS ARE DISCOVERED.
13. SHOP DRAWINGS SHALL BE PROVIDED FOR ALL MATERIAL, EQUIPMENT AND
PROCEDURES ASSOCIATED WITH THE INSTALLATION OF UTILITIES AS REQUIRED IN
THE SPECIFICATIONS. NO MATERIAL OR EQUIPMENT SHALL BE DELIVERED TO THE JOB
SITE, OR PROCEDURES PERFORMED IN THE FIELD UNTIL SUCH SHOP DRAWINGS HAVE
BEEN APPROVED BY THE ENGINEER AND ARE AVAILABLE AT THE JOB SITE FOR USE.
14. CONTRACTOR SHALL RETURN THE ENTIRE AREA DISTURBED BY CONSTRUCTION
ACTIVITIES TO THE ORIGINAL CONDITION AND TO THE FINISHED GRADES REQUIRED IN
ACCORDANCE WITH THE PROJECT SPECIFICATIONS.
15. THE CONTRACTOR SHALL PROVIDE ALL DEWATERING EQUIPMENT NECESSARY TO
KEEP EXCAVATIONS DRY AND SHALL PROVIDE ALL SHEETING, SHORING, AND BRACING
NECESSARY TO PROTECT ADJACENT STRUCTURES, UTILITIES, EXISTING PAVEMENT,
OR TO MINIMIZE TRENCH WIDTH, ALL IN ACCORDANCE WITH THE DETAILS AND
SPECIFICATIONS.
16. CONTRACTOR TO COORDINATE RELOCATION OF FP&L POLES WITH INSTALLATION OF
ANY NEW, RELOCATED OR OTHER BURIED UTILITIES TO AVOID IMPACTS DURING
CONSTRUCTION OR HAVING TO HOLD FP&L POLES.
17. CONTRACTOR SHALL VERIFY ALL DIMENSIONS AND ELEVATIONS PRIOR TO
COMMENCEMENT OF CONSTRUCTION. ANY DEVIATION IN PLAN INFORMATION SHALL BE
REPORTED TO THE ENGINEER AND OWNER'S REPRESENTATIVE IMMEDIATELY.
18. CONTRACTOR IS REQUIRED TO OBTAIN FROM THE ENGINEER WRITTEN APPROVAL FOR
ANY DEVIATIONS FROM THE PLANS AND/OR SPECIFICATIONS.
19. CONTRACTOR SHALL BE RESPONSIBLE FOR MAINTAINING TRAFFIC AND USAGE OF THE
EXISTING STREETS ADJACENT TO THE PROJECT. ALL TRAFFIC MAINTENANCE CONTROL
SHALL BE IN ACCORDANCE WITH FLORIDA MANUAL OF TRAFFIC CONTROL AND SAFE
PRACTICES FOR STREET CONSTRUCTION, MAINTENANCE, AND UTILITY OPERATIONS
AND/OR FDOT MAINTENANCE OF TRAFFIC PLANS, AS APPLICABLE. TRAFFIC CONTROL
OPERATION PROCEDURES SHALL BE SUBMITTED TO OWNER FOR APPROVAL PRIOR TO
BEGINNING CONSTRUCTION.
20. A PRE-CONSTRUCTION MEETING SHALL TAKE PLACE WITH COLLIER COUNTY PRIOR TO
THE START OF CONSTRUCTION. ANY REQUIRED PRE-CONSTRUCTION MEETING WITH
FDOT SHALL ALSO TAKE PLACE PRIOR TO CONSTRUCTION, IF REQUIRED.
21. THE REVIEW AND APPROVAL OF THESE IMPROVEMENT PLANS DOES NOT AUTHORIZE
THE CONSTRUCTION OF REQUIRED IMPROVEMENTS WHICH ARE INCONSISTENT WITH
EXISTING EASEMENTS OF RECORD.
22. CONTRACTOR SHALL BE RESPONSIBLE FOR OBTAINING COPIES OF AND COMPLYING
WITH ALL PERMITS OBTAINED BY THE PROPERTY OWNER, AND POST ALL PERMITS AT
THE JOB SITE.
23. THE CONTRACTOR IS RESPONSIBLE FOR OBTAINING A NATIONAL POLLUTION
DISCHARGE ELIMINATION SYSTEM (NPDES) GENERAL PERMIT FROM THE UNITED
STATES ENVIRONMENTAL PROTECTION AGENCY. ACCORDINGLY, THE CONTRACTOR IS
ALSO RESPONSIBLE FOR CREATING, IMPLEMENTING, AND MAINTAINING A STORM
WATER POLLUTION PREVENTION PLAN PER THE REQUIREMENTS OF THE NPDES
GENERAL PERMIT.
24. THE CONTRACTOR SHALL ACCURATELY PLOT THE LOCATIONS AND DEPTHS OF ALL
IMPROVEMENTS INSTALLED ON A FINAL SET OF RECORD DRAWINGS WHICH SHALL BE
DELIVERED TO THE ENGINEER.
25. THE CONTRACTOR SHALL BE RESPONSIBLE FOR REMOVAL AND DISPOSAL OF ALL
TREES, BRUSH, STUMPS, DEBRIS AND EXISTING INFRASTRUCTURE NOT REQUIRED TO
REMAIN IN THE PROPOSED CONSTRUCTION AREA. WHERE SO DIRECTED BY THE
OWNER, TREES AND VEGETATION WITHIN THE CLEARING LIMITS MAY BE PROTECTED,
LEFT STANDING, AND TRIMMED TO PREVENT DAMAGE TO LIMBS AND ROOTS DURING
CONSTRUCTION.
26. ALL MATERIALS AND CONSTRUCTION METHODS USED FOR THE PROPOSED
IMPROVEMENTS SHALL CONFORM TO THE APPROVED TECHNICAL SPECIFICATIONS
PROVIDED BY THE DESIGNER OR ENGINEER OF RECORD AND UTILITY PROVIDER, AND
ALL FEDERAL, STATE AND LOCAL REGULATIONS.
27. CONTRACTOR SHALL VERIFY ALL DIMENSIONS AND ELEVATIONS PRIOR TO
COMMENCEMENT OF CONSTRUCTION. ANY DEVIATION IN PLAN INFORMATION SHALL BE
REPORTED TO THE ENGINEER AND OWNER'S REPRESENTATIVE IMMEDIATELY.
28. CONTRACTOR IS REQUIRED TO OBTAIN WRITTEN APPROVAL FOR ANY DEVIATIONS
FROM THE PLANS AND/OR SPECIFICATIONS FROM THE ENGINEER AND OWNER. ANY
DISCREPANCIES ON THE DRAWINGS SHALL BE IMMEDIATELY BROUGHT TO THE
ATTENTION OF THE OWNER AND ENGINEER BEFORE COMMENCING WORK.
29. COLLIER COUNTY, FLORIDA POWER AND LIGHT, CENTURYLINK, COMCAST, AND/OR
OTHER ENTITIES HAVE EXISTING UTILITIES ADJACENT TO AND/OR WITHIN THE
PROPOSED CONSTRUCTION AREA. CONTRACTOR SHALL CONTACT THE RESPECTIVE
UTILITIES TO LOCATE THEIR FACILITIES PRIOR TO COMMENCEMENT OF
CONSTRUCTION.
30. THE CONTRACTOR SHALL PROTECT ALL EXISTING STRUCTURES, STORM DRAINS,
SEWERS, WATER MAINS, UTILITIES, AND OTHER FACILITIES IN THE CONSTRUCTION
AREA. THE CONTRACTOR SHALL REPAIR ANY DAMAGES DUE TO HIS CONSTRUCTION
ACTIVITIES AT NO ADDITIONAL COST TO THE OWNER.
31. THE CONTRACTOR SHALL NOT ALLOW ANY DISCHARGE OF WASTEWATER TO LANDS
AND/OR ADJACENT WATER BODIES OR STORM DRAINS.
32. CONTRACTOR SHALL MAINTAIN A CLEAR PATH FOR ALL SURFACE WATER DRAINAGE
STRUCTURES AND DITCHES DURING ALL PHASES OF CONSTRUCTION. THE
CONTRACTOR SHALL BE REQUIRED TO INSTALL ALL EROSION, SEDIMENT AND
TURBIDITY CONTROL MEASURES PRIOR TO CONSTRUCTION OF ANY COMPONENTS
ASSOCIATED WITH THE PROJECT. SEDIMENT CONTROL INCLUDES SILT DAMS, TRAPS,
EROSION PROTECTION, AND ANY OTHER APPURTENANCES NEEDED BUT NOT
NECESSARILY SHOWN ON THESE DRAWINGS.
33. CONTRACTOR SHALL PROVIDE PROTECTIVE MATTING, FUEL CONTAINMENT AND ALL
OTHER MATERIALS, EQUIPMENT AND LABOR TO PROTECT THE STAGING AREA DURING
CONSTRUCTION.
34. CONTRACTOR SHALL, PRIOR TO BEGINNING CONSTRUCTION, SUBMIT A "FUEL SPILL
PREVENTION PLAN" THAT SHALL CLEARLY INDICATE HOW FUEL SPILLS WILL BE
PREVENTED WHEN FUELING BOTH WITHIN AND OUTSIDE OF THE STAGING AREA.
35. THE CONTRACTOR SHALL BE RESPONSIBLE FOR COORDINATING WITH THE
APPROPRIATE JURISDICTIONAL BODIES AND UTILITY COMPANIES.

1. PRIOR TO COMMENCEMENT OF CONSTRUCTION ACTIVITY
APPROPRIATE EROSION CONTROL DEVICES SHALL BE INSTALLED TO

3. THESE BEST MANAGEMENT PRACTICES (BMP) ARE TYPICAL OF
REQUIREMENTS FOR SOIL EROSION CONTROL PER DIVISION 3.7 OF
THE COLLIER COUNTY LAND DEVELOPMENT CODE. THEY MAY NOT
CONSTITUTE COMPLETE REQUIREMENTS FOR COMPLIANCE WITH
REGULATORY AGENCIES AND SPECIFIC PERMIT CONDITIONS.
4. THE CONTRACTOR IS RESPONSIBLE FOR OBTAINING A NATIONAL
POLLUTANT DISCHARGE ELIMINATION SYSTEM (NPDES) GENERAL
PERMIT FROM THE UNITED STATES ENVIRONMENTAL PROTECTION
AGENCY. ACCORDINGLY, THE CONTRACTOR IS ALSO RESPONSIBLE
FOR CREATING, IMPLEMENTING AND MAINTAINING A STORM WATER
POLLUTION PREVENTION PLAN AS PER THE REQUIREMENTS OF THE
NPDES GENERAL PERMIT.
5. THIS PLAN IS A CONCEPT, PREPARED TO DEMONSTRATE ONE
APPROACH TO PROVIDING EROSION CONTROL FOR THE PROJECT,
