What should I bring to my appointment?
In order to expedite the care process and better serve you as our patient, bring
the following helpful medical information to your appointment:
Medical Center Map
List of all current medications and dosages
Current list of medical diagnoses and surgeries
Radiology images on disc and printed reports supplied by your local doctor
Any forms you were asked to bring with you

How do I prepare for my appointment?
Call (734) 615-0872 to create a patient portal account. This is important for
your care and through this online (web, mobile device) platform you can
receive information about your appointment, test results and communicate
with your care team. There may also be health questionnaires for you to
complete prior to your visit.
On the day of your visit, be sure to take all your regular scheduled
medications and have a good breakfast unless you were told not to eat
before a test. We do suggest you bring a snack, especially if you are diabetic
or if you have been instructed to not eat for testing.
What should I expect at my initial visit?
If you are being seen at the main hospital, parking can be a challenge so please
plan for extra time to find parking. After you park please follow the included
map to locate your clinic location.
Plan to be in the clinic for 2 to 6 hours. A typical visit will include:
pre-appointment testing- blood draw, ultrasound studies or ECG (a device
used to record the electrical activity of the heart),
the appointment with your provider
post- appointment testing (if needed).
You may need to complete pre-appointment testing before you come for your
visit. This gives your provider the opportunity to discuss results with you
during your visit. If this is needed, we will let you know and assist you in
finding a testing option close to your home.
Depending on the nature of your visit, there are times when you will meet with
trainees and/or advanced practice providers before meeting your surgeon.
After meeting with you, they will review your case with the vascular surgeon.
How do I get my medical test results?
If you have additional testing performed, you should expect to receive the test

results via the patient portal. Your surgeon’s office may contact you to review
results of testing if the surgeon feels it is necessary. If you would like to speak
to your care team you can call (888) 287-1082 to schedule time to review your
results and discuss your future care with us.
What kind of education will I receive?
During your appointment you will receive a blue and gold education folder
which includes information about your vascular disease and our contact
information. Additionally, when you check out the staff will print an after visit
summary which may have additional information.
If you are having surgery, you may visit a preoperative clinic ((locations at
Chelsea, Domino’s Farms, Northville and Taubman Center) where a provider
will give you specific written instructions for your surgery.
