Theprocessisasfollows:

1. FromtheCAUwebsite,navigatetoRegistrar,thenclicktheformssectionintheredboxandclickthe‘ApplicationforWithdrawalfromUniversityForm’link:

Registrar>Forms>ApplicationforWithdrawalfromtheUniversity

2. Fillouttheelectronicrequestform:

3. Uponreceiptofthecompletedelectronicrequestform,anofficialUniversitywithdrawalformwillbereturnedtoyouviaDocuSign.

4. ClickonReviewDocument;anagreementwillpopulateacrossthescreenforsignatureandverificationpurposes.

5. Click‘Iagree’touseelectronicrecordsandsignaturesandclickcontinue.

6. Clickstart,completetheformasguidedbyDocuSignandclickfinish.

7. AconfirmationofthecompletedUniversityWithdrawalwillbeemailedfromTyeAndrewsviaDocuSign.
