Vision
• To ensure the most vulnerable individuals living in Los Angeles County have the resources and support they need to thrive

Funding
Program period 2016-2020
• Program implementation started in 2017
Los Angeles County is one of 18+ counties in the
state to receive funding for 5-year WPC pilot
programs.
Funded by 1115 Medicaid Waiver
• Per participant per month funding for most programs
• Incentives for start up, reporting, and performance
Community Health Workers
• CHWs are carefully chosen community members who participate in capacitation (empowering training) so that they can promote health in their own communities. (Farquhar, Michael and Wiggins, 2005)
• Communities can be defined by race/ethnicity, geography, age, sexual orientation, disability status, other factors, or a combination of factors.

Tools of WPCOne Degree
• An online portal of resources that is open to the public. WPC uses One Degree to share and gain knowledge of community resources and organizations.
CHAMP• A care management platform that aims to integrate our WPC participant’s
health delivery services.
• Enables several functions to aid WPC CHWs to better serve WPC participants including: participant screening, eligibility, and enrollment; comprehensive needs assessment; care planning; streamlined note writing; metrics collection.
• One of the tools we are using to achieve our goal of county-wide data integration.
WPC Populations of Focus
WPC seeks to support Los Angeles County Medi-Cal beneficiaries with particular health challenges in six highly marginalized populations:
1. People experiencing homelessness2. People recently released from incarceration3. People with mental health issues4. People experiencing substance use disorder5. People with multiple medical conditions6. Women experiencing barriers to a healthy pregnancy
