MANAGEMENT OF CHANGE I have been devoted to skilling myself to a high level through project management (e.g. PRINCE2, 2009) and detailed learning in organisations, leaderships, strategy and change management in my MBA (2011-3) November 2017 Description Promotion of person-centred assets-based care Aim and measure To educate a local NHS workforce; measure, provision of a talk Corrections/actions The medical approach concerns "deficits", but it is equally

important to consider what people can do and why. This talk was organised for November 2019 at the Princess Alexandra Hospital, Harlow, for interested health and care staff of multidisciplinary nature. where I presented my thoughts on person-centred care (as a family carer) and the approach of 'home first' as welcome.

February 2017 Description Introduction of a MSc in dementia care and leadership Aim and measure To write a MSc course in dementia; measure, board approval by

the Health School Corrections/actions I designed a course curriculum together with other members of

a programme team proposing a new MSc in dementia care and leadership. This was approved by a senior management meeting by a private university in London. The course will need to be professionally rolled-out in the future.

I approached in 2016/7 the CEO of the health school of a private university about my wish to develop a MSc on dementia care and leadership based on the recently published document of core skills published by Skills for Health and Health Education England (https://www.hee.nhs.uk/our-work/dementia-awareness/core-skills). I organised a meeting with senior members of teaching staff, and I was able to influence and persuade them of the benefits in improving the skills of the workforce. We delegated roles in crafting a curriculum, and set realistic goals in writing the course modules, including learning objectives. The course was then approved by senior management.

January 2017 Description Improved identification of frailty in emergency admissions Aim and measure To raise awareness of frailty awareness; measure, analysis of a

frailty early warning score Corrections/actions I found as part of a statistical audit, under the supervision of a

geriatrics consultant at NHS London teaching hospital, that frailty early warning scores on acute admission can ultimately predict length of stay. This led ultimately to a business case for development of a 'frailty unit' and improved discharge planning.

In an audit of “frailty early warning scores”, I was able to demonstrate an association between frailty severity at admission and length of a hospital stay. This led to the development of a business case for the development of a frailty admission unit, providing an opportunity for better outcomes, better patient experiences, and better use of resources. That is indeed the “triple aim” described in “Leading change, adding value” from the Department of Health in August 2016. Early physiotherapy and occupational therapy can be instituted, as well as rationalisation of prescribing, achieving a goal of well-coordinated person-centred care. April 2016 Description Raising awareness of clinical nurse specialists in dementia Aim and measure Promotion of clinical nursing specialists in dementia; measure,

publication and discussionPromotion of clinical nursing specialists in dementia; publication and discussion

Corrections/actions The change required is a higher capacity in specialist dementia nursing. I published an article in Nursing Times, co-written by me and Head of Research, Dementia UK. This contribution has been discussed many times, including both nationally and in local meetings of the Dementia (UK) Admiral nurses steering committees.

June 2015 Description Improving community neurorehabilitation services Aim and measure To identify and address gaps in the care pathway; measure, my

report for Public Health England Corrections/actions I wrote a report for Public Health England which contributed

ultimately to evaluation of complex neurorehabilitation services in South London (2015). This report was used as part of a wider discussion of reconfiguring services by local commissioners and Public Health England. My recommendations therefore influenced the new care pathway.

I completed an initial evaluation of the enhanced neurorehabilitation service at Guy's and St Thomas's and King's College Hospital, in a voluntary capacity for Public Health England in 2015. In response to concerns about gaps in the care pathway for people with complex neurorehabilitation needs leading to long lengths of inpatient stay, local acute trusts were testing out an enhanced model of rehabilitation for people with more complex needs. I discovered from my quality improvement report using available evidence that access to a ‘navigator' early on in the pathway potentially could decrease length-of-stay and the cost of the overall pathway.
