VEHICLE PRE-START INSPECTION

1. INTRODUCTION
1.1 PURPOSE / OBJECTIVE
The purpose of this procedure is to ensure all Trackless Mobile Machines (TMM) used for work purposes within Richards Bay Minerals operations is inspected prior to use to ensure it is in a safe, serviceable, environmentally sound condition and complies with regulation 8.10.24, Machinery and Equipment of the MHSA
1.2 SCOPE
This procedure applies to all TMM used in operational areas within Richards Bay Minerals, including Mining Plants, exploration, Smelting, Processing and Logistics. It also applies to all TMM used on RBM Sites, RBM owned as well as contractor TMM, not exempted in terms of regulation 8.10.28 of the MHSA. A pre-use inspection is done in accordance with and by completing the vehicle inspection logbook for the type of TMM operated. The inspection must be conducted each time there is a change in the operator of the vehicle. Provision is made for the following vehicle logbooks. The contractor may use his own logo on the checklist. TMM is defined as any Trackless Mobile Machine, self-propelled or otherwise that is used for the purpose of performing mining, transport or associated operations on surface at a mine or surface at a mine and is mobile by virtue of its movement on wheels, skids, caterpillar tracks, mechanical shoes or any other device fitted to the machine but excludes rail bound

equipment, scraper winches, mono rail installations, static winches, draglines, winding machine installations, track mounted conveyors and equipment attached thereto.
2. RESPONSIBILITIES
2.1 Personnel carrying out work under this Procedure must be familiar with and comply with it in full.
2.2 All TMM drivers and operators are responsible for undertaking pre-start inspections of their equipment prior to the use of such equipment and to notify his/her supervisor of any deviations recorded.
2.3 The Engineer appointed in terms of section 2.13.1 or his/her duly appointed delegate in whose areas the TMM operate, will satisfy him/her that the TMM complies with all safety requirements and is maintained to a good standard.
3. STANDARDS
3.1 Any item in red is marked as faulty, the deviation must be reported to the direct
supervisor immediately and vehicle may not be used until the deviation is corrected or the Engineer has approved the use of the machine subject to specific provisions stipulated by him.
3.2 Where a deviation is marked for a “grey” item, the deviation must be reported to the direct supervisor, who will upon conducting a risk review, approve or disapprove the use of the vehicle
3.3 Any deviation or damage noted in the checklist must be reported and a notification
generated where repairs are required. The notification number must be recorded on the checklist.
3.4 The inspection must be conducted each time there is a change in the operator/driver
of the vehicle. 3.5 The checklist must be signed off by a supervisor before the end of the shift 3.6 The Engineer must review the pre-start checklist on a regular basis to satisfy him/her
of the safe operation and running of machinery in accordance with his/her letter of appointment
3.7 Records of the checklist must be maintained for a period of at least 4 months.
