Unit ted'1 DDPS SOFTWARE ERROR
Vendor designed softwarechange for Beacon was basedon latest copy maintained by
- vendor
The new scaling constantswere not permanently savedinto the master program or
sent to software vendor
A revision of the software wasinstalled that did not includethe new FW flow constants
$ 994 PWO changed span ofFW flow transmitters from
0 - 790.6"to
0- 800"
New FW flowscaling constantvalues were entered into DDPS
constant locations
PCIM 4029-196 provided thedesign for Installation ofBeacon system and itsconnection to DDPS
The 1994 changes to thefeedwater flow'constants were
overwritten

Uriit41 DDPS SOFTWARE ERROR
The Beacon PC/M scope waslimited to the hardware and DlD
NOT include the necessarychanges to the DDPS software
he PC/M DID NOT providepost modification testing
requirements for the DDPSsoftware
CM determined that the Fflowscaling constants were forthe old 0-790.6" transmitter
span.
The software V8Vspecified bythe software vendor challengedthe Beacon outputs but failedto check other aspects of theDDPS software such as the
secondary plant calorimetric.
Reactor startup for Unit ¹1Cycle ¹14
During repairs to a FW flowtransmitter, iCM found that
DDPS calculated reactor powerlevel was lower than actual
power by 0.63%.
ERT Long TermCorrective Action:
Develop generic V8Vrequirements to challenge allcritical attributes within both
UnitN and Unit¹2DDPS forall software changes.

Unit P2 DDPS SOFTWARE ERROR.
PC/M referenced generic V & V plandeveloped as corrective action to Unit¹1 event. Overlap between portions of
test plan was inadequate.
PC/M scope DID Includesoftware changes
Inadequate in-house DDPSsoftware expertise to catch
vendor error.
PC/M did not provide adequatepost modification testing for DDPS
software changes.
PC/M ¹96-147, supplement 1
provided design for rescalingof
FW flowtransmitter span from0 - 800"

DDPS vendor contracted torevise software.
FW flow Believable ValueProcessor scaling was not
revised.
New generic VBVof criticalDDPS attributes was
performed.Did not detect Incorrect FW

At 80% power ascensionplateau DDPS calorimetric was
determined to be 5% lowerthan manual calorimetric(This comparison was a

PMAIPRIORITIZATIONPROGRAM
Allopen Pcs to be sorted based on the followingprioritization system:
I. Musts
Includes all PMAIs required for continued plant operation.
Examples include: Written NRC/Regulatory commitments, QA responses,
justified personnel safety issues and load threatening/load reduction items.
II. Hi Wants
Includes PMAIs that have a quantifiable regulatory or economic benefit.
Examples include: PMAIs that impact plant indicators (OPWAs, PWOs > 12
months, old clearances, leak list, etc.), plant availability, plant/componentreliability or have a high economic payback.
III. Low Wants/Recommended Cancellation Candidates
Includes PMAIs that involve enhancements/betterment.
Examples include: Design enhancements, process improvements or items withlow payback. Includes PMAIs that are no longer an issue, should be
canceled based on management directive and are not cost effective.
Note the PMAIclosure/cancellation process shall be reviewed as part ofthiseffort to assure PMAI cancellation does not violate any existing plantprocedures.
Open PM&database reports have been generated and are currently under
Engineering review.
Plan and completion schedule to be reviewed with plant management.

INVENTORYREDUCTION GAME PLAN
~ Reorder Reviews (adjust min/max levels)
~ Vendor AgreementsIdentify areas were vendors are capable of maintaining inventory in lieu ofon site inventory, and establish agreements where possible.
~ Cons umablesCurrent St. Lucie inventory includes approximately one million in consumablesthat can be expensed and removed from inventory.
~ Inventory Technical AssessmentPerform technical review of inventory to determine appropriate stocking levelsand needs. This includes over max material. Write off unnecessary materialand place in investment recovery.
FastenersPerform technical review of stocked fasteners for consolidation, elimination ofredundant, and vendor supplied "standard" fasteners. Standard description willalso allow open market biding, resulting in at least a 20% savings per item.
PTN/PSL Common Simulator PartsCurrently PSL carries all PTN/PSL common simulator parts in the PSL inventory
($417,000). This burden should be shared 50/50 by both sites.
Extended Life RCP Seals (N-9000) 1998PSL carries $900,000 in seal cartridges, to facilitate RCP changes duringoutages. The N-9000 seal does not require cartridges, and can be rebuiltduring outages like PTN.
