Section I: Implementation

WelcomeHello and welcome to eClinicalWorks!

eClinicalWorks looks forward to assisting every client each step of the way through the installation process.

The purpose of this document is to help the client understand the different components of the Implementation Process. This document is broken down into four sections. The following describes what to expect in each section:

Section I: Implementation

Information on the complete Implementation process including timelines, integration features, hardware, and key milestone dates

Section II: Software Training

Overview of the Front Office, EMR, and Billing components of the system

Section III: Practice Information & Checklist

A checklist of features which the Project Manager will review with the client during an initial conference call, referred to as the Kick Off call. The client will be responsible for completing this section and returning to the Project Manager no later than 2 weeks after the Kick Off call.

Section IV: IT Information & Checklist

Valuable IT information when building the network and a checklist of information needed by eClinicalWorks to perform the Installation. The client is responsible for completing this section and returning to the Project Manager no later than 2 weeks prior to the assigned Network Check.

What is the Implementation Process?After the client signs the contract with eClinicalWorks, the Implementation team initiates the Implementation Process. The Implementation Process consists of the following activities:

1. Introduction phone call by the Project Manager to schedule a Kick-Off call (within 24 hours of receiving signed contract)

2. Receipt of the eClinicalWorks Welcome Pak via e-mail (within 48 hours of receiving signed contract), including the hardware requirements and credit card payment form

3. A Kick-Off Call with the Project Manager. He/she will help the client make choices with integration features and provide milestone dates as part of the Implementation timeline

4. A Billing Discovery call with a Billing Trainer. During this call, your Billing Manager will answer a series of questions to inform eClinicalWorks about your billing process.

5. Working on Integration Features

6. Performing Network Check

7. Performing Installation

8. Performing Training & Go-Live for client and staff

9. Performing Lab Integration (optional)

At this time the implementation process is completed, and the practice is up and running with eClinicalWorks.

Implementation TimelineThe following Implementation Timeline is a guideline, for a typical implementation. Each practice has its own individual requirements; therefore, it is only a suggested standard, not a guarantee.

Typically the Go-Live date is scheduled between 12-14 weeks after the Kick Off call.

Kick-Off CallThe Kick-Off Call starts the process of working with the client’s Project Manager and possibly other members of Implementation Team at eClinicalWorks. During this call the client will discuss integration features with their Project Manager and go over the Implementation Timeline including the Go-Live date.

eClinicalWorks suggests the following people be available for the Kick Off call:

From client’s practice:

At least one physician from your practice

Office Manager

IT Specialist

From eClinicalWorks:

Project Manager

Technical Support Specialist (as needed)

Coordinators for ExpressBill, Lab Integration, Clearinghouse or Data Migration (as needed)

After the Kick-Off Call is complete, the Project Manager will send a follow-up message by email recapping the implementation choices and schedule.

Billing Discovery Call

Shortly after the initial kick-off call with your Project Manager, we will conduct a Billing Discovery call with your Billing Manager, Office Manager and/or your Administrator.   This call will take approximately one hour and will be conducted by an eClinicalWorks Billing Trainer.  The purpose of this call is for our billing experts to understand your internal billing processes, policies and workflow.   The information gathered during this call will be used to prepare our on-site trainers to best train your practice on billing.   

Here is a sample agenda that should help you understand our goals and objectives for this call and also help you to identify which of your staff members should participate on the call.

Providers

How many providers do you bill for? What are their specialties? Do you employ any Ancillary providers? (NP, PA, Midwife) Are they credentialed for billing purposes?

Facilities How many office locations do the providers see patients at? What other locations do the providers see patients at? (Hospital, Nursing Home, Rehab

Center, etc.) Do the providers perform procedures at a free-standing Ambulatory Surgical Center? Do you operate a Medicare Independent Lab? Do you bill for any in-house labs? (rapid strep, urine dip, etc.) If you bill for labs, do you have a Medicare CLIA number? Do you operate an Urgent Care clinic? Is your practice a Medicare Approved Rural Health Center (RHC), Federally Qualified Health

Center (FQHC) or Community Health Center (CHC)? If YES – Do you split claims and bill both HCFA and UB92 claim forms? If YES – Do you bill for dental services? What system are you currently using for

dental? Do you use a Sliding Fee Schedule? Do you bill for Occupational Medicine?

Do you practice in a Medicare Health Professional Shortage Area (HPSA)? (Modifier AQ) Are Facility ID numbers needed by Medicare and BCBS when billing for hospital services? If YES - Do you split HCFA claims to a UB92 or another HCFA?

Insurances Do you use a Tax ID number for Billing?

If YES - Is it an Individual or Group number? Do the providers have their NPI numbers?

If YES - Do they have Individual or Group numbers? Do any of your office locations have Facility NPI numbers? Do you accept assignment on insurance claims? What is the estimated % of self-pay patients? Do you collect at time of service? Do you submit Workers’ Comp and Liability Claims? Do you sell products that are not covered by insurance?

If YES - Do you charge sales tax on these products? Do you participate with any Captitated Insurance plans? Does your state’s Medicaid have any special billing requirements or forms?

If YES – What are those requirements? Does any provider have more than one provider number issued by an insurance company,

determined by the facility where the services are rendered? Does your practice deal with Incoming or Outgoing referrals? Referring Provider – do you use the patient’s PCP or the provider who referred them to the

practice? Do you need the referring provider name & number (box 17/17A of the HCFA) on all

insurances? If YES – Do you use the UPIN number? If NO – What number is needed and for which insurance?

What Clearinghouse are you currently using? What Clearinghouse are you planning to use with eClinicalWorks? Do you currently submit electronic claims directly to any insurance payers (bypassing a

clearinghouse; not paper claims)? Do you currently receive insurance payments using Electronic Remittance Advice (ERA)?

Will you use ERA with eClinicalWorks? Which payers will you use ERA?

Do you currently print patient statements or send them electronically? Do you provide individual or family statements? Who are your top 10 insurances? (Please List) Are you currently having any major problems with any payers that you are submitting to

electronically?

Miscellaneous How knowledgeable are the providers about coding? Limited, Average, Above Average, Very

Knowledgeable? Will your billing staff create claims using the information contained in the Electronic Medical

Record? If NO – Will you be using a paper encounter form or superbill? Is the use of a paper encounter form temporary?

What is your billing workflow? If multiple locations, do you have a central billing office, or does each office do

their own billing? Is the process the same for both charges and payments?

Integration Features

DevicesMidmark Devices

eClinicalWorks has developed an interface with Midmark EKG, Spirometer and Holter devices.

If you purchase any of these devices from eClinicalWorks or an eClinicalWorks reseller, there is no charge for the installation of the interface on one computer.

If you already own a Midmark device or plan to purchase a Midmark device through any other vendor, eClinicalWorks charges $500 for the interface (first machine installation is free) and $100/hour for each additional machine installation.

Note: Midmark Return Policy: The device must not be opened and used (new condition) You will be subject to a 25% restocking fee You must pay for shipping back to Midmark Diagnostics Group

Welch Allyn Devices

eClinicalWorks integrates with several Welch Allyn Spot Vital Signs devices, EKG, and Stress Test machines.

eClinicalWorks charges $500 for the interface (first machine installation is free) and $100/hour for each additional machine installation.

For SPOT connectivity (the one with IR), you MUST purchase Welch Allyn Part# 4200-170 for each device. It connects to a serial port. The device communicates via RS232 protocol over infrared. IR ports on laptops use IrDA protocol. A USB to Serial converter can be purchased separately.

For Spot LXi device, you must purchase a standard USB cable to enable connectivity.

For a full list of devices eClinicalWorks has interfaced with, refer to the Customer Care Portal at http://support.eclinicalworks.com. You can receive a 5% discount on this purchase. When checking out, use eclinical as a checkout code.

MedicScaneClinicalWorks is compatible with the MedicScan OCR. MedicScan scans insurance cards as a document which can be attached to Patient Documents. The OCR reader gives the capability to scan in license information directly into the patient demographics. Appointment Reminder SystemseClinicalWorks has the ability to interface with some appointment reminder systems. These systems automatically deliver appointment notifications, recall notices, and/or medication reminders for patients. The following is a list of systems eClinicalWorks interfaces with:

There is no charge for the interface with any of the above systems. Please contact the company directly for pricing and any additional information. The client is responsible for enrolling directly with the company and notifying their Project Manager that they have enrolled.

ClearinghouseClearinghouses save you time and money by accelerating the process of claim submission. You can submit claims electronically to the clearinghouse, which, in turn, submits them electronically to insurance companies.

The Importance of Making an Early Decision for a Clearinghouse

You can choose from a variety of clearinghouses, depending on your geographical region, and other factors. The clearinghouse choice is important and changing clearinghouses mid stream is not recommended.

NPI Number

As of May 2007 all providers will be required to have a National Provider Identifier (NPI) number.

A health care provider can obtain a NPI number by submitting an application for an NPI-either on paper through the postal service or electronically over the internet. After the application is successfully processed, the healthcare provider will be notified of its NPI number.

The CMS website (www.cms.hhs.gov/hipaa/hipaa2 ) contains information on when, where and how the NPI application can be obtained.

Clearinghouse Choices

The most common clearinghouses currently used by eClinicalWorks customers are Emdeon and Gateway EDI, which use ANSI format. Should you choose another clearinghouse, please contact the clearinghouse directly to enroll. Your Project Manager should be notified of all approvals you receive from your specified clearinghouse. eClinicalWorks has certified many clearinghouses, and the list of certified clearinghouses continues to grow. For a complete list of clearinghouses, please refer to the Customer Care Portal at http://support.eclinicalworks.com.

Patient Statements

Printing Patient Statements

As part of the eClinicalWorks software, you are able to print Patient Statements. The statement can be configured with the Patient Statement cycle, dunning messages and number of days before the statement goes to collection. These statements are printed on 8 ½ x 11 paper and use Number 10 Left Window envelopes.

ExpressBill

ExpressBill is a patient billing service provided by ExpressBill, a subsidiary of Emdeon (formerly WebMD). This service prints, stuffs envelopes, and mails patient statements. The benefit of this service

is that the office staff can better utilize their time with other priorities versus printing and stuffing envelopes. The following details the fees for this service:

Print, Stuff, and Mail Letter/Statements

$ .56 for single-page letter/statement (includes outgoing envelope, postage, inner windowed remit envelope, and toll-free transmission line access)**

$ .17 per additional page

$ .03 per page of backside printing with variable data

If you already use ExpressBill or would like to add ExpressBill to your eClinicalWorks installation, inform the Project Manager and he/she will process the enrollment.

ePrescribing - SureScripts

ePrescription is an extra feature of eClinicalWorks that allows you to write and send prescriptions electronically to your local pharmacies. ePrescription uses a service called SureScripts that forwards prescriptions from the physician to the pharmacist. ePrescription can be set up for bi-directional communication so that pharmacists and doctors can send new prescriptions and refill requests electronically. SureScripts enrollment is done by eClinicalWorks. Please note the following information for SureScripts:

Physicians can sign up for ePrescribing and start using it as soon as they go live on eCW, if they wish.

For providers who currently use ePrescribing, the enrollment is fairly quick; your Project Manager will provide you with a Change of Vendor form to be filled out and returned for processing.

For providers who are new to ePrescribing, the enrollment may take from 4-6 weeks.

The fee for SureScripts is $25 per month, per provider; a sales agreement is required.

For enrollment purposes, please provide your physician information, including DEA # on page 23.

Patient PortalThe Patient Portal provides an innovative tool for sharing information and communicating with your patient. Certain medical record information such as lab tests, diagnostic imaging tests, immunization alerts, and prescription alerts are displayed on the home page on the Patient Portal. The home page is specific to a patient, and is based on the user name and a password used to log in to the Patient Portal.

The portal offers you the flexibility you need to set up your practice so that the information you want displayed on the Patient Portal reflects your communication needs. You set up lab profiles, reminder and alert messages, and standard visit types and times. You can define how messages are routed to your practice and the frequency with which information is uploaded and downloaded to and from the Patient Portal.

Functions of the Patient Portal are as follows:

Messages: Send/receive messages to/from doctor’s office. Received Faxed Medication request to Pharmacy

Appointments: Request a new appointment and view upcoming appointments

Medication: Request a prescription refill from pre-populated list of currently refillable prescriptions

Labs: Request a lab; review and examine lab reports

Statements: Examine current and past statements

Demographics: Enter or modify personal information and other demographic information

Reminders: Receive reminders from doctor’s office about when is the next immunization or lab is due

Referrals: List of prior referrals and ability to request new or existing referrals.

Receive an email for reminders, upcoming appointments and statements.

For further information on the Patient Portal options, please contact your project manager or the eCW sales department. A separate sales agreement is required for implementation of the portal.

Fee Schedule

Converting your current Master Fee Schedule to eClinicalWorks will involve a decision point for your practice. It will also present an opportunity for you to evaluate and modify your current fee structure or develop one that is better aligned to your practice needs.

eClinicalWorks offers a few options for loading a Master Fee Schedule into the system. Each of these options allow for alternative methods to make specific and/or global changes to your procedure fees.

Load a custom Fee Schedule – eCW requires the data to be in Excel format. To facilitate this,

your Project Manager will provide you with a fee schedule template. Up to 5 custom fee schedules will be loaded at no additional charge. More than 5 will be billed at standard Data Migration Rates.

Once in this format and prior to giving it to eCW, you may find it’s an ideal time and easier environment to edit or clean-up your existing fee schedule.

Load the Medicare Physician Fee Schedule - eCW will load the geographically adjusted Medicare Fee Schedule for the current year. eCW will then create a new fee schedule for you based on an agreed percentage of the Medicare allowable during training. Example: The practice sets their fee schedule at 150% of the Medicare allowable.

The practice can further modify or customize fee schedules by manually editing directly into the eCW application. Excel is required on the machine which will do any edits to the fee schedules. eCW will automatically update the yearly Medicare Physician Fee Schedule.

When changing your fees, please be aware that this may affect self-pay patients or patients with an 80-20 plan (indemnity). Finally, it is always wise to check the practice fees against the payer allowed amounts, especially for payers that comprise a large percentage of the practice’s revenue.

Data MigrationData migration involves extracting your data from your existing PM system and moving it to eClinicalWorks. eClinicalWorks can extract and migrate much of your existing data, including, but not limited to the following types of data:

Patient Demographics

Pharmacies

Referring Physicians

Insurances

Guarantor Information

There are two options eClinicalWorks offers clients for extracting the data for data migration. The first option, which most clients prefer, is having eClinicalWorks extract their existing system. The second option is having the client providing the data. The following lists what is required for each method:

eClinicalWorks Extracts

Inform eClinicalWorks of Application/System name

Practice must install TightVNC on the machine that will be used for data extraction.

Inform eClinicalWorks with an Admin username/password on system

Inform eClinicalWorks of location of data on system (path or directory)

Client Provide Data

Prepare data and supply in Excel format in eClinicalWorks’s Data Migration Template

Send file/s to Project Manager via email, CD, or FTP

Once the extraction is performed, regardless of method, the data is reviewed by eClinicalWorks to ensure that the data can be migrated successfully.

Important Note: eClinicalWorks cannot migrate binary, encrypted, or scrambled data

Important Note: eClinicalWorks does not migrate billing information

Standard data migrations are usually completed within 3-4 days. This time is spread over the course of your Implementation, including the many steps in which our Data Migration Team performs to ensure the integrity of your data.

Lab IntegrationeClinicalWorks can be integrated directly with many of the major labs such as Quest or Labcorp to transmit data electronically.

Interface

The lab interfaces may be Uni-directional or Bi-directional.

In order for eCW to have all the lab tests that your practice frequently transmits to the Lab Company, eClinicalWork’s Lab Programmer will be sending you a Lab Compendium Spreadsheet for you to forward onto your Labs. Please have your lab complete this template and return to your Lab Programmer.

Timeframe

The following general rules apply for lab interface lead times:

10-12 weeks lead time required from Go-Live Date for currently interfaced labs

Up to 6 months lead time required for hospital or local labs, in which eClinicalWorks has never interfaced with before

Cost

eClinicalWorks charges the lab for the interface and not the client.

For a list of labs that eClinicalWorks has developed interfaces with, please refer to the Customer Care Portal at http://support.eclinicalworks.com.

eClinicalWorks FaxingeClinicalWorks allows the client the ability to fax information such as prescriptions, progress notes, lab/diagnostic imaging orders, and results directly from the software environment.

Clients should do an analysis of the number of faxes sent and received on a daily basis. If you have multiple locations, this analysis should be done at each location. Client should monitor what type of documents they are sending out (referrals vs prescriptions) as well. If you plan to use SureScripts, this will decrease the number of faxes sent significantly as your prescriptions will be done electronically.

eClinicalWorks needs for the client to provide the provider(s) signature(s) in a .jpg format for training. Please save these signatures in a folder on your server entitled “eCW Signatures” and be sure to let your trainer know where to find this folder. Your trainer will set your system up to print the provider’s signatures for prescriptions, progress notes, referrals, etc. If you would like a copy of the default prescription format, please ask the Project Manager.

Refer to Section IV (FaxServer Requirements and Faxing Details) to see what is required for performing eClinicalWorks faxing.

HardwareProper hardware and network configuration is crucial to the success of your eClinicalWorks system. The network (including the server, workstations, tablets, laptops, PDA’s, modems, analog phone lines, printers and scanners) must be fully installed prior to the Network Check, so that during the Network Check eClinicalWorks can confirm all equipment specs and network speeds are within specifications.

eClinicalWorks provides every client with a Hardware Requirements document that states the minimum requirements needed for the network. The client should review this document with their IT specialist to ensure proper ordering or upgrading of equipment. For further information on the hardware refer to Section IV: IT Information & Checklist.

Once the client has determined all the components to their network, eClinicalWorks asks that you provide this information to your Project Manager for review. This way eClinicalWorks can make sure that all equipment meets specifications for the practice and nothing has been overlooked.

Network CheckThe Network Check is performed by eClinicalWorks approximately 6 weeks before training to ensure the network meets specifications and all hardware and devices are ready for Installation. eClinicalWorks remotely connects to the client’s network via TightVNC (a remote desktop sharing program) to perform this important step in the Implementation process. The following is a list of what is verified during the Network Check:

Check server power & network connectivity

Verify that all network components, machines, and software are configured correctly and in working order

Verify systems meet the minimum eClinicalWorks hardware specifications (processor speed, memory, hard drive space, operating system, routers)

Verify all Service Packs are installed

Test the scanner

Test the analog line, US Robotics Modem & FaxServer (no VOIP lines)

Verify all outgoing ports are open if part of a hospital network

Verify that you can FTP from other FTP servers

Verify that all machines are able to connect to the internet

Check for speed between the server and client machines (workstations, laptops, tablets)

Test the speed between multiple locations

Load setup files

The Installation team uses the information from Section IV: IT Information & Checklist of this document during the Network Check. It is crucial that the client completes and returns Section IV no later than two weeks prior to the scheduled Network Check. If this document is not received on time; the Network Check, Installation, and Training may be postponed.

Also, eClinicalWorks requires that the IT specialist be available at the practice during the Network Check, which typically lasts two hours.

InstallationThe installation of eClinicalWorks is performed 4 weeks before training. The eClinicalWorks Installation team will remotely connect to the client’s Network and install the software on the server and all the clients on the network via Tight VNC.

eClinicalWorks requires that the IT specialist be available at the practice during the Installation, which typically lasts between 6 – 8 hours.

Depending on the number of servers and client machines which require eClinicalWorks to be installed on, the installation could take more than one day. eCW can complete installations in one day when there is 1 server and up to 10 client machines. Additional clients will need additional time. eClinicalWorks will teach your IT individual how to install the client software.

Any hardware that is not ready for the installation will need additional time scheduled to complete. This additional time is billable. Please refer to the eClinicalWorks Labor Charge sheet for details on charges.

Following your Go Live of eClinicalWorks, you should attend a ‘Go Live Training’ webinar. This webinar is a free offering which covers Generic Templates, Patient Specific Templates, eCliniSense, Generic Alerts, Patient Specific Alerts, Telephone Encounters, Ordering and Reviewing Labs and Reviewing Documents.

Within the following weeks of your Go-Live date, eClinicalWorks will provide mandatory no charge Follow-Up Training sessions. The first one that is available is a Go Live Session. Please visit the Support Portal (support.eclinicalworks.com) for a scheudle and how to sign up for these sessions.

Additional sessions are available which are geared to billing – Claim Submission, Payment Posting & Statements, Collections & Billing Alert Recalls, and Billing Expert Session. Training OverviewThe eClinicalWorks training course is designed to instruct the client and staff on the use of the current version of eClinicalWorks EMR and Practice Management Software Suites. Part of the training includes hands-on scenarios, in which users are given the opportunity to practice using eClinicalWorks software.

eClinicalWorks does not recommend a full patient load during training. Work with the Project Manager to discuss the best scenario for the practice.

During training, eClinicalWorks recommends that doctor’s appointments be scheduled for double the appointment time (e.g. for a 15 minutes visit schedule 30 minutes). This will allow the doctor to have time to finish chart notes.

Allow time for personnel to get trained in the different areas if their roles overlap.

Trainers will bring in a Checklist for the Office/Practice Manager or Doctor to sign upon completion of training. Trainers in turn will give one copy to the practice and submit one copy to the Project Manager at eClinicalWorks.

Some practices have multiple locations or would like to access eClinicalWorks from home. A VPN connection is required if the practice has multiple locations but there are other options available. The following are some options to consider:

FaxServer Requirements and Faxing DetailsThe FaxServer must be installed on it’s own dedicated workstation. Refer to the Hardware Specifications document for workstation recommendations.

The FaxServer requires an external US ROBOTICS 5686E modem or an internal US ROBOTICS 5610B modem and a dedicated Analog Phone Line. If you choose to use the external US ROBOTICS 5686E modem, you will need a separate serial cable (not USB) and serial card with male connector in order to connect the US Robotics 5686E Modem. eClinicalWorks only supports the US Robotics Modem.

For practices that have greater than a total of 300-400 faxes per day (incoming and outgoing combined), the Digi Acceleport RAS modem would be recommended. With this modem the practice will receive faxes on multiple phone lines and send faxes on other phone lines. Note: Minimum of two dedicated phone lines will be required. The digiboard requires a larger chasis, please be sure to purchase the correct hardware for a digiboard installation. The analog line for the FaxServer has to be dedicated (separate from the analog line used for Clearinghouse transmission). This line can not be shared, split or VOIP.

eClinicalWorks FaxServer should always be logged in as ADMINISTRATOR

The FaxServer computer must have only one login at any given time. Multiple logins must be disabled on the FaxServer computer and the FaxServer application must be running in a console session.

eClinicalWorks FaxServer should always stay powered on for sending and receiving faxes

Printer drivers cannot be installed on the eClinicalWorks FaxServer computer

eClinicalWorks FaxServer and Windows FaxServer cannot work together on the same computer

Faxing prescriptions can be accomplished from all computers on the network
