It is through gathering information, gaining knowledge, learning to accept the reality of the situation and being open to change, that we have the best opportunity to provide good care for the family member or patient and find positive, personal growth as a caregiver. To support this caregiving journey, the Onondaga County Office for Aging and the Alzheimer’s Association CNY Chapter have partnered since 2006 to offer the Institute for Caregivers (I Care). This is a series of one or two session classes presented by local professional and practitioner experts offered spring and fall at no charge.

From those attending, we have learned the value of being pro-active, becoming knowledgeable about community resources, making plans in advance, putting documents in place, improving communication skills, having the family conversation early, exploring coping strategies and learning the importance of taking care of the caregiver.

This year, to increase awareness and opportunities for both current and future caregivers, I Care is collaborating with Upstate Oasis to offer some classes. These are all listed in the fall 2017 I Care catalog and the Upstate Oasis winter catalog. All I Care classes are free.

In this 90 minute class, you will learn ancient yogic technics focusing on breath, sound and gentle movement to help you find grounding and a sense of ease. Hospice & Palliative Care: What, When, Why?Palliative Care and Hospice consider more than the disease in providing health care. The goal is to improve quality of life for both the patient with a serious, chronic illness and the family caregivers. Dr. Pasniciuc will discuss the distinction between Palliative Care and Hospice and identify end of life care dilemmas and system issues. He will explain how critical it is to put advance care directive documents (health care proxy, MOLST forms) in place early on to maximize the preservation of patient’s rights.

Navigating Life After CaregivingGrief is a normal, appropriate emotional response to the loss of a loved one. There are no stages or timeline to how it unfolds. It is unique to you and your relationship with that person. Coping with the loss is difficult for everyone. For caregivers and seniors this is especially difficult. There can be a multitude of other losses that can accompany this loss which impact you. So how do you navigate life after the caregiving ends? You need to be the expert in identifying how your grief is impacting you. You need to be able to explore ways to ask for help and to manage your stress. It is a painful journey and grief needs to be shared. You need to make this journey with others just to know you are not alone. Handouts will be provided and there will be an opportunity for open discussion for participants to share personal experiences. An I Care/Upstate Oasis class.

Brain Fitness: Fact vs. Fiction - 2 Part ClassLong neglected, we are now in an exciting period of research into the brain, including how to reduce our risk of disease and improve our brain power as we age. New programs are being promoted that tap into our fears about developing Alzheimer’s and other types of dementia. Sometimes the hype is outpacing the research. This 2 part class will provide an overview of the research that exists, pointing out both the most promising findings and the current limitations of our knowledge. During session 2, we will have an opportunity to practice some of the strategies that appear to help people maintain or strengthen their cognitive abilities. An I Care/Upstate Oasis class.

Medicare: An Overview & Changes for 2018Confused by the numerous Medicare plans and options available to you and the changes taking place in 2018? But did you know the first nurses in recorded history were actually males? More and more men are stepping into the caregiver role in this day and age, and while it’s been debunked that women are from Venus and men from Mars, men do think, feel, and sometimes act differently in their responsibilities as caregivers. We are inviting men and women to please join us for an evening of fellowship and conversation surrounding this important role. Hopefully you’ll leave knowing yourself better, and feeling more confident in your role as caregiver.

Lee W. Livermore is the Public Education Coordinator for the Upstate New York Poison Center at Upstate Medical University. He has more than 15 years’ experience delivering poison prevention training programs and presentations to the general public, community based agencies, primary schools, colleges and universities. He is skilled in the areas of Drugs of Abuse, Train-the Trainer programs, Poison Prevention Awareness Programs and is active in developing partnerships with community coalitions. Lee serves on several Drug Task Forces within the 54 county coverage area of Upstate New York Poison Center. Lee has a bachelor’s degree in business and several certifications in communication, training, organizational development and personality profile assessments.

Christopher Norman is a Board Certified Geriatric Nurse Practitioner with 13 years of experience caregiving in Nursing. He holds a Bachelor’s Degree in Nursing from Dominican College, completed his advanced practice training and Master’s Degree at Yale University, and completed postgraduate training in Geriatrics at New York University. He has particular interest in whole-person/holistic care, preventing disease through physical activity, and promoting communication with people who have cognitive impairment.

Silviu Pasniciuc, MD is a Clinical Assistant Professor at Upstate Medical University Outpatient Medicine, Assistant Medical Director at Hospice of CNY, and Supportive Oncology at Hematology Oncology Associates of CNY. In addition to geriatrics, some of his current interests are use of integrative medicine in end of life symptom control, use of telemedicine in palliative medicine, ethics of research in vulnerable populations, and end of life communication. Dr. Pasniciuc is board certified in Internal Medicine and Hospice and Palliative Medicine.

Ellen Somers, MA, LMHC is the Assistant Director of Syracuse Jewish Family Service. She has worked with older adults and their family members for over 20 years in a variety of settings, such as adult day programs, assisted living, skilled nursing care, and their home. Her areas of interest include cognitive health, mental health and holistic wellness. Ellen has a Master’s Degree in Clinical Psychology from West Virginia University, a Graduate Certificate in Gerontology from the University of Utah, and is a NYS licensed Mental Health Counselor.

Clare Murray Volo is a 17 year student of Kripalu Yoga. She is also a certified Kripalu Yoga Instructor with over 500 hours of study. For close to 10 years, she has offered classes in the Liverpool/ Baldwinsville area. When she is not teaching yoga, Clare works with seniors. Using 30 years of work experience, with a dash of humor and a strong zest for life, Clare’s life’s work is dedicated to helping others find peace and ease in their bodies, minds and breath.

Susan Bachorik has a Master’s Degree Counseling Psychology. She presently works for Hospice of Central New York as a counselor in the bereavement center. Susan has worked with adults and children in one-on-one and group settings. She provides community and professional education on dying, death and bereavement through in-service trainings to nursing homes, home health aide trainings and family caregiver trainings. Recently, Susan assumed the role of coordinating and implementing HCNY Camp Healing Hearts Program for children ages 6-11 who have experienced a loss.

Andrea Berg, MD is an Assistant Professor of Medicine at SUNY Upstate Medical University. After graduating from Cornell University, she served in America Corps before earning her medical degree from George Washington University Medical Center. She completed her internship and residency in internal medicine/primary care at Yale New Haven Hospital and moved on to a Harvard Fellowship in Geriatric Medicine in Boston. As a clinician educator, Dr. Berg is particularly interested in fostering awareness and interest into the unique needs of older patients and our aging population. Her interests include comprehensive geriatric assessments with a special emphasis on cognitive evaluations and memory disorders.

Timothy P. Crisafulli, Esq. practices in the areas of elder law, trusts and estates, and tax law. His elder law practice is dedicated to assisting clients with long-term care planning. He also represents clients in Medicaid fair hearings and in guardianship proceedings. Through his trusts and estates practice, Mr. Crisafulli helps families protect, preserve and effectively transfer wealth. He utilizes his extensive knowledge of tax law to minimize capital gains, income, gift and estate taxes. He holds an LL.M in Taxation from the New York University School of Law; a J.D. magna cum laude from the Syracuse University College of Law; a Master’s degree from the Maxwell School of Citizenship and Public Affairs at Syracuse University; and a B.A. summa cum laude, from LeMoyne College.

Myrna Koldin, MSW is the Aging Services Specialist for the Onondaga County Office for Aging. She received her Masters of Social Work with a concentration in Gerontology from Syracuse University. She has worked as the New York State EPIC Outreach Representative covering nine counties. Previously, Myrna was the Senior Adult/Adult Services Director at the Syracuse Jewish Community Center overseeing all of their programs for seventeen years.

Caregiver Discussion Groups Family caregivers, of older or disabled persons, assume responsibilities and experience many changes in their lifestyles. They say they are frustrated in knowing how to solve the multitude of challenges facing them and don’t know where to turn for help. They need support. You need support!

Through the Onondaga County Department of Adult and Long Term Care, Office for Aging, Caregiver Services provides on-going support through Caregiver Discussion Groups. There are four groups to choose from, all meet regularly with an aging specialist facilitator. Family caregivers gather to share their situations, define the problems they face, explore solutions, learn about community resources and make a commitment to change.

Group Participants learn from, and support, each other as they reach out and try different strategies for providing care for their family members as well as themselves. The groups are trusting places where caregivers can express emotions and concerns and find validation and encouragement.

Family Caregiver Planning Onondaga County Office for Aging offers consultation and mediation services to bring family members together to design a plan to support their elders. Objective information on strategies and services is provided in a confidential setting where positive communication helps the family understand each other and focus on what is best for the person of concern. A written agreement is created which everyone can use as a guideline for action and sharing of responsibilities. There is no charge for this service. Contact: Cynthia at 315-435-2362 x4993.

The Alzheimer’s Association, Central New York Chapter provides programs and services to support individuals and families affected by Alzheimer’s disease. When Alzheimer’s disease or other dementias touch your life, we are here for you. Our services include:

• Information and referral services, including our 24-Hour Helpline (800.272.3900) and our award-winning website, alz.org.

• Care consultations, which assist individuals with dementia and their families to solve immediate problems and plan for the future. These meetings with our dementia experts include the preparation of needs assessments and action plans.

• Family consultations, bring caregivers and family members together in a safe, facilitated forum to identify and discuss issues of common concern and develop strategies to address them.

• Support groups that meet bi-weekly or monthly at nearly a dozen locations in Onondaga County.

• Safety services, including the pioneering MedicAlert® +Safe Return® emergency response service.

• Online services, including the ALZConnected® social community, Alzheimer’s Navigator® assessment tool, Community Resource Finder and the Marty Manning Online Education Center.

Nov. 3, 11 a.m.Everson Museum of Art (Syracuse)Bring art to life in a specially designed tour of the Everson Museum of Art meant for individuals living with Alzheimer’s and their care partners. An hour-long tour will be led by Everson docents and held during non-public hours of the museum.

Social engagement activities are a fun and comfortable way for people living with Alzheimer’s or other form of dementia and their caregiver to get out, get active, and get connected with one another through a variety of social events and community-based activities. Participation is free of charge.
