ELIGIBILITY Eligible employees must be actively at work on the plan effective date for new benefits to be effective.

BENEFITS ENROLLMENT REFERENCE CENTER The Reference Center on Benefit Solver is a one-stop-shop for you to find all things benefits related. On the website, you’ll find open enrollment and plan year dates, benefit descriptions, carrier contact information, product brochures, claim forms and enrollment details. Visit www.benefitsolver.com today! EXISTING EMPLOYEES When it’s time to enroll in your benefits, First Financial Account Representatives will be available to assist you with making your selections. Your selections can be made anytime during open enrollment online from your work or home computer. Before enrollment, take time to educate yourself on the available benefits and which options would work best for you and your family by visiting the Reference Center.

MID-YEAR BENEFIT CHANGES You may add or cancel coverage during the plan year only if you have a change in family status. You must notify the benefits department within 31 days of the change. QUALIFYING LIFE EVENTS INCLUDE:

• Changes in household, including marriage, divorce, legal separation, annulment, death of a spouse, birth, adoption, placement for adoption, or death of a dependent child

• Involuntary loss of health coverage, losing eligibility for Medicare, Medicaid, or CHIP, Dependent Child turning 26 and losing coverage through a parent’s plan

DECLINING COVERAGE If you are eligible for benefits, but wish to DECLINE coverage, please complete the online enrollment either on your work or home computer. Under each option, you will need to select “waive.” You must still complete the beneficiary information.

ENROLLMENT ASSISTANCE CALL CENTER

PRE-ENROLLMENT ASSISTANCE HOTLINES- To help answer questions prior to Open Enrollment Have a question about benefits? El Paso ISD employees can call into the Enrollment Assistance Center or The Customer Service Hotline and reach a representative with First Financial to help answer questions prior to Open Enrollment. Call the Enrollment Assistance Center at 855-765-4473 Option 5 or 800-365-3860 for assistance today!

ENROLLMENT ASSISTANCE CALL CENTER - The Enrollment Assistance Call Center is available for employees during Open Enrollment. The EAC will be open 6/29 – 7/31* Monday – Friday 8 AM – 5PM MST. Call 855-765-4473 Option 5 for Assistance. Enroll in benefits 2 ways:

1. Log into www.benefitsolver.com and enroll in your benefits

2. Schedule a time to visit with a First Financial Account Manager to enroll in your benefits. 15-minute appointment times are available with FFGA Account Managers who can answer any questions you have and get your benefits enrollment done at the same time. We have bilingual representatives available to assist you for enrollment and will call you at the number you provide for your scheduled time. Visit https://elpasoisd.timetap.com/to set up your appointment today. Please note the Account Managers will call you from a 281-area code number.

3. Don’t have an appointment scheduled? No Problem! Call the Enrollment Assistance center at

866-765-4473 Option 5 and get to an available representative. If all representatives are assisting other callers, or if it is after hours, leave a voicemail and a representative will call you back.

The district’s health plan choices are offered through El Paso ISD’s self-funded health plan administered by Cigna and TRS-Active Care administered by Blue Cross Blue Shield of Texas. Between the two health plans, there are six different l plan options to choose from. From in and out-of-network options to comprehensive prescription drug coverage and special health and wellness programs, El Paso ISD’s health plan options have been designed to flexibly meet the needs of the employees and their families. *The rates below reflect the rates for Full-Time (30-40 hour) employees. Please see the benefit website for the rates for the other coverage categories.

EL PASO ISD SELF-FUNDED HEALTH PLAN ADMINISTERED BY CIGNA Under this health plan, there are two options to choose from. Both options provide access to a National Network of Medical Providers and out-of-network options to comprehensive prescription drug coverage and special health and wellness programs. EPISD’S self-funded plan has been specifically designed to provide an improvement in benefits and family affordability, as well as exceptional customer service 24/7.

Self-Funded CDHP Plan • High deductible plan – must meet deductible before plan covers expenses • In-network and out-of-network benefits – separate out-of-network deductible/out-of-pocket maximum • Provider network is with Cigna • Deductible applies to medical and pharmacy • District contributes up to $500 a year to your individual Health Savings Account if you elect the HSA plan • Employee may elect to contribute additional funds to their Health Savings Account. No employee

contribution is required in order to receive the districts contribution. • Low Cost Telemedicine visits through Amwell and MDLive

Traditional PPO Plan • Co-pay plan – pay flat fees for office visits and prescriptions • Low in-network deductible • In-network and out-of-network benefits- Separate out-of-network deductible/out-of-pocket maximum • Compatible with Flexible Spending Accounts (FSA) • No requirement for a Primary Care Physician • No referrals for Specialist visits • No cost for telemedicine visits with Amwell and MDLive

A Health Savings Account (HSA) is a great way to help you control your healthcare costs. It works in conjunction with a qualified High Deductible Health Plan (HDHP) to combine tax-free savings earmarked for qualified medical expenses. An HSA allows you to set aside money to pay for higher deductibles associated with a lower monthly premium HDHP. Eligible expenses include things like co-pays and deductibles, prescriptions, vision expenses, dental care, therapy and medical supplies. HIGHLIGHTS

• Balances roll over from year to year and can earn interest along the way. • Portable – you keep it even after you leave employment. • Tax advantages – invest money in mutual funds to grow your tax savings for either future healthcare

costs or retirement. • Pay for expenses with a benefits debit card that gives you immediate access to your money at the time

of purchase. • Should you not use the benefits debit card, expenses can be reimbursed through our online portal,

online bill pay directly to your provider or submitting a distribution request form. • Receipts are not required for reimbursement but be sure to save them for tax purposes. • You cannot sign up for an HSA plan if your spouse has a Flexible Spending Account through their

HEALTHCARE FSA A Health Flexible Spending Account (Health FSA) is an IRS-approved program to help you save taxes and pay for out-of-pocket medical expenses not covered under your medical plan. Your plan includes a grace period option allowing you to have additional time to incur and claim against unused funds in the new plan year. Keep in mind that remaining balances after the grace period is exhausted will be forfeited under the IRS’ use-it-or-lose-it rule.

Your maximum contribution amount for 2020 is $2,750. HIGHLIGHTS

• Contributions are automatically deducted from your paycheck on a pre-tax basis. This helps reduce your taxable income and increase your spendable income.

• You can select the FSA with any medical plan option or even with no medical plan option • Your full election will be available to you on the first day of the beginning of the plan year. • Be conservative – any money left in your account at the end of the plan year will be forfeited. • Use your benefits card to pay for qualified expenses upfront so you don’t have to spend money out of

Your receipt must include: Date of purchase or service, amount you were required to pay after insurance, description of the product or service, merchant or provider name, and the patient name.

DEPENDENT DAYCARE FSA With a Dependent Daycare Flexible Spending Account, you can set aside part of your pay on a pre-tax basis to pay for eligible dependent care expenses like childcare, babysitters, and adult day care.

You may allocate up to $5,000 per tax year for reimbursement of dependent care services. If you are married and file a separate tax return, the limit is $2,500.

FSA RESOURCES BENEFITS CARD The First Financial Benefits Card is available to all employees that participate in Medical FSA and/or a Dependent Daycare FSA. For Medical FSA, the Benefits Card gives you immediate access to your money at the point of purchase. • The IRS requires validation of most transactions for FSAs. You must submit receipts for validation of expenses when requested. Voluntary Retirement Plans 457 (b) and 403 (b), can be excellent ways to save money for retirement. They can serve as a supplement to a traditional pension plan or other retirement plan(s), or as a stand-alone plan. These plans are offered by EPISD as determined by section 501(c)(3) of the Internal Revenue Code. Contributions and investment earnings grow tax deferred until withdrawal (assumed to be retirement), at which time they are taxed as ordinary income. HOW DO THESE PLANS WORK? Employees enroll and participate through their employer. Contributions to a voluntary retirement plan are deducted on a pre-tax basis through a Salary Reduction Agreement. This is an arrangement where the participating employee agrees to participate and selects the amount of contribution. The amount by which the salary is reduced is directed to investments offered through the employer and selected by the employee. These contributions are called elective deferrals and are excluded from the employee’s taxable income. Life is full of unexpected events that may impact your health insurance coverage. Under the Consolidated Omnibus Budget Reconciliation Act, better known as COBRA, you have the right to continue your group health coverage such as medical, dental, vision insurance and flexible spending accounts for a limited period.

HIGHLIGHTS

• Temporary continuation of coverage that generally lasts for 18 months due to employment termination or reduction of hours of work, divorce, death or a child no longer qualifying as a dependent. What is Express Issue? Express Issue refers to minimal questions on an application, no more than 3, to qualify for benefits.

What is Guaranteed Issue (GI)? Also referred to as Guaranteed Acceptance, or GA, means that you can’t be turned down for health reasons. Guaranteed Issue is typically offered during initial enrollment for benefits.

What is a "pre-existing condition"? A pre-existing condition is a disease or physical condition for which symptoms existed or medical advice or treatment was recommended or received prior to the effective date of coverage.

What is a deductible? A deductible is what you must pay for your health care before your insurance pays its part. Most plans have deductibles, which start over when your “PLAN YEAR” starts over. For example, if your plan has a $1,000 deductible and you have surgery that costs $5,000, you’ll pay $1,000 before your insurer helps you cover your bills.

What is a co-pay? A copay is a fixed amount that you pay for covered services like a prescription or a doctor’s visit. Some health plans also apply coinsurance to certain services. With it, you pay a percentage of the total cost of care. For example, if you have a 20% coinsurance, and your doctor's appointment costs $300, you'd pay $60. That's if you've met your deductible.

What does out-of-pocket maximum mean? Your out-of-pocket maximum is the most you have to pay each year toward your medical services or prescription drugs before your insurance pays for all your care. This amount does not include what you pay in premiums. The Affordable Care Act limits the out-of-pocket maximums. In 2018, for one adult using the in-network providers, it can be no more than $7,350, and for a family, it can be no more than $14,300.

What does EOB mean? After you’ve visited your doctor or had a procedure in a hospital, you’ll receive an Explanation of Benefits (EOB) form explaining how much of the charges your insurance will pay. The EOB isn’t a bill itself, but it can tell you what your doctor may charge you. Look for the words “due from patient” to see how much you may owe after your insurance pays.

Before you get certain tests or procedures, do you need permission from your health insurance plan? If your doctor says you need a test or procedure, your health plan may have to give permission if it's to be covered by insurance. Giving that permission is called preauthorization. Your plan's overview of benefits lists what care needs to be preauthorized. If you don't get it when it's required, your health plan won't pay its part of the costs.

Dependent Day Care Accounts

If I contribute to a Dependent Day Care Account, can I also write-off my daycare expenses on my taxes? No, you may not. If you use the Dependent Day Care Account, you save money up-front on your taxes. Your per-paycheck deductions are taken out of your paycheck before you pay taxes on your income. Thus, your taxable income is less, and you pay less in taxes.

What kinds of care does this cover? • Before-school and after-school care • Expenses for preschool/nursery school • Extended day programs • Au pair services (amounts paid for the actual care of the dependent) • Babysitter (in or out of the home) • Nanny services (amounts paid for the actual care of the dependent) • Summer day camp for your qualifying child under the age of 13 • Elder day care for a qualifying individual

Can I use the Dependent Day Care account to fund elder care for my mother/father/spouse? Yes, you may use your Dependent Day Care account to fund care for individuals who qualify as your dependent who lives with you for more than half the year (and for whom you are the custodial parent in cases of divorce) your spouse, or other tax dependent, who is incapable of self-care and lives with you for more than half the year.
