ELEMENT 4
LESSON 3
1. Explain the mean of hierarchy of control.
Hierarchy of control is concept used a great deal in health and safety. It is a list of options in order of importance,
effectiveness or priority, written so that the most extreme and effective method of control is at the top of the
hierarchy, with the least effective at the bottom.
2.
Using a sketch, outline the hierarchy of control that should be adapted when controlling.
Elimination
Substitution
Engineering control
Administrative control
Personal protective equipments
3.

4.
Outline the duties of an employer as it relates to the prevision of PPE
Duties of an employer as it relates to the prevision of PPE are:-
Suitableappropriate, if it fits, ergonomic, does it increase risk.
Compatiblewhen two or more PPE is in use at the same time.
Storage accommodation for PPE.
Provide information, instruction and training on the use of PPE.
Enforce the use of PPE.
Replace or repair damaged or lost item.
5.
List (4) advantages with the use of PPE.
Cheap
Immediate protection
Back up emergencies
Interim control6.
List (4) disadvantages with the use of PPE.
Does not remove the hazard
Only protect one person
Uncomfortable
Expensive
7.
Identify (8) sources of information.
There are internal and external sources of information
Internal sources
Accident record
Risk assessment
Absence record
Audit report
External sources
National legislation

8.
Explain safe system of work.
A safe system of work is a formal procedure base on a systematic examination of work in order to identify the
hazards. It defines safe methods of working which eliminate those hazards or minimize the risk associated with
them.
9.
Outline factors that should be considered when developing a SSW.
Factors that should be considered when developing a SSW are:-
Peoplewho is the SSW for?
Equipmentswhat plant or equipment will be worked on?
Materialswhat materials will be used or handling during the work?
Environmentin what type of environment will work take place?
10.
Define the term confine space.
A confine space is any place such as a chamber, tank, vat, silo, pit, well, pipe, sewer, flue, or similar, in which by
virtue of it enclosed nature there is a foreseeable risk.
11.
List the items to be included on a SSW for confine system work operation.
General principles to be included on a SSW for confine system work operation are:-
Do not work in confine space if there are another way to get it done.
If confined space is the only way to do the work, competent person must carry out a risk assessment.
A safe system of work must be developed for the confined space entry.
Emergency arrangement must be put in place as part of the SSW.
Confined space entry must be under permit-to-work control only.
All personnel must be trained.

12.
Identify the control measures that should be included in a SSW.
Control measures that should be included in a SSW are:-
No long working for high risk activities
Arrangement for remote supervisor
Procedures for logging workers location when lone working
Use of mobile phone or radio to ensure good communication
Alarm system to raise alarm and pin-point the workers
Procedures to be adapt by worker when lone working
Emergency procedures
Training for workers in those procedures
13.
Outline or list the arrangement to be put in place when traveling abroad.
Arrangement to be put in place when traveling abroad are:-
Accommodation
In-country travel
Emergency arrangement
Insurance arrangement
24hr organization contact
Financial arrangement
Personal health advise and vaccinations
Personal security and advise
