It gives me great pleasure to write this message for the EndoQE 2015 souvenir programme.

The Ministry of Health, Malaysia has always been very committed towards the development and growth of subspecialities in Medicine. It has a very serious commitment to Fellowship Training in these supspecialities. Gastroenterology Fellowship training begin in the 1990s in the Ministry of Health. Today numerous Ministry of Health-trained gastroenterologists serve both in the public and private sectors in Malaysia. There are currently 146 gastroenterologist registered under the NSR.

Gastrointestinal Endoscopy essentially began in Malaysia as a surgical discipline. However, over the past 30 years, GI Endoscopy has increasingly been performed by physician gastroenterologists in Malaysia.

The progress in GI Endoscopy over the past decade has been nothing short of phenomenal. Various new Therapeutic GI Endoscopic procedures have been developed and boldly explore therapeutic areas which were formerly the domain of surgeons and radiologists. Therapeutic ERCP, Therapeutic Endoscopic Ultrasound, NOTES (Natural Orifice Transluminal Endoscopic Surgery), Image-Enhanced Endoscopy and Per Oral Endoscopic Myotomy (POEM) are some of the new exciting new areas in Therapeutic GI Endoscopy.

I would also like to pay tribute to Queen Elizabeth Hospital, Kota Kinabalu for being the only KKM centre of training for Post Basic Endoscopic Nursing. It has helped to produce more than 340 endoscopy nurses and assistant medical officers, i.e. Gastrointestinal Assistants (GIAs), for the whole country serving in both the public and private sectors.

I would like to congratulate the Gastroenterology Unit of the Department Of Medicine, Queen Elizabeth Hospital for organising EndoQE 2015, the 2nd Sabah International Endoscopy Workshop. I understand that EndoQE 2014 was very successfully held last year.

I understand that a “Hands-On” Workshop for GI Fellows and Live Endoscopy featuring various advanced therapeutic endoscopic procedures in ERCP and EUS will be highlights of EndoQE 2015 along with a workshop on Advanced Endoscopic Imaging.

I am told that among the outstanding international faculty assembled are two living legends, namely current president of the WEO (World Endoscopy Organisation), Dr Nageshwar Reddy from Hyderabad, India and Dr David Carr-Locke from New York, USA who was past president of ASGE (American Society of Gastrointestinal Endoscopy). We are truly honoured to have such a world-renowned faculty hailing from India, USA, Singapore, Australia and Malaysia who are true masters of Endoscopy.

I am impressed that prestigious GI organisations such as EUS-TAP (EUS TASK FORCE ASIA PACIFIC) and WIGNAP (Women in Gastroenterology Asia Pacific) are participating in this workshop.

I am pleased that the program includes an Endoscopy Nursing Workshop which was held yesterday, a “Hands-On” Endoscopy workshop for GI Fellows focusing on ERCP and Endoscopic Ultrasound today, State-of-the-art lectures and of course, Live Endoscopy. In addition, I am given to understand that there will be a teaching workshop focusing entirely on Advanced Colonoscopy Imaging.

I am confident that all 350 attendees will benefit immensely from this workshop. Do have an enjoyable workshop.

EndoQE 2015

It is a great pleasure to know that EndoQE 2015 is being organised byDr Jayaram Menon in Kota Kinabalu, Malaysia. In recent years gastrointestinal endoscopy has made remarkable progress because of advances in technology and techniques. Image enhanced endoscopy has know resulted in better visualisation of lesions in GI tracts and similarly endocytoscopy, optical coherence tomography and confocal endo microscopy have all enhanced our ability to obtain optical biopsies even before the pathologists sees the lesion. Parallel development in stent technology and instrumentation have resulted in

remarkable advances in interventional endoscopy. Combinations of palliative procedures like stenting with Radio Frequency Abiation and Photo Dynamic Therapy have resulted in not only better quality of life in advanced malignancies but also increased survivals. In the beginning of this new millennium we are on the threshold of pathbreaking developments in the field of GI endoscopy. Workshops, live demonstrations and video based lecturers are extremely important methods of transmitting the rapidly expanding knowledge in diagnostic and interventional endoscopy. EndoQE 2015 comes at a right time to showcase all these developments. I have known Dr Jayaram Menon over a decade now and I am impressed by his knowledge, organisational ability and his leadership and I am sure he and his team will organise a superb meeting to convey the excitement of all these new developments in GI endoscopy to the audience. This year, the hardworking organising committee headed by Datuk Dr Jayaram Menon have outdone themselves. Not only does the workshop cover what is at the cutting edge of endoscopy such as Spyglass Cholangioscopy, they have

managed to bring some of the worlds’ foremost endoscopists from USA, India, Australia, Singapore and Malaysia, to this workshop.

The Endoscopy Workshop caters for all those interested in gastrointestinal diseases including gastrointestinal assistants, gastroenterologists and general surgeons. Spread over the four days are state-of-the-art lectures, live demonstrations as well as hands-on workshops in EUS and ERCP for fellows.

The International and Local Faculty from India, USA, Australia, Singapore and Malaysia are indeed very impressive. On behalf of the JKNS I would like to welcome you to the beautiful state of Sabah, Land Below the Wind. I understand

that some of you have been here before. Some members of the Faculty have also been involved in the training of GI Fellows in Sabah for more than 10 years. It is with pride that we welcome you as the presence of this World-Class Faculty will truly enhance this workshop. We are honoured that 2 living legends in Endoscopy, i.e. Dr Nageshwar Reddy and Dr David Carr-Locke, are here to grace this meeting.

The workshop will feature Live Endoscopy, a “Hands-On” Workshop in ERCP and EUS (Endoscopic Ultrasound), State-of-the-Art Lectures, a Colonoscopy Workshop as well as an Endoscopy Nursing Workshop.

I understand that 350 registrants have signed up for this workshop. This is indeed an impressive turnout of doctors, gastroenterologists, surgeons and Endoscopy nurses.

We are also proud that the Endoscopy Unit, Queen Elizabeth Hospital is the only recognised training centre for Endoscopy Nursing in KKM. Thus far we have produced 340 Endoscopy nurses for the private as well as government sectors.

A unique feature of this workshop is its involvement of EUS-TAP (EUS Task Force Asia Pacific) and WIGNAP (Women in Gastroenterology Asia Pacific). The latter organisation is indeed unique with its focus on Women in Gastroenterology. EUS-TAP, I am made to understand, has conducted teaching workshops in QEH before. I hope that there will be several more teaching workshops in the future.

Finally, I wish all of you a very fruitful workshop.

On behalf of the Organising Committee for Endo QE 2015 I would like to extend a warm welcome to Kota Kinabalu, Sabah to all our delegates. A special word of welcome to our outstanding Faculty from India, USA, Australia, Singapore and Malaysia. We are proud to have assembled such a stellar cast to helm this 2nd Sabah International Endoscopy Workshop here in Queen Elizabeth Hospital. This workshop is jointly organised by the Malaysian Society of Gastroenterology and Hepatology (MSGH), EUS-TAP (EUS Task Force Asia Pacific) and WIGNAP (Women in Gastroenterology Asia-Pacific). I am very grateful to these three

organisations for their unswerving support, guidance and commitment to teaching and training in Gastroenterology.

EndoQE 2014 was very successful and proved especially useful to GI Fellows who benefitted immensely from the Hands-On workshop. This current workshop also places a strong emphasis on teaching.

So what do we have in store for you? EndoQE 2015 starts off with a GIA Conference on10th September. This will be followed by State-of-the-Art lectures on 11th September. We are very honoured and grateful that Datuk Dr Noor Hisham Abdullah, the Director-General of Health, has kindly consented to open the workshop. This will be followed by a hands-on EUS and ERCP workshop mentored by our illustrious faculty. Dr Nageshwar Reddy and Dr David Carr Locke, two living legends in GI Endoscopy, will conduct the ERCP session. The EUS sessions will be led by Dr Ryan Ponnudurai, Dr Ben Devereaux, Dr Roy Soetikno, Dr Christopher Khor, Dr Sharmila Satchithanandan and Dr Payal Saxena. Live Endoscopy will be featured on 12th September. Dr Rajvinder Singh will round off with an Advanced Endoscopy Imaging Workshop which will be held on 13th September.

I am very grateful to my indefatigable Organising Committee for their commitment and dedication without which this workshop would not have been possible. The unstinting support of the sponsors has helped ensure the realisation of this workshop. FACULTY BIODATA

Nageshwar Reddy India

Nageshwar Reddy is Chairman of the Asian Institute of Gastroenterolgy, Hyderabad, India. He completed his undergraduate medical degree at Kurnool Medical College, Kurnool in 1978 and later did his specialization in Gastroenterology at the Post Graduate Institute of Medical Education and Research, Chandigarh, India. He is internationally recognized in the field of therapeutic endoscopy particularly in the field of ERCP. He has received many awards in recognition of his contributions to gastroenterology. In 2009, he received the Master Endoscopist Award from the American Society of GI Endoscopy. He also received the Master of the World Gastroenterology Organisation (MWGO) in 2013 from the World Gastroenterology Organisation. He is also a board member of multiple international committees and has also received numerous awards for social work in India. He is currently the President of the World Endoscopy Organization (WEO).

Benedict Devereaux Australia

Ben Deveraux is now Senior Consultant Gastroenterologist at the Royal Brisbane and Women’s Hospital. He completed his undergraduate medical degree at the University of Queensland in 1991. He subsequently underwent his gastroenterology training at the Royal Brisbane Hospital, Brisbane, Australia. This culminated in his obtaining Fellowship of the Royal Australasian College of Physicians in 1998. Ben then completed fellowships in ERCP and EUS at Indiana University Medical Centre, Indianapolis, Indiana, USA. During this time, he developed a special interest in the staging and management of pancreatic cancer and was awarded an American Digestive Health Foundation 2000 Endoscopic Research Grant.

Christopher Khor Singapore

Dr Christopher Khor attended medical school at the National University of Singapore, and completed clinical fellowships in ERCP and in Endoscopic Ultrasound (EUS) at the Medical University of South Carolina and Indiana University Medical Center respectively. More recently, he trained in Endoscopic Submucosal Dissection in Japan. His main practice areas are in pancreato-biliary disease (ERCP and EUS), endoscopic resection and general gastroenterology. Dr Khor has more than 20 peer-reviewed papers to his name, serves on the editorial board of Endoscopy, and is a reviewer for a number of other endoscopy and GI journals. Dr Khor currently co-chairs an Asia-Pacific group focused on EUS education.

David Carr-Locke USA

David Carr-Locke is Chief of Digestive Diseases and Co-Director of the Centre for Digestive Health at the Mount Sinai Beth Israel Medical Centre, USA. He is internationally recognized in the field of therapeutic endoscopy. He received his medical degree from the University of Cambridge in England, followed by residency training in medicine and gastroenterology at the University of Leicester and fellowship training at the New England Baptist Hospital. He is a Past President of the American Society for Gastrointestinal Endoscopy (ASGE) and the International Hepatopancreatobiliary Association. He was recipient of the ASGE’s highest award for members, the Rudolf Schindler Award, in 2007. He is currently on the Governing Council of the New York Society for Gastointestinal Endoscopy.

Payal Saxena Australia

Payal Saxena is Consultant Gastroenterologist and Hepatologist at the Concord Hospital Sydney. She completed her Gastroenterology training in Sydney then travelled to the USA where she undertook a two-year International Endoscopy and Research fellowship at the renowned Johns Hopkins Hospital. Payal received expert training in specialist areas including interventional EUS and ERCP. She also performs Per-Oral Endoscopic Myotomy (POEM). Payal has special interest in pancreatic disease including pancreatic cancer screening and pancreatic cysts. She contributes to the International Cancer of the Pancreas Screening Consortium and International Registry.

Rajvinder Singh Australia

Rajvinder is a fellow Malaysian and is now the Head of Gastroenterology & Interventional Endoscopy at Lyell McEwin Hospital, South Australia and a Clinical Associate Professor at the University of Adelaide, South Australia, Australia. Upon completion of his gastroenterology training, he received the Lancet International Fellowship which enabled him to undertake a higher research degree on Narrow Band Imaging in Barrett’s Esophagus at The University of Nottingham,UK. Raj has a special interest in Advanced endoscopic imaging techniques and endoscopic treatment of premalignant and malignant lesions in the gastrointestinal tract.

Ryan Ponnudurai Malaysia

Ryan is board certified in Gastroenterology from the USA, having done his residency at the Cleveland Clinic and fellowship at Scripps Clinic in La Jolla California. He then spent a year in interventional endoscopy at the Wellesley Hospital in Toronto and EUS training with Professor Soehendra in Hamburg. On returning to Malaysia, he spent six years at Selayang Hospital pioneering the teaching of EUS in Malaysia. Ryan chairs the Asia Pacific Task Force in EUS which was formed three years ago. This has been a vehicle for hands-on training in EUS for specialists in the region. He was recently elected Vice President of the International EUS Society. Ryan has been invited to over 80 live endoscopy workshops and has written book chapters and published in peer review journals.

Roy Soetikno Singapore

Roy Soetikno was Clinical Professor of Medicine at Standford University, Palo Alto, California, USA. He graduated from the University of Chicago, Printzker School of Medicine. In January 2015, he moved to Singapore where he is now Consultant Gastroenterologist at the Singapore General Hospital and Professor at the Dukes University-Singapore Medical School based at the Singapore General Hospital. He is well known for his work on image-enhanced endoscopy and recently received the prestigious 2014 American Society of Gastroenterology Endoscopy Master Endoscopist Award. He recently received the 2015 UAB Basil I. Hirschowitz Master Endoscopist Award. This award is presented to an outstanding individual who has advanced the art and science of endoscopy in the World.

Sharmila Sachithanandan Malaysia

Sharmila is Consultant Gastroenterologist at the Subang Jaya Medical Centre, Selangor, Malaysia. She underwent her gastroenterology training in Malaysia. She was previously the Head of Endoscopy Division of Gastroenterology in Selayang Hospital, Selangor from 2006-2011. She is also a member of various local and international societies. Sharmila has a special interest in hepato-pancreatico-biliary (HPB) medicine, advanced endoscopic procedures Endoscopic Ultrasound (EUS) and ERCP, and neuroendocrine tumours. She is the recipient of several International research awards, the most recent being the prestigious American Society of Gastroenterological Endoscopy (ASGE) award (2011) for Audio Visual DVD on Endoscopic Ultrasound - the First time an Asian has won this in 30 years.

Ahmad ShukriAhmad Shukri is Consultant Gastroenterologist at the Hospital Sultanah Nur Zahirah, Kuala Terengganu. He was a fellow in gastroenterology in Royal Liverpool University Hospital, UK, St Thomas Hospital, London and Eppendorf University, Hamburg, Germany. He has special interest in Interventional Endoscopy and is trained in management of GI fistulas, perforation and bleeding using Over-the-Scope-Clip (OTSC).

Ganesananthan ShanmuganathanGanesananthan is Consultant Gastroenterologist at the Pantai Hospital, Kuala Lumpur. He graduated from the National University of Malaysia and subsequently obtained his MRCP (UK). He underwent his gastroenterology training in Malaysia and Australia. His special interests are in endoscopy, ERCP, Manometry and Ambulatory pH studies.

Rajesh KumarRajesh Kumar Muniandy is a lecturer in the Faculty of Medicine and Health Science, at Universiti Malaysia Sabah (UMS). He holds an MD from Universiti Kebangsaan Malaysia (UKM) and a Master of Medicine in Anaesthesiology from Universiti Sains Malaysia (USM). Besides research and clinical work, he is deeply involved in undergraduate teaching. His current interest is in Medical Simulation and Pain Management.

Mutyala Vinod KumarMutyala Vinod Kumar is Consultant Surgeon at Gleneagles Hospital, Kota Kinabalu. He has qualifications from India, UK & USA. He has served Ministry of Health, Malaysia for more than 19 years as aconsultant minimal access surgeon and endoscopist. He is a lecturer in University Malaysia Sabah. He has many publications in regional, national and international journals. His main interest is surgical gastroenterology.

Jernih LimMs Jernih Lim is Nurse Manager at the Penang Adventist Hospital. Her interest is in quality and infection control in endoscopy. She is presently secretary of the Malaysian GIA Society. She is also a member of Society of International Gastroenterological Nurses and Endoscopy Associates (SIGNEA). She obtained her certificate in Gastroenterology Nursing for RN (QUT) in 2008. She obtained her GI Endoscopy Nursing Post Basic training (MOH, Malaysia) in 2009.

Rosalin SulitRosalin Maria P Sulit is a nursing tutor with 24 years of experience and currently the Head of Post Basic and Advanced Diploma Training Program in Kota Kinabalu. She holds a bachelors degree in Health Sciences and Masters Degree in Management. She was awarded a study grant on endoscopy to the United Kingdom and thereafter, started the post basic endoscopy nursing training program in Malaysia. She is the scientific director of the Malaysian GIA Society. She is committed towards uplifting endoscopy nursing in Malaysia.

Sanjiv MahadevaSanjiv Mahadeva is Consultant Gastroenterologist and Professor of Medicine at the University of Malaya. Following completion of his sub-specialisation training in Gastroenterology and Hepatology in the U.K., culminating in the award of the U.K. Certificate of Completion of Specialist Training (CCST), he joined the Gastroenterology unit of the University of Malaya in 2004. He has actively contributed to clinical, teaching, administrative and research services in UMMC. His areas of expertise include colon cancer screening, therapeutic GI & Biliary endoscopy, enteral nutrition, management of chronic liver disease and difficult functional bowel disorders.

1st ENDO QE 2014 - “THE THINKING ENDOSCOPIST”

The Gastroenterology Unit of the Queen Elizabeth Hospital (QEH), Kota Kinabalu in collaboration with the Ministry of Health Malaysia organised the First Sabah International Endoscopic Ultrasound (EUS) and Endoscopic Retrograde Cholangio-pancreatography (ERCP) hands-on Workshop on5th - 6th December 2014 in the QEH.

Graced by renowned EUS experts, both local and international, this two-day workshop with the theme “The Thinking Endoscopist” was conducted with the aim of training and familiarising endoscopists with the fundamentals of the EUS and ERCP. This event was put together by course director, Datuk Dr Raman Muthukaruppan and his team. The invited faculty members comprised Dr Arthur Kaffes and Dr Saurabh Gupta from Australia and Datuk Dr Ryan Ponnudurai, Mr Krishnan Raman and Dr Mohd Rizal Roslan from Malaysia. The event was enriched with discussions among the expert panel on difficult cases as well as short lectures.

This workshop was attended by gastroenterologists from all over Malaysia, and witnessed a wide spectrum of cases ranging from TB of the liver to pancreatic malignancies. The integration of didactics, live cases, and hands-on experience under the guidance of the experts allowed the participants to enhance their knowledge and technique in EUS and ERCP.

QEH is the only Hospital in East Malaysia that provides EUS services. The Endoscopy Unit was established in 1998 by Datuk Dr Jayaram Menon and has since evolved into a training centre of excellence in the Ministry of Health (MOH). The services offered in the centre range from upper and lower endoscopy (OGDS and Colonoscopy), small bowel enteroscopy, capsule endoscopy, ERCP (Endoscopic Retrograde Cholangiopancreatography) and Endoscopic Ultrasound (EUS).
