Applying for your rebate is easy. Just mail in a copy of your receipt and utility bill with this rebate form.

Committed toenergy efficiency

You must be a PSO residential customer.

There is a limit of two rebates per customer per year you apply. You must purchase an ENERGY STAR® certified bathroom ventilation fan between January 1, 2017, and October 31, 2018.

The rebate must be postmarked no later than October 31, 2018.

Bathroom ventilation fan must be on the qualified ENERGY STAR list found at:https://www.energystar.gov/productfinder/product/
