To make your benefit elections, you will login to our employee portal called The Zone. Directions on how to enroll are included in the back of this package of information. There is additional information on the plans available in The Zone as well.

Blue Cross Blue Shield will mail personalized Medical/Prescription ID cards for you and your covered dependents to your home address, within 14-21 days after completing the enrollment process. MetLife Dental ID cards can be retrieved from the MetLife My Benefits website.

Getting Started with Enrollment Review Medical & Rx Choices (HDHP and HSA)

Health Savings Account (HSA) Enrolling in the High Deductible Health Plan allows you to deposit funds into a Health Savings Account (HSA) that can be used to pay for qualified medical expenses. Enrolling in the HSA is a great way to set pre-tax dollars aside to cover the higher initial costs of the HDHP.

Here are some important things to know about Health Savings Accounts:

• You must be enrolled in the HDHP to contribute to a Health Savings Account.

• The annual contribution limits for Health Savings Accounts are determined by the HDHP tier you elect:

HDHP Employee Only - HSA maximum contribution limit of $3,550 per year (individual).

All other HDHP - HSA maximum contribution limit of $7,100 per year (family).

• Individuals over age 55 can contribute an additional $1,000 per year.

• There is no time limit on reimbursements from your HSA as long as the claims occur after the HSA effective date.

• You can change payroll deductions on the HSA at any time and a family status change is NOT required to do so.

Not Insured?Consider enrolling in the High Deductible Health Plan (HDHP) to avoid paying the healthcare tax. The HDHP qualifies as minimum essential coverage under the Affordable Care Act and has no monthly premium.

Base High Deductible Health Plan (HDHP) In the HDHP you are responsible for medical and pharmacy costs until you reach your deductible in exchange for no monthly premium. Preventative care is still covered at 100% and not subject to the deductible when using in-network providers. Once the deductible has been met, the plan will process eligible charges at 70% up to the out-of-pocket maximum. The covered expenses under the HDHP are the same as the Buy Up PPO and you can use the tools available through Blue Cross Blue Shield of Texas and Optum to “shop around” for the lowest cost options.

We understand that a health plan with no monthly premium can be attractive, especially if you’re on a tight budget.

NOTE: The HDHP and HSA option is not designed for everyone.

You should carefully consider the impact to your healthcare budget in the event you have an unexpected medical emergency. To put it in perspective, a single trip to the Emergency Room can start at $3,000.

Next StepConsider Flexible SpendingFlexible Spending AccountYou may allocate pre-tax dollars to be set aside from each paycheck for medical out-of-pocket expenses. This can include office visit co-pays, deductibles, vision exams, dental work and prescriptions. For a list of covered & non-covered expenses, please go to www.irs.gov and review Publication 502.

You do not have to be enrolled in the health plan to participate. The elected amount will be available immediately and a debit card will be provided. Substantiation for amounts other than co-pays will be required so you’ll need to save receipts and insurance statements.

You may also allocate pre-tax dollars for day care expenses. Day care expenses are reimbursed up to the amount that you contribute each pay period.

The plan has a Grace Period, giving participants an additional 2.5 months to incur expenses after the last day of the plan year. For plan year 2020, participants have until 3/15/21 to incur expenses. All claims must be filed no later than 90 days after plan year end. For plan year 2020, the claims must be filed by 3/31/2021. All charges must be incurred during the Plan Year. Reimbursement will not be allowed for expenses incurred from prior years or for ineligible dependents.

Vision CareEmployees who enroll in the Davis Vision benefit are eligible for a free pair of safety lenses annually AND upon request at the initial visit. (Brazers may also be eligible for tinted glasses. Please contact Corporate Benefits for additional information.)

Vision benefits are administered by Davis Vision. There is no ID card. Simply tell the provider that you are covered by Davis Vision when you make the appointment.

You may visit any doctor, but if you use an in-network doctor you pay a co-pay for most services. If you visit a doctor outside of the network, you pay the full amount at the time of the visit and submit a claim form for reimbursement of $50.

Company Provided BenefitsBasic Life InsuranceHourly employees are eligible for one times their annual salary, up to $150,000. Salaried employees are eligible for one times annual salary, up to $500,000. This benefit is available to all eligible (regular, full-time) employees, at no cost. **Beneficiaries are recommended to be over the age of 18.

Accidental Death & Dismemberment InsuranceIn the event of accidental death, your beneficiary will receive an amount equal to your basic Life benefit.

Long-Term Disability (LTD)The Company provides long-term benefits at no cost to all regular, full-time employees. If needed, LTD benefits begin after 26 weeks of disability and cover 60% of your salary up to $12,000 per month. In most cases, you may receive benefits until you recover or age 65, whichever comes first. LTD also coordinates with Social Security Disability.

Accident Insurance

This policy helps to cover you and your family for out-of-pocket expenses associated with an accidental injury, and helps to protect your savings should an accidental injury occur.

Critical Illness Insurance

This plan protects you and your family from the financial impact of cancer, a heart attack, stroke, or kidney failure. You can purchase coverage in increments of $10,000 or $20,000.

Additional Benefits401(k) Retirement Plan – Wells FargoAfter 90 days of service, full-time employees may contribute up to 50% of their salaries on a pre-tax basis up to the annual maximum of $19,500. If you are age 50 or older, you may contribute additional catch-up contributions up to the annual maxiumum of $6,500. The company matches annual contributions up to 6% or a maximum of $3,000. In order to receive a matching contribution you must be employed on December 31. After three years of service, you are 100% vested in the company-matching contribution. Employee Assistance ProgramSometimes balancing work and family activities can create stress that is hard to handle on your own. To help you through these times, you can receive confidential counseling and referrals through the EAP at no cost to you. You and your eligible dependents can use the program. No enrollment is necessary and all employees are eligible.

Counseling is available through the EAP for personal issues such as:

• Family, parenting or marital concerns/problems

• Emotional difficulties like stress, depression, anxiety, guilt or grief

• Drug and alcohol dependency

• Eating disorders like anorexia or bulimia

STEP 1After reviewing this brochure, begin your benefit enrollment by logging in to THE ZONE

STEP 2Add your dependents and beneficiaries under MY DEPENDENTS and MY BENEFICIARIES options in the benefits menu. Beneficiaries must be age 18 or over.

STEP 3Go to BENEFITS > BENEFIT CHANGES in The Zone. The Benefit Change Date is your hire date and the Benefit Change Reason will be “New Hires”. Click the button to continue.

Pre-Tax Benefits• MEDICAL & DENTAL - Select the Buy Up,

Core, or Base HDHP plan and the level of coverage

• VISION - Select the level of coverage only

• FLEX SPENDING AND HEALTH SAVINGS - Select Health and/or Dependent Care, or HSA Single or Family – You will designate the annual amount later (see Step 6).

Voluntary Benefits• Walk through each of the voluntary benefits.

Specific details on each are included in the various plan documents available under Benefit Enrollment Docs in The Zone.

STEP 4Select only those dependents to be covered on each plan. When you are done, press RECALCULATE and, if satisfied, CONTINUE.

STEP 5Designate beneficiaries for each life insurance plan selected.

STEP 6 If you elected to participate in the flex spending or the health account you will add the amounts on the savings plan worksheet.

STEP 7Confirm your benefit changes. If you need to make changes use the back button on the screen (not on your browser). You will be able to scan and upload any forms needed at this step. When you are finished with your selections, hit the SUBMIT icon (looks like the SAVE button).

You will be prompted to re-enter your password for The Zone upon submitting your elections.

If you have not been prompted to enter your password, your elections have NOT been submitted!

Making ChangesIf you have a family status change this year, you will be able to make changes to your elections. Salaried employees are eligible for short and long term disability on the first of the month following 90 days of employment. Explore reasons for enrolling in a qualifying high-deductible health plan (HDHP) paired with a health savings account (HSA).

When it’s time to select your health plan, the choice is usually between two types:

1. A traditional copay plan (PPO)

2. A qualifying high-deductible health plan coupled with a health savings account

Save your receipts.

Be careful how you use your FSA. There are rules. You will want to keep receipts from your doctors, dentists, clinic, pharmacy and hospital for all eligible health care expenses. All receipts should include the date and description of service, provider’s name and amount paid. That way, you have proof if you need to verify that all of your FSA expenses meet IRS requirements for eligibility.

Plan ahead.

Each year, you have to use all of the money in your FSA, or you will lose money left in your FSA at the end of the plan year. It’s important to think about how much you spend on health care before you decide how much to contribute. Some plans may have a grace period extension. Check your employer’s plan document to see if this applies to you.

Changing your contributions.

In special situations — such as marriage, divorce, or, if you have a baby — you may be able to adjust the amount you contribute to your FSA. This is called a qualifying change in status. If you have a change in status, your benefits representative can help you adjust your contributions. For specific details, check your employer’s plan document.

If you leave the company during the plan year, you may submit FSA claims for expenses incurred while you were still covered under the plan.

With an Optum Bank® dependent care flexible spending account (DCFSA), you can save for day care, child care, nursery school and preschool tax-free. If you are working, you are able to use your account to pay for the care of your child under the age of 13 or to care for qualifying dependent adults, like elderly parents, who can’t care for themselves.

How it works.

You can enroll in a dependent care FSA as long as you and your spouse are working, looking for work or are a full-time student.

With a dependent care FSA, you choose how much to contribute, up to a maximum of $5,000 per household, per year. Your employer deducts this amount from each paycheck, before taxes. You don’t have to pay federal, state or payroll taxes on the money credited to your account, although a couple of states do tax contributions. You save money as you lower your income taxes.

Dependent care funds are deposited every pay period.

The money you contribute to your dependent care FSA is deducted from your paychecks and deposited into your account. The total funds you contribute annually are not immediately available at the beginning of the plan year. This is an important difference between a dependent care FSA and a health care FSA.
